  • Links between blood pressure and life-style factors reported via a mobile phone-based self-management support system

    • Objective: To explore relationships between patients’ self-monitoring of blood pressure and their concurrent self-reports of medication intake, wellbeing stress, physical activity and symptoms.Design and method: This was a prospective study exploring the eight-week effectiveness of a mobile phone based self-management support system for patients with hypertension. 50 patients undergoing treatment for hypertension, from four primary health care centers situated in urban and suburban communities in Sweden, self-reported through the system once daily during eight weeks.Scientific data: Associations between systolic and diastolic blood pressure and 10 self-report lifestyle-related variables.Results: The single strongest association was found between medication intake and systolic blood pressure, where failure to take medications was associated with an estimated 7.44 mmHg higher systolic blood pressure. To a lesser degree, medication intake was also associated with diastolic blood pressure. Wellbeing and stress were consistently associated with systolic blood pressure and diastolic blood pressure, whereas physical activity was associated with only systolic blood pressure. None of the symptoms dizziness, headache, restlessness, fatigue or palpitations were significantly associated with blood pressure.Conclusions: Blood pressure was associated with patients’ blood pressure management behaviors, eg drug intake and experiences of wellbeing and stress. No association was found between blood pressure and side effects. Enabling persons with hypertension to monitor and track their BP in relation to medication intake, symptoms and life-style variables may be a fruitful way to help them gain first-hand understanding of the importance of adherence and persistence to treatment recommendations.

    • Objectives To explore relationships between patients’ self-monitoring of blood pressure (BP) and their concurrent self-reports of medication intake, well-being, stress, physical activity and symptoms. Design This study is a secondary analysis of a prospective study exploring the 8-week effectiveness of a mobile phone-based self-management support system for patients with hypertension. Setting Four primary healthcare centres situated in urban and suburban communities in Sweden. Participants 50 patients undergoing treatment for hypertension. Primary and secondary outcome measures Associations between systolic (SBP) and diastolic blood pressure (DBP) and 10 self-report lifestyle-related variables were analysed using linear mixed effects modelling. Results Medication intake, better well-being, less stress and greater physical activity were associated variously with lower same-day SBP and DBP. The single strongest association was found between medication intake and SBP, where failure to take medications was associated with an estimated 7.44mm Hg higher SBP. To a lesser degree, medication intake was also associated with DBP, where DBP was 4.70mm Hg higher in cases where medications were not taken. Well-being and stress were consistently associated with SBP and DBP, whereas physical activity was associated with only SBP. None of the symptoms—dizziness, headache, restlessness, fatigue or palpitations—were significantly associated with BP. Conclusions Our findings that BP was associated with patients’ BP management behaviours and experiences of well-being and stress, but not symptoms suggest that enabling persons with hypertension to monitor and track their BP in relation to medication intake, physical activity, well-being, stress and symptoms may be a fruitful way to help them gain first-hand understanding of the importance of adherence and persistence to treatment recommendations.   • Flytande inflytande : Affektiva relationer mellan barn och miljön i förskolan

    • The purpose of this doctoral thesis is to develop more understandings concerning children’s influence in preschool. By adopting an immanent approach the study centers on young children’s affective relations with the milieu in preschool.The empirical material was produced through approximately 30 hours of observations by video camera and field notes in a preschool department with children aged one to three years. Focus of the observations has been to follow the children in their everyday lives in preschool.The result shows three forms of influence characterized by four qualities. One quality is that influence is understood as collective. This means that the children’s relations always involves other actors, both human and non-human which produces an understanding of influence as fluent in the relations between different actors. Children’s relations also emerge through active affects with a creative quality to produce difference and alternatives. This produces an understanding of a child-influence. The result shows how the active intensity in child-influence also sometimes involves the pedagogues. Other affects that has emerged between the children and the milieu in preschool are the passive ones. These affects decreases the intensity and are characterized by acting as usual and according to routines. The pedagogues’ responses to the children’s relations are characterized by both a standardized response that points to a territorialization of earlier routines and the recognizable, and an affective attunement which emerge as a quality where the pedagogues’ tune in to the children’s affective relations. The affective attunement becomes a possibility for a togetherness-influence.The study concludes that instead of arranging certain situations where children verbally can express their opinions and take part in decision-making the influence of young children can be based on their bodily and affective relations with the milieu in preschool.

