Abdominal obesity, weight gain during adulthood and risk of liver and biliary tract cancer in a European cohort

- General obesity has been positively associated with risk of liver and probably with biliary tract cancer, but little is known about abdominal obesity or weight gain during adulthood. We used multivariable Cox proportional hazard models to investigate associations between weight, body mass index, waist and hip circumference, waist-to-hip and waist-to-height ratio (WHtR), weight change during adulthood and risk of hepatocellular carcinoma (HCC), intrahepatic (IBDC) and extrahepatic bile duct system cancer [EBDSC including gallbladder cancer (GBC)] among 359,525 men and women in the European Prospective Investigation into Cancer and Nutrition study. Hepatitis B and C virus status was measured in a nested casecontrol subset. During a mean follow-up of 8.6 years, 177 cases of HCC, 58 cases of IBDC and 210 cases of EBDSC, including 76 cases of GBC, occurred. All anthropometric measures were positively associated with risk of HCC and GBC. WHtR showed the strongest association with HCC [relative risk (RR) comparing extreme tertiles 3.51, 95% confidence interval (95% CI): 2.095.87; ptrend < 0.0001] and with GBC (RR: 1.56, 95% CI: 1.122.16 for an increment of one unit in WHtR). Weight gain during adulthood was also positively associated with HCC when comparing extreme tertiles (RR: 2.48, 95% CI: 1.494.13; <0.001). No statistically significant association was observed between obesity and risk of IBDC and EBDSC. Our results provide evidence of an association between obesity, particularly abdominal obesity, and risk of HCC and GBC. Our findings support public health recommendations to reduce the prevalence of obesity and weight gain in adulthood for HCC and GBC prevention in Western populations.

- Objective The purpose of this study was to explore licensed practical nurses (LPNs) views of being exposed to secondhand smoke in municipal home nursing care. Design and sample Using a qualitative approach, data were collected through three focus group interviews with 15 LPNs, smokers, and nonsmokers. Qualitative content analysis was used to interpret the interview text. Results Exposure to secondhand smoke during working hours, which presents a risk to the health and wellbeing of the LPNs, is a consequence of placing the interests of the smoking care recipients first and the employers reluctance to take action on behalf of the LPNs. These factors prompted LPNs to propose solutions aimed at resolving a major dilemma in routine home nursing care. Conclusions A combination of actions, particularly by managers, is required to improve the working environment for LPNs while concurrently respecting the care recipients right to take decisions in their own homes.

- Abstract BACKGROUND: Several studies have demonstrated an association between obesity and asthma. However, it is uncertain if fraction of exhaled nitric oxide (FENO), which is used as a marker of airway inflammation, and atopy are associated with BMI. The aim was to examine if obese with asthma symptoms have a different phenotype of asthma than non-obese as indicated by FENO. METHODS: The subjects (n=2187) consist of women and men, aged 25-74, living in Gothenburg, Sweden, participating in the randomly selected INTERGENE study cohort. Measurements include anthropometric measures, bioelectric impedance, FENO, pulmonary function, blood samples for IgE and questionnaires including items on respiratory symptoms. Obesity was defined as BMI≥30 kg/m(2). In this cross-sectional analysis, general linear models were used to analyse how FENO was associated with anthropometry, body composition, wheezing and atopy. RESULTS: In non-obese subjects, wheezing was associated with raised FENO and atopy, whereas, in contrast, obese with wheezing had lower FENO than obese without wheezing (16.1 v.s. 19.1 ppb, p<0.01). The prevalence of atopy was similar in both those sub-groups (25.0 v.s. 20.7%, p=0.4). Similarly, in 395 subjects (19%) who reported wheezing, FENO was negatively associated with BMI, waist-hip ratio and percentage of body fat, while no significant relationships were observed in those without respiratory symptoms. CONCLUSIONS: Wheezing was significantly associated with reduced FENO in obese subjects, whereas there was a positive association between wheezing and FENO among the non-obese, indicating a possible difference in asthma phenotype, based on body weight.

- Både i verkliga livet och i experimentella studier har det visat sig att större portioner leder till högre energiintag. Stora portioner har också visat sig förenat med högre kroppsvikt. Att portionsstorlekarna blivit större över tid visar såväl återkommande kostundersökningar som inventeringar av utbud på marknaden. Det beror inte på slumpen att många studier fokuserat på snabbmat. Portionsstorlekarna för hamburgare, pommes frites, och läsk har ökat två till fem gånger under de senaste 50 åren.
