  • The effect of statins on mortality and cardiovascular disease in primary care hypertensive patients without other cardiovascular disease or diabetes.

    • Studies in primary health care (PHC) assessing the effect of primary prevention with statins on mortality and cardiovascular disease (CVD) are scarce. This study aimed to estimate the effect of statins on all-cause mortality, cardiovascular mortality, myocardial infarction (MI) and stroke in individuals in PHC with hypertension without CVD or diabetes.Using the Swedish PHC quality assurance register QregPV, the study included 13 193 individuals with hypertension without CVD or diabetes, who had filled a first statin prescription between 2010 and 2016, and 13 193 matched controls without a filled statin prescription at index date. Controls were matched on sex and propensity score using clinical data and data from national registers on co-morbidities, prescriptions, and socioeconomic status. The effect of statins was estimated in Cox regression models.During a median of 4.2 years of follow-up, 395 individuals in the statin group versus 475 in the control group died, 197 versus 232 died of cardiovascular disease, 171 versus 191 had a MI, and 161 versus 181 had a stroke. The treatment effect of statins was significant for all-cause mortality (HR 0.83, 95% confidence interval [CI] 0.74-0.93) and cardiovascular mortality (HR 0.85, 95% CI 0.72-0.998). Overall, no significant treatment effect of statins was seen for MI (HR 0.89, 95% CI 0.74-1.07), but there was a significant interaction with sex (p=0.008) with decreased risk of MI for women but not for men (HR 0.66, 95% CI 0.49-0.88 versus HR 1.09, 95% CI 0.86-1.38).Primary prevention with statins in PHC was associated with reduced risk of all-cause mortality, cardiovascular mortality, and in women, lower risk of MI.

  • Comparison of contrast-to-noise ratios of different detection methods in ultrasound optical tomography

    • Ultrasound optical tomography (UOT) is a hybrid imaging modality based on interaction between ultrasound and light, with a potential to extend optical imaging capabilities in biological tissues to depths of several centimeters. Several methods have been developed to detect the UOT signal. To better understand their potential for deep tissue imaging, we present a theoretical contrast-to-noise comparison between the spectral hole burning, single-shot off-axis holography, speckle contrast, and photorefractive detection methods for UOT. Our results indicate that spectral hole burning filters have the potential to reach the largest imaging depths. We find that digital off-axis holography and photorefractive detection can have good contrast-to-noise ratio at significant depths. The speckle contrast method has a smaller penetration depth comparatively.

    • Diffusion kinetics and structural properties of chemically strengthened titania-doped soda-lime silicate glasses were studied by depth-resolved X-ray photoelectron spectroscopy, Raman spectroscopy and spectrophotometry. The glasses were ion exchanged, whereby Na+ in the glass was replaced by K+ in a molten salt bath, at four different treatment temperatures between 350 and 500 °C. The alkali diffusion coefficient, DK-Na, and corresponding activation energy were calculated to be between 3.26×10−12 and 4.47×10−11 cm2s−1 and between 101.1 kJmol−1 and 105.6 kJmol−1, respectively. DK-Na was observed to decrease as the TiO2 concentration was increased. Raman analysis showed Q3-silicate species with different bond lengths, which was attributed to surface compressive stresses, and increasing Si-O-Si bond angle with increasing ion exchange temperature. Ti3+ ions exist as a minor species in the glasses and its concentration depends on the TiO2 content. Deconvolution of the optical absorption spectra reveals Jahn-Teller compressive distortion of the Ti3+ octahedral coordination.