    • Our aim was to assess causes of death and temporal changes in excess mortality among patients with new-onset type 2 diabetes in Skaraborg, Sweden.Patients from the Skaraborg Diabetes Register with prospectively registered new-onset type 2 diabetes 1991-2004 were included. Five individual controls matched for sex, age, geographical area and calendar year of study entry were selected using population records. Causes of deaths until 31 December 2014 were retrieved from the Cause of Death Register. Adjusted excess mortality among patients and temporal changes of excess mortality were calculated using Poisson models. Cumulative incidences of cause-specific mortality were calculated by competing risk regression.During 24years of follow-up 4364 deaths occurred among 7461 patients in 90,529 person-years (48.2/1000 person-years, 95% CI 46.8-49.7), and 18,541 deaths in 479,428 person-years among 37,271 controls (38.7/1000 person-years, 38.1-39.2). The overall adjusted mortality hazard ratio was 1.47 (p<.0001) among patients diagnosed at study start 1991 and decreased by 2% (p<.0001) per increase in calendar year of diagnosis until 2004. Excess mortality was mainly attributed to endocrine and cardiovascular cause of death with crude subdistributional hazard ratios of 5.06 (p<.001) and 1.22 (p<.001).Excess mortality for patients with new-onset type 2 diabetes was mainly attributed to deaths related to diabetes and the cardiovascular system, and decreased with increasing year of diagnosis 1991-2004. Possible explanations could be temporal trends of earlier diagnosis due to lowered diagnostic thresholds and intensified diagnostic activities, as well as improved treatment.

  • ADAMTS-7 is associated with a high-risk plaque phenotype in human atherosclerosis

    • Several large-scale genome-wide association studies have identified single-nucleotide polymorphisms in the genomic region of A Disintegrin And Metalloproteinase with ThromboSpondin type 1 repeats (ADAMTS)-7 and associations to coronary artery disease. Experimental studies have provided evidence for a functional role of ADAMTS-7 in both injury-induced vascular neointima formation and development of atherosclerotic lesions. However, whether ADAMTS-7 is associated with a specific plaque phenotype in humans has not been investigated. Carotid plaques (n = 206) from patients with and without cerebrovascular symptoms were analyzed for expression of ADAMTS-7 by immunohistochemistry and correlated to components associated with plaque vulnerability. Plaques from symptomatic patients showed increased levels of ADAMTS-7 compared with lesions from asymptomatic patients. High levels of ADAMTS-7 correlated with high levels of CD68-staining and lipid content, but with low smooth muscle cell and collagen content, which together are characteristics of a vulnerable plaque phenotype. ADAMTS-7 levels above median were associated with increased risk for postoperative cardiovascular events. Our data show that ADAMTS-7 is associated with a vulnerable plaque phenotype in human carotid lesions. These data support previous observations of a potential proatherogenic role of ADAMTS-7.

    • PRELP is a member of the leucine-rich repeat family of extracellular matrix proteins in connective tissue. In contrast to other members of the family, the amino-terminal domain of PRELP has a high content of proline and positively charged amino acids. This domain has previously been shown to bind chondrocytes and to inhibit osteoclast differentiation. Here we show that PRELP mediates cell adhesion by binding to cell surface glycosaminoglycans. Thus, rat skin fibroblasts bound to full-length PRELP and to the amino-terminal part of PRELP alone, but not to truncated PRELP lacking the positively charged amino-terminal region. Cell attachment to PRELP was inhibited by addition of soluble heparin or heparan sulfate, by blocking sulfation of the fibroblasts, or by treating the cells with a combination of chondroitinase and heparinase. Using affinity chromatography, we identified syndecan-1, syndecan-4 and glypican-1 as cell surface proteoglycans binding to the amino-terminal part of PRELP. Finally, we show that the amino-terminal domain of PRELP in combination with the integrin-binding domain of fibronectin, but neither of the fragments alone, induced fibroblast focal adhesion formation. These findings provide support for a role of the amino-terminal region of PRELP as an important regulator of cell adhesion and behavior, which may be of importance in pathological conditions.
