  • Emergency department crowding and mortality : an observational multicenter study in Sweden

    • Background: Emergency department (ED) crowding is a serious problem worldwide causing decreased quality of care. It is reasonable to assume that the negative effects of crowding are at least partially due to high staff workload, but previous crowding metrics based on high workload have not been generalisable to Swedish EDs and have not been associated with increased mortality, in contrast to, e.g., occupancy rate. We recently derived and validated the modified Skåne Emergency Department Assessment of Patient Load model (mSEAL) that measures crowding based on staff workload in Swedish EDs, but its ability to identify situations with increased mortality is unclear. In this study, we aimed to investigate the association between ED crowding measured by mSEAL model, or occupancy rate, and mortality. Methods: All ED patients from 2017-01-01 to 2017-06-30 from two regional healthcare systems (Skåne and Östergötland Counties with a combined population of approximately 1.8 million) in Sweden were included. Exposure was ED- and hour-adjusted mSEAL or occupancy rate. Primary outcome was mortality within 7 days of ED arrival, with one-day and 30-day mortality as secondary outcomes. We used Cox regression hazard ratio (HR) adjusted for age, sex, arrival by ambulance, hospital admission and chief complaint. Results: We included a total of 122,893 patients with 168,900 visits to the six participating EDs. Arriving at an hour with a mSEAL score above the 95th percentile for that ED and hour of day was associated with an non-significant HR for death at 7 days of 1.04 (95% CI 0.96–1.13). For one- and 30-day mortality the HR was non-significant at 1.03 (95% CI 0.9–1.18) and 1.03 (95% CI 0.97–1.09). Similarly, occupancy rate above the 95th percentile with a HR of 1.04 (95% CI 0.9–1.19), 1.03 (95%CI 0.95–1.13) and 1.04 (95% CI 0.98–1.11) for one-, 7- and 30-day mortality, respectively. Conclusion: In this multicenter study in Sweden, ED crowding measured by mSEAL or occupancy rate was not associated with a significant increase in short-term mortality.

  • Adult attachment's unique contribution in the prediction of postpartum depressive symptoms, beyond personality traits

    • Background:Personality traits such as neuroticism can help identify pregnant women at risk of postpartum depressive symptoms (PPDS). However, it is unclear whether attachment style could have an additional contribution to this risk elevation. This study aimed to examine the overlap of adult attachment insecurity and neuroticism/trait anxiety as PPDS predictors, taking into account baseline depressive symptoms.Methods:A Swedish population-based sample of pregnant women reported on adult attachment and either neuroticism (n = 1063) or trait anxiety (n = 555). Depressive symptoms were assessed at baseline, and at six weeks and six months postpartum. Correlations between attachment and neuroticism/trait anxiety were calculated. Generalized linear models of PPDS tested the effect of attachment anxiety and avoidance, adjusting for neuroticism/trait anxiety and baseline depression. Logistic regression models with combined high attachment anxiety and-neuroticism/trait anxiety visualized their value as risk factors beyond antenatal depression.Results:Attachment and neuroticism/trait anxiety were highly correlated (r = .55.77). Attachment anxiety exerted a partially independent effect on PPDS at six weeks (p < .05) and at six months (p < .05) adjusting for neuroticism. Among antenatally non-depressed, combined high attachment anxiety and high neuroticism or trait anxiety was predictive of PPDS at both assessment points. Limitations: Low acceptance rate, exclusive use of self-reports.Conclusions:Beyond personality, attachment anxiety had a small independent effect on the risk of PPDS. Combining items of adult attachment and neuroticism/trait anxiety could prove useful in antenatal screening for high risk of PPDS.

    • The present article is a translation—to Farsi—of an article by Dr. Ali Mirchi (postdoctoral research associate at the Department of Civil and Environmental Engineering, Michigan Technological University), Dr. Kaveh Madani (lecturer in Environmental Management at the Centre for Environmental Policy, Imperial College London) and Dr. Amir Aghakouchak (assistant professor at the Department of Civil and Environmental Engineering, University of California, Irvine), entitled "Lake Urmia: how Iran’s most famous lake is disappearing", published in The Guardian 23 January 2015. The article discusses how Lake Urmia, once the largest salt-water lake in the Middle East and twice as large as Luxembourg, is now disappearing. This study is conducted by an independent group of 10 concerned Iranian researchers at universities and research centers in US, UK and Canada. Due to the unavailability of reliable and consistent ground-truth data, they used NASA's high-resolution satellite observations to estimate the physiographic changes of the lake over the past four decades. The results show that the lake’s surface area, in September 2014, was about 12% of its average size in the 1970s. They discuss how and why Lake Urmia’s desiccation requires active involvement of local, national and international organisations and urge action.

    • The rotary motor enzyme FoF1-ATP synthase uses the protonmotive force across a membrane to synthesize ATP from ADP and P-i (H2PO4-) under cellular conditions that favor the hydrolysis reaction by a factor of 2 x 10(5). This remarkable ability to drive a reaction away from equilibrium by harnessing an external force differentiates it from an ordinary enzyme, which increases the rate of reaction without shifting the equilibrium. Hydrolysis takes place in the neighborhood of one conformation of the catalytic moiety F-1-ATPase, whose structure is known from crystallography. By use of molecular dynamics simulations we trap a second structure, which is rotated by 40 degrees from the catalytic dwell conformation and represents the state associated with ATP binding, in accord with single-molecule experiments. Using the two structures, we show why Pi is not released immediately after ATP hydrolysis, but only after a subsequent 120 degrees rotation, in agreement with experiment. A concerted conformational change of the alpha(3)beta(3) crown is shown to induce the 40 degrees rotation of the gamma-subunit only when the beta(E) subunit is empty, whereas with Pi bound, beta(E) serves as a latch to prevent the rotation of gamma. The present results provide a rationalization of how F-1-ATPase achieves the coupling between the small changes in the active site of beta(DP) and the 40 degrees rotation of gamma.

    • Consider a robot to measure or operate on man made objects randomly located in the workspace. The optronic sensing onboard the robot are a scanning range measuring time-of-flight laser and a CCD camera. The plane surfaces are modeled and parameters extracted using the Radon/Hough transform. This extraction is very robust and motion is also included in a natural way. This paper gives additional results for range discontinuities. A multiple model framework for fusion of sensor information from laser and camera using parametric models of planar and cylindrical surfaces is suggested. An important issue is the mutual consistency between the motion, the range discontinuing, occlusion and properties of the sensor combination. Typical applications are; Robust features for use during navigation in cluttered areas. Model for verification and updating of CAD-models when navigating inside buildings and industrial plants. Accumulating sensor readings into a map during operation of a telecommanded robot.

    • Organizations build information systems to support their business processes. Precise modeling of an organization's processes is a prerequisite for building information systems that support those processes. Our goal is to help business analysts produce detailed models of the business processes that best reflect the needs of their organizations. To this end, we propose to a) leverage the best practices in terms of a kernel of generic business processes, and b) provide analysts with tools to customize those processes by generating new process variants. We use business patterns from the Resource Event Agent ontology to identify variation points, and to codify the transformations inherent in the generation of the process variants. We developed a prototype process specialization tool using the Eclipse modeling ecosystem. We tested our approach on a set of processes from the Enterprise Resource Planning literature, and a set of variation points to assess the extent to which: 1) the identified variation points made sense, and 2) whether the generated variants made sense, from a business point of view. The results showed that 94.12% of the variation points made sense, and that 80.6% of the generated process variants corresponded to what the business process management specialists expected.