    • Diffusion kinetics and structural properties of chemically strengthened titania-doped soda-lime silicate glasses were studied by depth-resolved X-ray photoelectron spectroscopy, Raman spectroscopy and spectrophotometry.Chemical strengthening (CS) is frequently used to strengthen thin glasses. CS of glass is based on ion exchange of larger ions from a molten salt into glass. Both the ion and counter ion are conventionally monovalent alkali ions.Diffusion kinetics and structural properties of chemically strengthened titania-doped (TiO2) soda-lime silicate (SLS) glasses were studied by depth-resolved X-ray photoelectron spectroscopy, Raman spectroscopy and spectrophotometry. The glasses were ion exchanged, whereby Na+ in the glass was replaced by K+ in a molten salt bath, at four different treatment temperatures between 350 and 500 °C.The following samples were prepared and analyzed by X-ray Photoelectron Spectroscopy (XPS): (1) SLS, (2) 4.7% TiO2, and (3) 9.9% TiO2. The ion exchange procedure was performed for 5 h at four different temperatures below Tg (350, 400, 450 and 500 °C). Before XPS measurements, the samples were wet-etched using hydrofluoric (HF) acid to produce samples with six different etching depths.The Raman scattered light was detected in the backscattering configuration employing linear polarization and 2400 lines/mm grating, and a 100x objective lens. Depth profile spectra were collected at six different depths of 0, 10, 20, 30, 40, and 50 µm for each glass sample, employing 12 scans with a 10 s exposure time for each scan.Spectrophotometric measurements were conducted before and after K+/Na+ ion-exchange treatmeatment for 5 h at 500 °C, collected between 300 and 2500 nm.

    • Scholars have theorized that congenital health endowment is an important determinant of economic outcomes later in a person's life. Field, Robles, and Torero (2009, American Economic Journal: Applied Economics 1, 140-169) find large increases in educational attainment caused by a reduction of fetal iodine deficiency following a set of iodine supplementation programs in Tanzania. We revisit the Tanzanian iodine programs with a narrow and wide replication of the study by Field et al. We are able to exactly replicate the original results. We find, however, that the findings are sensitive to alternative specification choices and sample restrictions. We try to address some of these concerns in the wide replication; we increase the sample size fourfold, and we improve the precision of the treatment variable by incorporating new institutional and medical insights. Despite the improvements, no effect is found. We conclude that the available data do not provide sufficient power to detect a possible effect, as treatment assignment cannot be measured with sufficient precision.

    • This paper seeks to reclaim irony as more than a way of humorously pointing out that the times we live in are out of joint or coming to an end, instead emphasizing its potential as a productive force and method in both educational thinking and teaching practice. By interrogating the educative potential of irony as method and humorous experience in the Bildung-oriented Didaktik tradition, we argue that irony can reveal the tensions and dilemmas of didactic posturing in order to facilitate a productive decentring and self-alienating critique of our anthropocentric predicament in education. It can be difficult to tease out the inherent distance between educational content and its substance in times where educators are caught between crowded curricula and strict testing regimes, but this irony is rarely lost on either pupils or teachers. Irony, we argue, is more than a constant fissure in the efforts to streamline ideas about what constitutes the most efficient didactic approaches. Instead, it could be used as a method to make educative processes attentive to what didactic posturing might be missing and how self-alienation might be a premise of education as formation of the self.

    • This paper analyses how ‘criticality’ is negotiated in the global policy frameworks on Education for Sustainable Development (ESD) and re-conceptualised in Vietnamese ESD policymaking. Taking the context of Viet Nam, this paper reflects on what constitutes criticality in education in the light of cultural and historical contexts of the education systems. The comparative perspective helps explore whether (1) universal or decontextualised ‘criticality’ exists or (2) whether ‘criticality’ is culturally negotiated based on the premise that educational imaginaries of societal formation and transformation are historically and contextually embedded and contingent. In addition, this paper connects the ongoing debate on the critical potential of ESD within the field of environmental education (EE) research to comparative education research by highlighting both what a comparative perspective might offer to EE research and what recent developments in EE research might contribute to comparative education research.

    • This article draws on the emerging speculative realist philosophical movement in order to develop new understandings of the issues and content of education that needs framing and reframing within environmental and sustainability education (ESE) research. We argue for the potential of using speculative realist concepts such as correlationism, hyperobjects, the strange stranger, undermining, overmining and duomining in order to develop a dark pedagogy that could draw on insights from speculative realism and object-oriented ontology in particular in order to develop further key concepts and discussions within ESE. Based on inspiration from speculative realism, our conception of a dark pedagogy could strive to: develop an understanding of the hyperobjects and the strangeness of objects of education at the center of ESE. Our conception of dark pedagogy insists on the inherent withdrawnness of the objects at play in ESE, and envisions to develop a sensibility towards eco-, geo-, infra- and chrono-factors in ESE and make explicit the potential educational implications of speculative realisms.
