  • A Glycemia Risk Index (GRI) of Hypoglycemia and Hyperglycemia for Continuous Glucose Monitoring Validated by Clinician Ratings

    • Background: A composite metric for the quality of glycemia from continuous glucose monitor (CGM) tracings could be useful for assisting with basic clinical interpretation of CGM data. Methods: We assembled a data set of 14-day CGM tracings from 225 insulin-treated adults with diabetes. Using a balanced incomplete block design, 330 clinicians who were highly experienced with CGM analysis and interpretation ranked the CGM tracings from best to worst quality of glycemia. We used principal component analysis and multiple regressions to develop a model to predict the clinician ranking based on seven standard metrics in an Ambulatory Glucose Profile: very low–glucose and low-glucose hypoglycemia; very high–glucose and high-glucose hyperglycemia; time in range; mean glucose; and coefficient of variation. Results: The analysis showed that clinician rankings depend on two components, one related to hypoglycemia that gives more weight to very low-glucose than to low-glucose and the other related to hyperglycemia that likewise gives greater weight to very high-glucose than to high-glucose. These two components should be calculated and displayed separately, but they can also be combined into a single Glycemia Risk Index (GRI) that corresponds closely to the clinician rankings of the overall quality of glycemia (r = 0.95). The GRI can be displayed graphically on a GRI Grid with the hypoglycemia component on the horizontal axis and the hyperglycemia component on the vertical axis. Diagonal lines divide the graph into five zones (quintiles) corresponding to the best (0th to 20th percentile) to worst (81st to 100th percentile) overall quality of glycemia. The GRI Grid enables users to track sequential changes within an individual over time and compare groups of individuals. Conclusion: The GRI is a single-number summary of the quality of glycemia. Its hypoglycemia and hyperglycemia components provide actionable scores and a graphical display (the GRI Grid) that can be used by clinicians and researchers to determine the glycemic effects of prescribed and investigational treatments.

  • Field evaluation of a prototype tuberculosis lipoarabinomannan lateral flow assay on HIV-positive and HIV-negative patients

    • Detection of tuberculosis at the point-of-care (POC) is limited by the low sensitivity of current commercially available tests. We describe a diagnostic accuracy field evaluation of a prototype urine Tuberculosis Lipoarabinomannan Lateral Flow Assay (TB-LAM LFA) in both HIV-positive and HIV-negative patients using fresh samples with sensitivity and specificity as the measures of accuracy. This prototype combines a proprietary concentration system with a sensitive LFA. In a prospective study of 292 patients with suspected pulmonary tuberculosis in Uganda, the clinical sensitivity and specificity was compared against a microbiological reference standard including sputum Xpert MTB/RIF Ultra and solid and liquid culture. TB-LAM LFA had an overall sensitivity of 60% (95%CI 51–69%) and specificity of 80% (95%CI 73–85%). When comparing HIV-positive (N = 86) and HIV-negative (N = 206) patients, there was no significant difference in sensitivity (sensitivity difference 8%, 95%CI -11% to +24%, p = 0.4351) or specificity (specificity difference -9%, 95%CI -24% to +4%, p = 0.2051). Compared to the commercially available Alere Determine TB-LAM Ag test, the TB-LAM LFA prototype had improved sensitivity in both HIV-negative (difference 49%, 95%CI 37% to 59%, p<0.0001) and HIV-positive patients with CD4+ T-cell counts >200cells/μL (difference 59%, 95%CI 32% to 75%, p = 0.0009). This report is the first to show improved performance of a urine TB LAM test for HIV-negative patients in a high TB burden setting. We also offer potential assay refinement solutions that may further improve sensitivity and specificity.

