    • Past research in population health trends has shown that injuries form a substantial burden of population health loss. Regular updates to injury burden assessments are critical. We report Global Burden of Disease (GBD) 2017 Study estimates on morbidity and mortality for all injuries.MethodsWe reviewed results for injuries from the GBD 2017 study. GBD 2017 measured injury-specific mortality and years of life lost (YLLs) using the Cause of Death Ensemble model. To measure non-fatal injuries, GBD 2017 modelled injury-specific incidence and converted this to prevalence and years lived with disability (YLDs). YLLs and YLDs were summed to calculate disability-adjusted life years (DALYs).FindingsIn 1990, there were 4 260 493 (4 085 700 to 4 396 138) injury deaths, which increased to 4 484 722 (4 332 010 to 4 585 554) deaths in 2017, while age-standardised mortality decreased from 1079 (1073 to 1086) to 738 (730 to 745) per 100 000. In 1990, there were 354 064 302 (95% uncertainty interval: 338 174 876 to 371 610 802) new cases of injury globally, which increased to 520 710 288 (493 430 247 to 547 988 635) new cases in 2017. During this time, age-standardised incidence decreased non-significantly from 6824 (6534 to 7147) to 6763 (6412 to 7118) per 100 000. Between 1990 and 2017, age-standardised DALYs decreased from 4947 (4655 to 5233) per 100 000 to 3267 (3058 to 3505).InterpretationInjuries are an important cause of health loss globally, though mortality has declined between 1990 and 2017. Future research in injury burden should focus on prevention in high-burden populations, improving data collection and ensuring access to medical care.

    • This study provides an overview of the influence of occupational risk factors on the global burden of disease as estimated by the occupational component of the Global Burden of Disease (GBD) 2016 study.MethodsThe GBD 2016 study estimated the burden in terms of deaths and disability-adjusted life years (DALYs) arising from the effects of occupational risk factors (carcinogens; asthmagens; particulate matter, gases and fumes (PMGF); secondhand smoke (SHS); noise; ergonomic risk factors for low back pain; risk factors for injury). A population attributable fraction (PAF) approach was used for most risk factors.ResultsIn 2016, globally, an estimated 1.53 (95% uncertainty interval 1.39–1.68) million deaths and 76.1 (66.3–86.3) million DALYs were attributable to the included occupational risk factors, accounting for 2.8% of deaths and 3.2% of DALYs from all causes. Most deaths were attributable to PMGF, carcinogens (particularly asbestos), injury risk factors and SHS. Most DALYs were attributable to injury risk factors and ergonomic exposures. Men and persons 55 years or older were most affected. PAFs ranged from 26.8% for low back pain from ergonomic risk factors and 19.6% for hearing loss from noise to 3.4% for carcinogens. DALYs per capita were highest in Oceania, Southeast Asia and Central sub-Saharan Africa. On a per capita basis, between 1990 and 2016 there was an overall decrease of about 31% in deaths and 25% in DALYs.ConclusionsOccupational exposures continue to cause an important health burden worldwide, justifying the need for ongoing prevention and control initiatives.

  • Patterns of change of multisite pain over 1 year of follow-up and related risk factors

    • Background Multisite musculoskeletal pain is common and disabling. This study aimed to prospectively investigate the distribution of musculoskeletal pain anatomically, and explore risk factors for increases/reductions in the number of painful sites. Methods Using data from participants working in 45 occupational groups in 18 countries, we explored changes in reporting pain at 10 anatomical sites on two occasions 14 months apart. We used descriptive statistics to explore consistency over time in the number of painful sites, and their anatomical distribution. Baseline risk factors for increases/reductions by >= 3 painful sites were explored by random intercept logistic regression that adjusted for baseline number of painful sites. Results Among 8927 workers, only 20% reported no pain at either time point, and 16% reported >= 3 painful sites both times. After 14 months, the anatomical distribution of pain often changed but there was only an average increase of 0.17 painful sites. Some 14% workers reported a change in painful sites by >= 3. Risk factors for an increase of >= 3 painful sites included female sex, lower educational attainment, having a physically demanding job and adverse beliefs about the work-relatedness of musculoskeletal pain. Also predictives were as follows: older age, somatizing tendency and poorer mental health (each of which was also associated with lower odds of reductions of >= 3 painful sites). Conclusions Longitudinally, the number of reported painful sites was relatively stable but the anatomical distribution varied considerably. These findings suggest an important role for central pain sensitization mechanisms, rather than localized risk factors, among working adults. Significance Our findings indicate that within individuals, the number of painful sites is fairly constant over time, but the anatomical distribution varies, supporting the theory that among people at work, musculoskeletal pain is driven more by factors that predispose to experiencing or reporting pain rather than by localized stressors specific to only one or two anatomical sites.

  • Hydrogen production from landfill biogas: Profitability analysis of a real case study

    • Hydrogen is not only considered as a cornerstone within renewable energy portfolio but it is also a key enabler for CO2 valorisation being a central resource for industrial decarbonization. This work evaluates the profitability of hydrogen production via combined biogas reforming and water–gas shift reaction, based on a real case scenario for landfill biogas plant in Seville (Spain). A techno-economic model was developed based on a process model and the discounted cash-flow method. A biogas flow of 700 m3/h (input given by the landfill biogas plant) was used as plant size and the analysis was carried out for two different cases: (1) use of already available energy sources at the industrial plant, and (2) solar energy generation to power the process. The economic outputs obtained showed that under the current circumstances, this hydrogen production route is not profitable. The main reason is the relatively low current hydrogen prices which comes from fossil fuels. A revenues analysis indicates that hydrogen from biogas selling prices between 2.9 and 5.7 €/kg would be needed to reach profitability, which are considerably higher than the current hydrogen cost (1.7 €/kg). A subsidy scheme is suggested to improve the competitiveness of this hydrogen production process in the short-medium term. A cost analysis is also performed, revealing that electricity prices and investment costs have a high impact on the total share (23–40% and 8–22%, respectively). Other potential costs reduction such as catalyst, labour and manteinance & overhead are also evaluated, showing that cutting-down production costs is mandatory to unlock the potential of hydrogen generation from biogas. Our work showcases the techno-economic challenge that green energy policies face in the path toward sustainable societies.
