  • Assessment of the clinical and molecular impact of different cytogenetic subgroups in a series of 272 lipomas with abnormal karyotype

    • Conventional lipomas harbor karyotypic changes that could be subdivided into four, usually mutually exclusive, categories: rearrangement, in particular through translocations, of chromosome bands 12q13-15, resulting in deregulation of the HMGA2 gene, loss of material from or rearrangement of chromosome 13, supernumerary ring or giant marker chromosomes, and aberrations of chromosome band 6p21. In the present study, 272 conventional lipomas, two-thirds of them deep-seated, with acquired clonal chromosome changes were assessed with regard to karyotypic and clinical features. A nonrandom distribution of breakpoints and imbalances could be confirmed, with 83% of the cases harboring one or more of the previously known cytogenetic hallmarks. Correlation with clinical features revealed that lipomas with rings/giant markers were larger, occurred in older patients, were more often deep-seated, and seemed to have an increased tendency to recur locally, compared with tumors with other chromosome aberrations. The possible involvement of the HMGA2 gene in cases that did not show any of the characteristic cytogenetic changes was further evaluated by locus-specific metaphase fluorescence in situ hybridization (FISH) and RT-PCR, revealing infrequent cryptic disruption of the gene but abundant expression of full length or truncated transcripts. By FISH, we could also show that breakpoints in bands 10q22-23 do not affect the MYST4 gene, whereas breakpoints in 6p21 or 8q11-12 occasionally target the HMGA1 or PLAG1 genes, respectively, also in conventional lipomas.

    • The evolution of bulk flow structures and their influence on the spatial distribution of heat release zones and of partially oxidized fuel and particulate matter (soot) is examined experimentally in a swirl-supported, direct-injection diesel engine. Vector fields describing the bulk flow structures are measured with particle image velocimetry (PIV), while complementary scalar field measurements of partially oxidized fuel and soot are obtained in the same vertical plane using broadband laser-induced fluorescence (LIF) and laser-induced incandescence (LII) techniques, respectively. The two-dimensional divergence of the mean velocity fields is also employed to provide information on the mean locations of heat release. Measurements are performed at a highly dilute, 12% O-2, operating condition characteristic of low-NO,, low-temperature diesel combustion systems. The spatial distributions of unburned fuel rapidly develop a structure characterized by two separate zones of high fuel concentration, an inner zone in the cylinder center and an outer zone in the squish volume. Single-cycle measurements show that this two-zone structure is present on an individual cycle basis, and is not an artifact of averaging distinct, single-zone distributions. For this engine build, the mean flow structures developed do not actively promote mixing of either zone, although bulk flow structures in the upper-central region of the cylinder vary significantly on a cycle-by-cycle basis. The measured spatial distributions of particulates indicate that particulates are formed primarily in the inner zone-and remain un-oxidized late in the cycle.     • This paper presents numerical simulations and laser diagnostic experiments of a swirling lean premixed methane/air flame with an aim to compare different Large Eddy Simulations (LES) models for reactive flows. An atmospheric-pressure laboratory swirl burner has been developed wherein lean premixed methane/air is injected in an unconfined low-speed flow of air. The flame is stabilized above the burner rim in a moderate swirl flow, triggering weak vortex breakdown in the downstream direction. Both stereoscopic (3-component) PIV and 2-component PIV are used to investigate the flow. Filtered Rayleigh scattering is used to examine the temperature field in the leading flame front. Acetone-Planar Laser Induced Fluorescence (PLIF) is applied to examine the fuel distribution. The experimental data are used to assess two different LES models: one based on level-set G-equation and flamelet chemistry, and the other based on finite rate chemistry with reduced kinetics. The two LES models treat the chemistry differently, which results in different predictions of the flame dynamic behavior and statistics. Yet, great similarity of flame structures was predicted by both models. The LES and experimental data reveal several intrinsic features of the low swirl flame such as the W-shape at the leading front, the highly wrinkled fronts in the shear layers, and the existence of extinction holes in the trailing edge of the flame. The effect of combustion models, the numerical solvers and boundary conditions on the flame and flow predictions was systematically examined. (c) 2008 The Combustion Institute.   • Orlistat in responding obese type 2 diabetic patients : meta-analysis findings and cost-effectiveness as rationales for reimbursement in Sweden and Switzerland

    • OBJECTIVE: The aim of this study is to review the clinical and economic rationale for the reimbursement of orlistat in responding obese patients with type 2 diabetes.METHODS: Data from seven randomized controlled clinical trials of orlistat in overweight and obese patients with type 2 diabetes were pooled. A subgroup analysis involving patients who achieved a response (defined as a weight loss of >/=5% after 12 weeks of treatment) was conducted. The outcomes of the pooled analysis were then used to construct a Markov health economic model covering an 11-y period. The incidences of diabetes-related micro- and macrovascular complications were derived from the United Kingdom Prospective Diabetes Study. The effects of changes in body mass index, and the impact of micro- and macrovascular complications on utilities were derived from published sources. Publicly available cost data were used and are presented here in 2001 Euros. Discounting of 3% was applied. A probabilistic sensitivity analysis was conducted to examine the robustness of results.RESULTS: A total of 1249 patients treated with orlistat and 1230 given placebo were eligible for the intent-to-treat analysis. At the end of the study period, 23% of orlistat patients achieved a weight reduction of >/=5%. These patients showed a mean decrease in HbA1C of 1.16%, a weight reduction of 8.6 kg, a reduction in total cholesterol of 5.3% and a reduction in systolic blood pressure of 5.2 mmHg. The base-case economic analysis revealed costs per quality-adjusted life year gained of euro14 000 in Sweden and euro13 600 in Switzerland.CONCLUSION: The data presented here support the utilization and reimbursement of orlistat in overweight and obese diabetic patients who respond to the treatment.
