  • Re-treatment with radium-223 : first experience from an international, open-label, phase I/II study in patients with castration-resistant prostate cancer and bone metastases

    • Background: Six radium-223 injections at 4-week intervals is indicated for patients with castration-resistant prostate cancer and symptomatic bone metastases. However, patients usually develop disease progression after initial treatment. This prospective phase I/II study assessed re-treatment safety and efficacy of up to six additional radium-223 injections. Patients and methods: Patients had castration-resistant prostate cancer and bone metastases and six initial radium-223 injections with no on-treatment bone progression; all had subsequent radiologic or clinical progression. Concomitant agents were allowed at investigator discretion, excluding chemotherapy and initiation of new abiraterone or enzalutamide. The primary endpoint was safety; additional exploratory endpoints included time to radiographic bone progression, time to total alkaline phosphatase and prostate-specific antigen progression, radiographic progression-free survival, overall survival, time to first symptomatic skeletal event (SSE), SSE-free survival, and time to pain progression. Results: Among 44 patients, 29 (66%) received all six re-treatment injections. Median time from end of initial radium-223 treatment was 6 months. Forty-one (93%) reported >= 1 treatment-emergent adverse event. No grade 4-5 hematologic treatment-emergent adverse events occurred. Only one (2%) patient had radiographic bone progression; eight (18%) had radiographic soft tissue tumor progression (three lymph node and five visceral metastases). Median times to total alkaline phosphatase and prostate-specific antigen progression were not reached and 2.2 months, respectively. Median radiographic progression-free survival was 9.9 months (12.8-month maximum follow-up). Five (11%) patients died and eight (18%) experienced first SSEs. Median overall survival, time to first SSE, and SSE-free survival were not reached. Five (14%) of 36 evaluable patients (baseline worst pain score <= 7) had pain progression. After 2 years of follow-up, 28 (64%) patients died, and the median overall survival was 24.4 months. Conclusions: Re-treatment with a second course of six radium-223 injections after disease progression is well tolerated, with minimal hematologic toxicity and low radiographic bone progression rates in this small study with limited follow-up. Favorable safety and early effects on disease progression indicate that radium-223 re-treatment is feasible and warrants further evaluation in larger prospective trials.

  • Stråkvis inventering av Vänerns strandvegetation : Övervakningssystem för framtida kontroll av igenväxning och vegetationsförändringar

    • Föreligande arbete utgör basen i ett kontrollprogram för att följa vegetationsutvecklingeni Vänerns strandnära områden. Tanken är att återinventeringar skall ske regelbundet medca 5 års intervall.Inventeringen har inriktats på växter av igenväxningskaraktär, d v s vedartade växter samtvass, säv och flytbladsväxter. Vegetationen har inventerats i stråk, 1 m breda och med enlängd varierande mellan ca 30 m och 150 m. Totalt har 61 stråk fördelat på 25 områdeninventerats. Områdena har valts ut i IR-flygbilder och med tanke på att de skall varalättillgängliga samt att mänsklig påverkan inte skall vara allt för stor. Områdena är avskiftande typ och exponeringsgrad. Stråkens placering och riktning inom de olika områdenaär markerade med i marken nedslagna kopparrör. Vidare är placering och riktning inmättamed GPS-mottagare i Rikets nät RT 90 och dessutom finns stråken beskrivna i bildoch text. Vid stråkinventeringen har förekommande vegetation noterats liksom dess utbredningoch avstånd från startpunkten.

    • Preterm birth and associated brain injury are the primary cause of cerebral palsy and developmental disabilities and are among the most serious global health issues that modern society faces. Current therapy for infants suffering from premature brain injury is still mainly supportive, and there are no effective treatments. Thus there is a pressing need for comparative and translational studies on how to reduce brain injury and to increase regeneration and brain repair in preterm infants. There is strong supporting evidence for the use of umbilical cord blood (UCB)-derived stem cell therapy for treating preterm brain injury and neurological sequelae. UCB-derived stem cell therapy is effective in many animal models and has been shown to be feasible in clinical trials. Most of these therapies are still experimental, however. In this review, we focus on recent advances on the efficacy of UCB-derived stem cell therapy in preterm infants with brain injury, and discuss the potential mechanisms behind their therapeutic effects as well as application strategies for future preclinical and clinical trials.

  • Impact of nurses taking daily work breaks in a hospital garden on Burnout

    • Background Nurses working in hospital environments are at risk for burnout. Exposure to nature has psychological benefits, but the effect of hospital gardens on nurse burnout is less understood. Objective To compare the effect on nurse burnout of taking daily work breaks in a hospital-integrated garden with the effect of indoor-only breaks. Methods A prospective crossover trial was conducted of nurses assigned to either 6 weeks of a work break in an outdoor hospital garden or 6 weeks of indoor-only breaks. After a 1-week washout period, break assignments were switched for a subsequent 6 weeks. The Maslach Burnout Inventory was administered at the beginning and end of each 6-week period, and a Present Functioning Visual Analogue Scale was completed at the start and end of each break to capture immediate psychological symptoms. Change scores were analyzed by using generalized estimating equations. Results For 29 nurses, for garden compared with indoor breaks, significant improvement was apparent in scores on the Maslach Burnout Inventory subscales for emotional exhaustion (4.5 vs -0.2; P < .001) and depersonalization (1.8 vs 0.0; P = .02) but not for personal accomplishment (-0.6 vs -0.0; P = .55). Compared with indoor breaks, total symptom scores on the Present Functioning Visual Analog Scale improved significantly when nurses took a break in the garden (garden vs indoor breaks, 4.0 vs 2.4; P = .04). Conclusions Taking daily work breaks in an outdoor garden may be beneficial in mitigating burnout for nurses working in hospital environments.

    • The Swedish welfare state has been subject to a substantial re-organization in recent decades, not always recognized in the international literature. Almost every area of social policy have changed, often in the downward direction and with potential far-reaching consequences for the social sustainability of the Swedish welfare state. In this research note, I discuss significant changes to Swedish social policy and place the reorganization of the Swedish welfare state in an international perspective. Focus is on old-age pensions and unemployment benefits. Both areas are characterized by significant changes.

    • Recently, to reach zero emission levels, some new energy systems based on fuel cells have been developed. This paper presents a review of investigations of these systems by thermodynamics performance, mainly, energy and exergy analysis. With energy analysis, the performance of an energy conversion system cannot be effectively and accurately evaluated. But exergy analysis complements and reinforces energy analysis. Using exergy analysis in relation to the degree of irreversibility in each section is a quality method and can be useful in fuel cell analysis. Analysis based on the first and second laws of thermodynamics seems useful in most cases and here reviews for low-temperature and high-temperature fuel cells are given. As a result, regardless of the type of the FC-system and its application, there is always a need of thermo-economic analysis to investigate the effect of various parameters on the system performance, such as different fuel sources, different operating conditions, different subsystems and energy sources to combine with the fuel cell.
