    • BackgroundProvision of developmentally appropriate support for child visitors in adult intensive care units (ICUs) would benefit patients and young visitors. Research on best practices for child visitation in adult ICUs is limited.ObjectivesTo explore the perceptions and practices of nurses working in adult ICUs in the United States regarding child visitation and the role of child life specialists in this setting.MethodsData were collected from 446 adult ICU nurses via a cross-sectional survey. The survey explored perceptions and practices regarding child visitation, access to child-friendly resources, and the feasibility of having a child life specialist in adult ICUs.ResultsSeveral participants (303, 67.9%) felt that children were at risk for psychological trauma from visiting an adult ICU. Some participants (122, 27.4%) reported that their ICUs did not have policies for child visitation. Logistic regression showed that nurses with a master’s degree were 1.8 times (P < .05) more likely to believe that young children (0-5 years) should visit. Nurses (105 of 197, 53.3%) were more likely to allow young children to visit if the patient was the child’s parent or if the patient was dying. Child-friendly resources were not routinely available. Nurses expressed that adult ICUs could benefit from child life specialists facilitating child visitation.ConclusionsNurses were inconsistently open to child visitation. Exceptions for older children (> 6 years), children whose parent was the patient, patients’ illness severity, and end of life allowed more child visitation. Ways to facilitate child-friendly visitation in adult ICUs are discussed.

    • A novel autotuning procedure is presented through application to an industrial in-line pH control system. The procedure has three advantages over classical relay auto-tuners: experiment duration is very short (no need for limit-cycle convergence); all data is used for identification (instead of only peaks and switch instances); a parameter uncertainty model is identified and utilized for robust controller synthesis.

    • In many system identification methods, process model parameters are considered stochastic variables. Several methods do not only yield expectations of these, but in addition their variance, and sometimes higher moments. This paper proposes a method for robust synthesis of the proportional-integral-derivative (PID) controller, taking parametric process model uncertainty explicitly into account. The proposed method constitutes a stochastic extension to the well-studied minimization of integrated absolute error (IAE) under H∞-constraints on relevant transfer functions. The conventional way to find an approximate solution to the extended problem is through Monte Carlo (MC) methods, resulting in high computational cost. In this work, the problem is instead approximated by a deterministic one, through the unscented transform (UT), and its conjugate extension (CUT). The deterministic approximations can be solved efficiently, as demonstrated through several realistic synthesis examples.

    • Neurofilaments, a major cytoskeletal constituent of neuronal cells, can be released into the cerebrospinal fluid during several neurodegenerative diseases. By means of a new sensitive ELISA capable of measuring 60 ng/l of neurofilament light, significant elevations were observed for different neurological disorders. Cerebral infarction presented levels of 19800+/-9100 ng/l, amyothropic lateral sclerosis 3600+/-1200 ng/l, 'relapsing-remitting' MS 2500+/-1500 ng/l, extrapyramidal symptoms 1100+/-300 ng/l, late onset AD 300+/-100 ng/l and vascular dementia 1400+/-800 ng/l. In patients with no signs of neurological diseases the upper normal level and cut-off values was determined to be below 100 ng/l. NF-L determinations will be a valuable complement in identifying neuronal degradation and can be used clinically for diagnostic and monitoring purposes.
