  • Assessment of continuous pain in newborns admitted to NICUs in 18 European countries

    • Aim: Continuous pain occurs routinely, even after invasive procedures, or inflammation and surgery, but clinical practices associated with assessments of continuous pain remain unknown.Methods: A prospective cohort study in 243 Neonatal Intensive Care Units (NICUs) from 18 European countries recorded frequency of pain assessments, use of mechanical ventilation, sedation, analgesia, or neuromuscular blockade for each neonate upto 28 days after NICU admission.Results: Only 2113/6648 (31·8%) of neonates received assessments of continuous pain, occurring variably among tracheal ventilation (TrV, 46·0%), noninvasive ventilation (NiV, 35·0%), and no ventilation (NoV, 20·1%) groups (p<0·001). Daily assessments for continuous pain occurred in only 10·4% of all neonates (TrV: 14·0%, NiV: 10·7%, NoV: 7·6%; p<0·001). More frequent assessments of continuous pain occurred in NICUs with pain guidelines, nursing champions, and surgical admissions prompted (all p<0·01), and for newborns <32 weeks gestational age, those requiring ventilation, or opioids, sedatives-hypnotics, general anesthetics (O-SH-GA) (all p<0·001), or surgery (p=0·028). Use of O-SH-GA drugs increased the odds for pain assessment in the TrV (OR:1·60, p<0·001) and NiV groups (OR:1·40, p<0·001).Conclusion: Assessments of continuous pain occurred in less than one-third of NICU admissions, and daily in only 10% of neonates. NICU clinical practices should consider including routine assessments of continuous pain in newborns.

  • Monitoring of chromatin organization in live cells by FRIC. Effects of the inner nuclear membrane protein Samp1

    • In most cells, transcriptionally inactive heterochromatin is preferentially localized in the nuclear periphery and transcriptionally active euchromatin is localized in the nuclear interior. Different cell types display characteristic chromatin distribution patterns, which change dramatically during cell differentiation, proliferation, senescence and different pathological conditions. Chromatin organization has been extensively studied on a cell population level, but there is a need to understand dynamic reorganization of chromatin at the single cell level, especially in live cells. We have developed a novel image analysis tool that we term Fluorescence Ratiometric Imaging of Chromatin (FRIC) to quantitatively monitor dynamic spatiotemporal distribution of euchromatin and total chromatin in live cells. A vector (pTandemH) assures stoichiometrically constant expression of the histone variants Histone 3.3 and Histone 2B, fused to EGFP and mCherry, respectively. Quantitative ratiometric (H3.3/H2B) imaging displayed a concentrated distribution of heterochromatin in the periphery of U2OS cell nuclei. As proof of concept, peripheral heterochromatin responded to experimental manipulation of histone acetylation. We also found that peripheral heterochromatin depended on the levels of the inner nuclear membrane protein Samp1, suggesting an important role in promoting peripheral heterochromatin. Taken together, FRIC is a powerful and robust new tool to study dynamic chromatin redistribution in live cells.

    • The young adults with cerebral palsy (CP) consider that, despite life being so demanding, it is important to perform occupations themselves because "When I do, I become someone". Hence, it is necessary to advance intervention methods based on personally important occupations to enable individuals with CP to find their own way of occupational performance to promote health. Further research is needed to enhance individuals with CP to perform everyday occupations without too much struggle.

  • Life is doing. Facilitators and hindrances for occupational performance in adults with cerebral palsy - including methods of intervention

    • Introduction: Occupational performance is a complex process which poses difficulties to persons with CP even where they have relatively good motor function. Aim: To explore and describe the perception of occupational performance in everyday life in adults with cerebral palsy, to examine whether a certain intervention has the potential to improve opportunities for occupational performance, and to test an instrument that can be used to detect occupational imbalance in this target group. Methods: Study I took a phenomenographic approach, focusing on variation in the participants’ perceptions of occupational performance in everyday life. Directed content analysis was used in Study II, where the interview material from Study I was related to the Model of the Process of Doing. Studies III and IV investigated the feasibility of an intervention, the Cognitive Orientation to daily Occupational Performance (CO-OP) Approach. Study III had an exploratory multiple-case design while in Study IV the semi-structured interviews about the participants’ experiences were analysed using content analysis. Study V used a sequential exploratory design. Results: The participants described it as extremely important to perform occupations independently, since by doing they formed their identity. Facilitators and hindrances for doing were described, showing difficulties in all phases of the ‘process of doing’ where each participant had difficulties in one or more phases. Doing often came with a high cost in terms of deterioration, pain, stress and mental and/or physical fatigue. However, the participants also strove for inclusion when performing occupations. Strategies to be able to process what happens during the performance of a task as well as knowledge about one’s strengths and limitations were characterised as crucial for belief in one’s capability. The participants’ satisfaction with the CO-OP Approach was high and they found that it boosted them. CO-OP was deemed to support their way of thinking and doing during the various phases of the ‘process of doing’. Their self-rated goal attainment as well as their measured executive functions improved. The modified Mental Fatigue Scale showed the potential to be a useful instrument for self-rating of mental fatigue and related symptoms in adults with CP. Conclusion: Young adults with CP (MACS I–II) consider it important to perform everyday occupations themselves and to develop by doing. Hence there is a need for person-centred interventions where persons can develop their potential to solve performance problems that may arise during the various phases of the ‘process of doing’. Moreover, there is a need for interventions that create good opportunities to attain occupational balance.

    • Persons with cerebral palsy, even if they have relatively good motor functions, have a lower level of independence and participation in everyday activities than persons of the same age without disabilities. However, there are few descriptions of how persons with cerebral palsy themselves perceive their performance of activities in everyday life. The aim of this study was to describe the perceptions that young adults with cerebral palsy have of occupational performance in everyday life.This qualitative interview study includes 10 participants with cerebral palsy classified with Manual Ability Classification System level I-II, aged 19-30 years. The data were analyzed using a phenomenographic approach.The interviews resulted in five categories: "Important to do"; "Demanding but can be facilitated"; "Excludes or includes"; "Diminishes me or makes me grow"; and "Comes at a price".The young adults with cerebral palsy consider that, despite life being so demanding, it is extremely important to perform activities themselves and to feel included, as this enables personal growth. Hence, it is necessary to advance intervention methods based on personally important activities to enable individuals with cerebral palsy to find their own way to perform activities. Further research is needed to increase opportunities for individuals with cerebral palsy to perform everyday activities without too much fatigue and struggle. Implications for Rehabilitation For young adults with cerebral palsy it is extremely important to perform everyday activities independently; by DOING activities they form their identity. Intervention models aimed to enable persons with cerebral palsy to be involved and find their own way to perform everyday activities should be emphasized. Attention must be paid to how mental fatigue is manifested in persons with cerebral palsy. To build self-awareness and self-efficacy, individuals with cerebral palsy need information, early in life, about cerebral palsy and the multifaceted difficulties the disability might lead to.
