  • Cytolytic capabilities of lamina propria and intraepithelial lymphocytes in normal and chronically inflamed human intestine

    • Cell-mediated lymphocyte cytotoxicity in ileum and colon of patients with ulcerative colitis (UC), Crohn's disease (CD) and controls was investigated. Frequencies of cells expressing perforin and Fas-ligand (FasL) were determined by immunomorphometry. mRNA expression of perforin, granzyme B and FasL in T cells and subsets was assayed by reverse transcriptase-polymerase chain reaction. Cytotoxicity of intraepithelial and lamina propria lymphocytes was analysed without ex vivo activation in three functional assays: (1) anti-CD3-dependent T-cell receptor (TCR)-/CD3-mediated redirected cytotoxicity, (2) Fas-/FasL-mediated TCR-/CD3-independent cytotoxicity and (3) natural killer (NK) cell cytotoxicity. Inflammation in ileum of CD patients caused increased frequency of perforin-expressing cells and enhanced perforin-dependent TCR-/CD3-mediated cytotoxicity. In contrast, lymphocytes in the inflamed colon of UC or Crohn's colitis patients did not display this cytotoxicity nor did lymphocytes of normal colon. Normal colon lymphocytes showed spontaneous Fas-/FasL-mediated cytotoxicity. This activity was retained but not enhanced in inflamed UC colon. In contrast, a significant increase of FasL-expressing cells was seen in situ. Inflammation did not induce NK cell activity in colonic lymphocytes. Intestinal lymphocytes comprise effectors active in two different cytolytic processes. 'Classical' cytotoxic T lymphocytes in small intestine and lymphocytes executing TCR-/CD3-independent FasL-/Fas-mediated killing of unknown biological role present throughout the intestinal mucosa. Ongoing normal cytolytic processes seem to be enhanced by chronic inflammation.

  • Health care and social welfare costs in home-based and hospital-based rehabilitation after stroke

    • During the 1990s most western European and Organization of Economic Cooperation and Development (OECD) countries experienced financial difficulties and were forced to cut back on or restrain health care expenditures. Home rehabilitation has received attention in recent years because of its potential for cost containment. Often forgotten, however, is the redistribution of costs from one caregiver to another. The aim of this study was to analyse whether a redistribution of costs occurs between health care providers (the County councils) and social welfare providers (the municipalities) in a comparison of home-based rehabilitation and hospital-based rehabilitation after stroke. The study population included 123 patients, 53 in the home-based rehabilitation group and 68 in the hospital-based rehabilitation group. The patients were followed up at 6 and 12 months after onset of stroke. Resource use over a 12-month period included acute hospital care, in-hospital rehabilitation, home rehabilitation and use of home-help service as well as nursing home living. The hospital-based rehabilitation group had significantly fewer hospitalization days after a decision was made about rehabilitation at the acute care ward and consequently the cost for the acute care period was significantly lower. The cost for the rehabilitation period was significantly lower in the home-based rehabilitation group. However, the cost for home help service was significantly higher in the home-based rehabilitation group. The total costs for the care episode did not differ between the two groups. The main finding of this study is that there seems to occur a redistribution of costs between health care providers and social welfare providers in home rehabilitation after stroke in a group of patients with mixed degree of impairment.

  • Electrical and Frictional Performance of Tin-Coated Contacts Exposed to Wear and Fretting Corrosion

    • The complexity of electronic systems in vehicles is rapidly increasing, and as a consequence, the long-term reliability of automotive connectors has become an important issue. The aims of this thesis have been: 1. to characterize the friction, wear and electrical properties of tin coatings, 2. to achieve an improved fundamental understanding of the deterioration processes caused by wear and fretting corrosion, 3. to propose improvements of tin coatings systems for electrical connectors. The required insertion force has a significant impact on the design and the cost of a connector. A reduced insertion force can be achieved by reduced contact load and/or reduced tin coating thickness, but this will increase the risk of fretting corrosion. Wear during the insertion stroke is essential in order to wipe off the non-conductive oxide layer. However, a thin tin layer may become worn off after only a few insertion strokes. The rider will then partly slide on the hard intermetallic compound formed between the substrate and the residual tin. Due to the restricted wear when sliding on the intermetallic compound more oxide will be formed during sliding than is removed and consequently the electrical contact resistance will increase. Two semi-empirical models are proposed, the first describing the contact resistance when the rider plows the free tin layer, and the second describing the contact resistance increase due to oxidation when the rider slides on the intermetallic compound. Two novel methods to improve the performance of pre-tinned material were evaluated. The first involves the use of a lubricant in combination with a textured surface, to improve the supply of lubricant into the contact spot. The second involves the addition of small amounts of a preferential oxidation additive to the tin. This increases the possibility of achieving a cold-welded contact spot, which results in an ultimately stable connection.

    • c-Met is a receptor tyrosine kinase that has beenimplicated in the pathogenesis and growth of a wide variety ofhuman malignancies, including CRC, but its role in metastasisis largely unknown. We compared c-Met expression in primaryhuman colorectal carcinomas and distant metastases from thesame patients. Formalin-fixed paraffin-embedded tissuesamples from 69 colorectal cancer patients were obtained. Theprotein expression of c-Met was evaluated immunohistochemicallyusing a commercial antibody. The difference inexpression between primary tumors and their correspondingdistant metastases was analyzed using the Wilcoxon signedranktest. c-Met expression was statistically significantlylower in the distant metastases compared to their correspondingprimary tumors (p<0.001), whereas no difference was foundbetween lymph node metastases and their correspondingprimary tumors (p=0.957). The degree of c-Met expressionwas not related to clinicopathological characteristics such astumor grade and Dukes' stage at the time of primary tumordiagnosis, or to the location of the distant metastases. Wedemonstrated that c-Met expression is often reduced in distantmetastases compared to their corresponding primary colorectaltumors. Additional studies of c-Met activation and signaltransduction will increase our knowledge about the role ofc-Met in colorectal cancer metastasis.

Då boken vänder sig till läsare med olika bakgrund har enklare avsnitt brutits ur den löpande texten i basfaktarutor och svårare avsnitt i teknikrutor.
