  • Lipid phenotyping of lung epithelial lining fluid in healthy human volunteers

    • Background: Lung epithelial lining fluid (ELF)-sampled through sputum induction-is a medium rich in cells, proteins and lipids. However, despite its key role in maintaining lung function, homeostasis and defences, the composition and biology of ELF, especially in respect of lipids, remain incompletely understood. Objectives: To characterise the induced sputum lipidome of healthy adult individuals, and to examine associations between different ELF lipid phenotypes and the demographic characteristics within the study cohort.Methods: Induced sputum samples were obtained from 41 healthy non-smoking adults, and their lipid compositions analysed using a combination of untargeted shotgun and liquid chromatography mass spectrometry methods. Topological data analysis (TDA) was used to group subjects with comparable sputum lipidomes in order to identify distinct ELF phenotypes.Results: The induced sputum lipidome was diverse, comprising a range of different molecular classes, including at least 75 glycerophospholipids, 13 sphingolipids, 5 sterol lipids and 12 neutral glycerolipids. TDA identified two distinct phenotypes differentiated by a higher total lipid content and specific enrichments of diacyl-glycerophosphocholines, -inositols and -glycerols in one group, with enrichments of sterols, glycolipids and sphingolipids in the other. Subjects presenting the lipid-rich ELF phenotype also had significantly higher BMI, but did not differ in respect of other demographic characteristics such as age or gender.Conclusions: We provide the first evidence that the ELF lipidome varies significantly between healthy individuals and propose that such differences are related to weight status, highlighting the potential impact of (over)nutrition on lung lipid metabolism.

  • The cognitive use of artifacts in cooperative process management : Rescue management and underground line control

    • Work performed in various types of centers of coordination is highly dynamic and requires that a team continuously take actions and make multiple decisions in real time. This type of Work is here refered to as cooperative process management. Through empirical studies, carried out in an underground line control room and in rescue management, this thesis analyzes how the various cognitive artifacts are used by the operator teams in order to match the particular nature of the tasks undertaken. The cognitive artifacts considered range from specially designed wall-mounted electronic boards and computer databases all the way to paper maps, refrigerator magnets, and Post-it notes. Of specialinterest is the local designs, redesigns, and customizations undertaken by the teams in order to create and maintain a mutually shared understanding of the situation at hand. Tiris thesis also examines thecognitive artifacts as coordination and conununication tools.The findings are presented in four articles. The first article focuses on the role of the public artifacts in rescue management, i.e. artifacts which are immediately available to the whole team at the same time and place. Special focus is placed on the role these artifacts play for the creation of common conceptual ground. In the second article, the organization of an underground line control room is analyzed, showing how the same cognitive artifacts play significantly different roles dUring the day andnight shift. The third article describes how cognitive artifacts, which are customized by rescue management teams, structure their work in ways which make the tasks less cognitively demanding. Finally, the fourth article goes through an unusual incident which occurred during underground line operations, in which the team lost its situation awareness. This was due to the difficulty of integrating information which was distributed over several cognitive artifacts.

    • Severe sepsis and septic shock are life threatening conditions that require immediate medical attention. Such patients must be given highest priority in the health care system in a similar way that multitrauma and myocardial infarction are given today. During the first 24 h, the condition may deteriorate and after the initial resuscitation, the patient must be observed and monitored for vital functions and matched against the treatment goals. Severe sepsis and septic shock may occur within all medical and surgical specialities. Increased awareness of how elusive this condition may appear and of the necessity for prompt care is needed among all healthcare professionals.

This chapter covers the analysis of two separate but related cases, formation of learning objectives and religious canvassing. The cases clarify the explanatory power of universals. One of them relates to a theoretical and conceptual study of assessment in higher education. The other case relates to an empirical study of the methodological reliability and validity of interview data. Results verify to the analytical “power” of action-oriented universals

  • Therapists of Nonimproved Patients Looking Back on Their Work

    • Aim: Despite clear evidence that about 5 to 10% of patients deteriorate and 35 to 40% does not improve in psychotherapy, nonimproved cases are seldom scrutinized. The therapists’ experiences of their work with nonimproved patients might be a rich source of clinically relevant knowledge. Our study explores therapists’ experiences of therapeutic process in unsuccessful cases of psychoanalytic psychotherapy with young adults. Method: A two-stage, mixed-method design was used. Nonimproved cases were identified according to the criterion of reliable and clinically significant change in self-rated symptoms. Transcripts of 16 therapist interviews (8 pretreatment and 8 at termination) were analyzed applying Grounded Theory methodology. Results: A tentative conceptual process model was constructed around the core category Having Half of the Patient in Therapy. Initially, the therapists experienced collaboration as stimulating, at the same time as the therapeutic relationship was marked by distance. At termination negative processes predominated: the patient reacted with aversion to closeness and the therapist experienced struggle and loss of control in therapy. The therapists described therapy outcome as favorable in form of increased insight and alleviated problems, while core problems remained. Discussion: This split picture was interpreted as a sign of a pseudo-process emerging when the therapist allies herself with the patient’s capable and seemingly well-functioning parts. The therapists’ experiences could be compared to the nonimproved patients’ descriptions of “spinning one’s wheels” in therapy. The therapists seem not to have succeeded in adjusting their technique to their patients’ problems, despite their attempts to meta-communicate.