  • A detailed microstructural and corrosion analysis of magnesium alloy WE43 manufactured by selective laser melting

    • The production of magnesium alloy WE43 was achieved by selective laser melting (SLM). The alloy was investigated after SLM, hot isostatic pressing (HIP), and solutionising heat treatment. The microstructure and corrosion behaviour of the specimens were carefully characterised, whilst assessed and contrast relative to the conventionally cast alloy counterpart. The SLM prepared specimens possess a unique microstructure comprising fine grains growing with a strong [0001] texture along the building direction with a low fraction of process-induced and metallurgical defects, reaching < 0.1 %, after optimising the SLM parameters and the HIP treatment. Electrochemical measurements demonstrated that the SLM prepared WE43 is cathodically more active as compared with its cast counterpart. It is proposed that this behaviour is due to a high density of zirconium-rich oxide particles uniformly distributed throughout the alloy microstructure as well as the alterations in the chemical composition of the solid-solution matrix originating from the high cooling rates of SLM. It was also noted that the oxide particles are mainly sourced by powder. The present results suggest that the corrosion of SLM prepared Mg alloys could be greatly improved once the influence of powder characteristics is further understood and controlled.

    • Chondroitin sulfate (CS) is the placental receptor for the VAR2CSA malaria protein, expressed at the surface of infected erythrocytes during Plasmodium falciparum infection. Infected cells adhere to syncytiotrophoblasts or get trapped within the intervillous space by binding to a determinant in a 4-O-sulfated CS chains. However, the exact structure of these glycan sequences remains unclear. VAR2CSA-reactive CS is also expressed by tumor cells, making it an attractive target for cancer diagnosis and therapeutics. The identities of the proteoglycans carrying these modifications in placental and cancer tissues remain poorly characterized. This information is clinically relevant since presentation of the glycan chains may be mediated by novel core proteins or by a limited subset of established proteoglycans. To address this question, VAR2CSA-binding proteoglycans were affinity-purified from the human placenta, tumor tissues and cancer cells and analyzed through a specialized glycoproteomics workflow. We show that VAR2CSA-reactive CS chains associate with a heterogenous group of proteoglycans, including novel core proteins. Additionally, this work demonstrates how affinity purification in combination with glycoproteomics analysis can facilitate the characterization of CSPGs with distinct CS epitopes. A similar workflow can be applied to investigate the interaction of CSPGs with other CS binding lectins as well.

    • The Open Databases Integration for Materials Design (OPTIMADE) consortium has designed a universal application programming interface (API) to make materials databases accessible and interoperable. We outline the first stable release of the specification, v1.0, which is already supported by many leading databases and several software packages. We illustrate the advantages of the OPTIMADE API through worked examples on each of the public materials databases that support the full API specification.

  • Recycling of waste toner derived from exhausted printer cartridges as adsorbent for defluoridation of water

    • Due to the broad adoption of electronic and electrical equipment and the quick advancement of contemporary innovations in this domain, significant amounts of electronic waste have been produced. This category of waste includes the toner powder used by printers, copiers, and fax machines to print text and images. This paper describes a sustainable and environmentally friendly method of recycling waste toner powder. The chemical composition of this printer cartridge toner (PCt) powder is carbon, Fe3O4, polypropylene (polymeric resin), and SiO2 composite. Toner powder from exhausted printer cartridges was utilized as an adsorbent to remove fluoride from water. It has a fluoride adsorption capacity of 60 mg/g and a specific surface area of 20 m2/g. X-ray diffraction and electron microscopic investigations were used to investigate the chemical composition, structure, and surface morphology of the material. To analyze the collected experimental data, the Freundlich and Langmuir adsorption isotherm models were used. Time-dependent kinetic experiments were conducted to determine the mechanism of the adsorption process using pseudo-first-order kinetics, pseudo-second-order kinetics, and intraparticle diffusion kinetic models. The fluoride adsorption process was shown to be feasible and spontaneous (ΔG < 0) based on calculated thermodynamic characteristics, which included enthalpy, Gibbs free energy, entropy (ΔS > 0), and adsorption activation energy. The study also discussed its reusability as an adsorbent and examined its functioning capability in actual water. © 2024 The Authors
