  • One-year excess mortality and treatment in surgically treated patients with colorectal cancer : A EURECCA European comparison

    • Background: Mortality in the first postoperative year represents an accurate reflection of the perioperative risk after colorectal cancer surgery. This research compares one-year mortality after surgery divided into three age-categories (18-64, 65-74, ≥75 years), focusing on time trends and comparing treatment strategies.Material: Population-based data of all patients diagnosed and treated surgically for stage I-III primary colorectal cancer from 2007 to 2016, were collected from Belgium, the Netherlands, Norway, and Sweden. Stratified for age-category and stage, treatment was evaluated, and 30-day, one-year and one-year excess mortality were calculated for colon and rectal cancer separately. Results were evaluated over two-year time periods.Results: Data of 206,024 patients were analysed. Postoperative 30-day and one-year mortality reduced significantly over time in all countries and age-categories. Within the oldest age category, in 2015–2016, one-year excess mortality varied from 9% in Belgium to 4% in Sweden for colon cancer and, from 9% in Belgium to 3% in the other countries for rectal cancer. With increasing age, patients were less likely to receive additional therapy besides surgery. In Belgium, colon cancer patients were more often treated with adjuvant chemotherapy (p < 0.001). For neoadjuvant treatment of rectal cancer, patients in Belgium and Norway were mostly treated with chemoradiotherapy. In the Netherlands and Sweden, radiotherapy alone was preferred (p < 0.001).Conclusions: Despite improvement over time in all countries and age-categories, substantial variation exists in one-year postoperative mortality. Differences in one-year excess postoperative mortality could be due to differences in treatment strategies, highlighting the consequences of under- and over-treatment on cancer survival.

    • Background: A decade ago, it was demonstrated that the difference in survival between older patients and younger patients with colorectal cancer (CRC) was mainly due to mortality in the first postoperative year. Over the last few years, improvements - especially in perioperative care - have increased survival. The current research investigates whether a survival gap between younger and older patients with CRC still exists on a national level in four European countries.Methods: Population-based data from Belgium, the Netherlands, Norway, and Sweden were collected from patients that underwent surgical resection for primary stage I-III CRC between 2007 and 2016. Relative survival and conditional relative survival (CS), with the condition of surviving the first postoperative year, were calculated for colon and rectal cancer separately, stratified for country and age category (<65, 65–75, ≥75 years). In addition, relative excess risk of death (RER) was estimated, and one-year excess mortality was calculated.Results: Data of 206,024 patients were analyzed. In general, compared to patients <65 years, patients ≥75 years had a worse survival during the first year after surgery, which was most pronounced in Belgium (RER colon cancer 2.5 [95% confidence interval (CI) 2.3–2.8] and RER rectal cancer 2.6 [95% CI 2.3–2.9]). After surviving the first year, CS was mostly not statistically different between patients <65 years and patients ≥75 years with stage I-II, with the exception of stage II colon cancer in Belgium. However, CS remained worse in the largest part of the patients ≥75 years with stage III colon or rectal cancer (except for rectal cancer in Norway).Conclusions: Although differences exist between the countries, the survival gap between young and older patients is based mainly on early mortality and remains only for stage III disease after surviving the first year.

Totalt redovisas underlag och råd för 48 fisk- och skaldjursarter.De bestånd som förvaltas nationellt baseras på de biologiska underlagen, och rådgivningen i huvudsak på den forskning och övervakning samt analys som bedrivs av Institutionen för akvatiska resurser vid Sveriges lantbruksuniversitet (SLU Aqua) samt yrkesfiskets rapportering.

  • Assessment of the eel stock in Sweden, spring 2021 : fourth post-evaluation of the Swedish eel management

    • For decades, the population of the European eel has been in severe decline. In 2007, the European Union decided on a Regulation establishing measures for the recovery of the stock, which obliged Member States to implement a national Eel Management Plan by 2009. Sweden submitted its plan in 2008. According to the Regulation, Member States shall report regularly to the EU-Commission, on the implementation of their Eel Management Plans and the progress achieved in protection and restoration. The current report provides an assessment of the eel stock in Sweden as of spring 2021, intending to feed into the national reporting to the EU later this year. This report updates and extends the reports by Dekker (2012, 2015) and Dekker et al. (2018).In this report, the impacts on the stock - of fishing, restocking and mortality related to hydropower generation - are assessed. Other anthropogenic impacts (climate change, pollution, increased impacts of predators, spread of parasites, disruption of migration due to disorientation after transport, and so forth) probably have an impact on the stock too, but these factors are hardly quantifiable and no management targets have been set. For that reason, and because most factors were not included in the EU Eel Regulation, these other factors are not included in this technical evaluation. Our focus is on the quantification of biomass of silver eel escaping from continental waters towards the ocean (current, current potential and pristine) and mortality risks endured by those eels during their whole lifetime. The assessment is broken down on a geographical basis, with different impacts dominating in different areas (west coast, inland waters, Baltic coast).In 2011, a break in the downward trend of the number of glass eel was observed throughout Europe, the trend since being upward, but erratic. Whether that relates to recent protective actions, or is due to other factors, is yet unclear. This report contributes to the required international assessment, but does not discuss the causing factors behind that recent trend and the overall status of the stock across Europe.

  • Chinese eel products in EU markets imply the effectiveness of trade regulations but expose fraudulent labelling

    • The stock of the European eel (Anguilla anguilla) is in a multi-decadal decline. Therefore, trade in European eel is now restricted by EU law and the listing in CITES Appendix II. EU law prohibits the trade of European eel across the EU’s outer border and CITES regulates the global trade elsewhere. In November and December 2019, we purchased 108 eel products in 21 cities in five major eel importing countries in Europe (Germany, Belgium, Netherlands, Great Britain, France) and three online shops. All were imported from China and 73 samples were genetically identified as American eel (A. rostrata), 33 as Japanese eel (A. japonica), and a single sample each as European eel and Indian shortfin eel (A. bicolor pacifica). The one European eel sample violated the EU trade ban and CITES trade regulations. However, 28.7 % of the product labels violated EU Regulation (EU) No 1169/2011 on the provision of food information to consumers (FIC). Our results imply that Chinese exporters sell their European eel products outside the EU market and therefore avoid violating EU law. However, fraudulent labelling point at inadequate existing EU labelling requirements for prepared and preserved products and ongoing molecular genetic control of eel commodities entering the EU from China.

  • The commercial push net fisheries for glass eels in France and its handling mortality

    • Glass eels, the young life stage of the endangered European eel entering the European continental waters, are exploited for human consumption (direct, or on-grown) and restocking. Restocking is considered as a potential contribution to the conservation of the stock. For this, the mortality of glass eels during capture should be low. We studied the handling mortality of glass eels obtained by commercial push net fisheries in different rivers in France in 2019 and 2020. The direct mortality of glass eels in 41 fishing trips of 29 boats varied from zero to 3.1% (mean 0.3%). Skin lesions identified by staining occurred in 31% (range: 4%–98%) of the eels. Post fishing mortality of glass eels varied from zero to 67.2%. The mean total fishing mortality was with 7.4% (range 0%–56.2%), considerably lower than in previous studies. The Sustainable Eel Group (SEG) issues certificates to fishers applying best practices for a responsible fishery. Comparing certified to uncertified fishers, the certified fishers have on average significant lower lesions and post-fishing mortality rates, However, some uncertified fishers have an equal or better glass eel quality than certified fishers. Noting that certified fishers score better on average, and avoid the worst scores, we conclude that the SEG-certification scheme for responsible fishing is effective.
