  • Septic shock induced by group A streptococcal infection : clinical and therapeutic aspects

    • During 1988 and 1989 > 500 cases of serious group A streptococcal infections were reported in Sweden, many with a fatal outcome. We report here on 11 consecutive patients with septic preshock/shock and multiorgan failure, including acute renal failure. 10 had verified group A streptococci (GAS) serotype T1M1 infections while 1 patient was culture negative but with clinical signs of severe infection and serological evidence of GAS infection. Presenting symptoms were high fever, relative bradycardia, edema and renal failure. In all patients the condition deteriorated despite conventional treatment including volume substitution and antibiotics. Systolic blood pressure was transiently < 80 mmHg in 10 patients and 9 of them needed infusion of inotropic agents to avoid fatal circulatory shock. In 9 patients respiratory aid was instituted and 7 were dialysed. Plasma exchange was performed in 7, while the remaining 4 received transfusions with blood and plasma without plasma exchange. 10 patients improved and were discharged within 8 weeks. One woman died within 2 days after admission to the hospital. Renal function recovered in all survivors, with a follow-up serum creatinine < 80 mumol/l. The complicated clinical picture in these patients with many simultaneous therapeutic events confounds the interpretation of the effect of single actions. The favourable outcome in these severely ill patients suggests that potent inotropic agents, immunoglobulin therapy and plasma exchange might be beneficial in severe streptococcal disease when conventional treatment fails.

    • The increasing focus on the chronology of environmental and climatic changes of the last glacial-interglacial transition has led to several independent attempts to try to calibrate the 14C time-scale beyond the Holocene. The Late Weichselian Gotiglacial varved clays of the Swedish Time Scale could potentially be used for this purpose. The reliability of the Swedish Time Scale is discussed as well as different ways of using the Swedish varved clays for calibrating the 14C chronology. The strategy and initial results from an ongoing calibration project are presented. They show clearly that, if the right strategy is adopted, varved clay may be dated by accelerator mass spectrometry (AMS) 14C measurement of terrestrial macrofossils. A Late Weichselian 'event stratigraphy', including the Vedde Ash fall-out, is established for south Scandinavia using three dating categories: clay varve measurements, terrestrial macrofossil measurement, and lake sediment (including aquatic mosses) measurements. It suggests that a 14C chronology based on terrestrial organic remains is not consistent with the traditional Late Weichselian chronostratigraphy based on lake-sediment samples, and that 'clay varve years' exceed 'terrestrial 14C years' by c. 900 years at the end of, and by 1100–1200 years at the beginning of the Younger Dryas Chronozone. Further back in time, the time-scales appear to converge. These results are compared with other recently published calibration studies.

    • Protein G, a bacterial cell-wall protein with high affinity for the constant region of IgG (IgGFc) antibodies, contains homologous repeats responsible for the interaction with IgGFc. A synthetic peptide corresponding to an 11-amino acid-long sequence in the COOH-terminal region of the repeats was found to bind to IgGFc and block the interaction with protein G. Moreover, two other IgGFc-binding bacterial proteins (proteins A and H), which do not contain any sequences homologous to the peptide, were also inhibited in their interactions with IgGFc by the peptide. Finally, a decapeptide based on a sequence in IgGFc blocked the binding of all three proteins to IgGFc. This unusually clear example of convergent evolution emphasizes the complexity of protein-protein interactions and suggests that bacterial surface-protein interaction with host protein adds selective advantages to the microorganism.

    • The Late Quaternary (c. 130000-10000 BP) glacial history of the central west coast of Jameson Land, East Greenland. is reconstructed through glacial stratigraphical studies. Seven major sedimentary units are described and defined. They represent two interglacial events (where one is the Holocene). one interstadial event and two glacial events. The older interglacial event comprises marine and fluvial sediments, and is correlated to the Langelandselv interglacial. corresponding to oxygen isotope sub-stage 5e. It is followed by an Early Weichselian major glaciation during the Aucellaelv stade. and subsequently by an Early Weichselian interstadial marine and deltaic event (the Hugin S0 interstade). Sediments relating to the Middle Weichselian have not been recognized in the area. The llugin S0 interstade deposits have been overrun by a Late Weichselian ice advance, during the Flakkerhuk stade. when the glacier. which probably was a thin, low gradient fjord glacier in Scoresby Sund, draped older sediments and landforms with a thin till. Subsequent to the final deglaciation, some time before 10 000 BP. the sea reached the marine limit around 70 m a , and early Holocene marine. fluvial and littoral sediments were deposited in the coastal areas.
