  • Preoperative localization of endocrine pancreatic tumours by intra-arterial dynamic computed tomography

    • Eleven patients with biochemically confirmed endocrine pancreatic tumours were examined with intra-arterial (i.a.) dynamic computed tomography (CT) and angiography preoperatively. Seven of the patients suffered from the multiple endocrine neoplasia type 1 (MEN-1) syndrome. All patients were operated upon and surgical palpation and ultrasound were the peroperative localization methods. Of the 33 tumours which were found at histopathologic analysis of the resected specimens in the 11 patients, 7 tumours in 7 patients were correctly localized by both i.a. dynamic CT and angiography. Six patients with MEN-1 syndrome had multiple tumours and this group of patients together had 28 tumours, of which 5 (18%) were localized preoperatively by both CT and angiography. I.a. dynamic CT, with the technique used by us, does not seem to improve the localization of endocrine pancreatic tumours, especially in the rare group of MEN-1 patients, as compared with angiography.

    • The elastodynamic scattering by a penny-shaped crack with spring boundary conditions is investigated. The transition (T) matrix of the crack is determined and the usefulness of this is illustrated by considering also the scattering by two cracks. The T matrix of a single crack is first determined by a direct integral equation method which gives the crack-opening displacement and the integral representation which subsequently gives the scattered field expanded in spherical waves. Two cracks are considered by a multi-centred T matrix approach where matrix inverses are expanded in Neuman series. Rotation matrices are employed so that the cracks may have an arbitrary rotation. The back-scattered longitudinal far field amplitude is computed both in the frequency and time domain in a few cases and the effects due to multiple scattering are in particular explored.

    • The propagation of ultrasonic waves through a perfectly elastic medium containing a random distribution of equally-sized penny-shaped cracks with spring boundary conditions across the crack faces are considered. As limiting cases results for open and fluid-filled cracks are derived also. The medium with the crack distribution is modelled as an effective viscoelastic medium, using the non-interecting scatterer approximation and Foldy's thery. For this purpose the scattering by a single crack is solved by an integral equation method. Distributions of both randomly oriented and parallel cracks are considered. Numerical results are presented for the phase velocity and attenuation. For parallel cracks when the effective medium becomes transversely isotropic two further issues are investigated.The first is the extension of a static result due to Kachanov, who showed the transverse isotropy to be of a very special kind. The second is consistency of the wave speeds obtained by using Foldy's theory, with the fact that the effective material is transversely isotropic. In particular, the vertically polarized shear wave should have the same wave speeds in directions parallel and normal to the cracks. It is found that the relations established by Kachanov and the consistency requirements are satisfied by the phase velocity for all frequencies considered.

    • With the radionuclide tracers available today, 50-90 per cent of neuroendocrine tumours of the gastro-intestinal tract can be visualised with PET (positron-emission tomography). PET also enables the effect of tumour treatment to be monitored in terms of biochemical and functional variables, which is not possible with other radiological techniques. Owing to the very good tumour resolution possible with PET, it serves as a complement to other routine methods such as computed tomography and ultrasonography, and can be used to screen the chest and abdomen for small primary tumours that can not be detected with other methods. In several pre-operative trials PET has been shown to demonstrate more changes in the pancreas and liver than was possible with other methods. In the near future it will be possible to demonstrate the presence of and quantify growth factor receptors, hormones, enzymes, DNA synthesis, mRNA synthesis and protein synthesis. Access to these tumour biological data will be of crucial importance to the individualisation of treatment.

  • Cellular adaptation of the trapezius muscle in strength-trained athletes

    • The aim of this study was to elucidate the cellular events that occur in the trapezius muscle following several years of strength training. In muscle biopsies from ten elite power lifters (PL) and six control subjects (C), several parameters were studied: cross-sectional area of muscle fibres, myosin heavy chain composition (MHC) and capillary supply [capillaries around fibres (CAF) and CAF/fibre area]. A method was also developed for counting the number of myonuclei and satellite cell nuclei. The proportion of fibres expressing MHC IIA, the cross-sectional area of each fibre type and the number of myonuclei, satellite cells and fibres expressing markers for early myogenesis were significantly higher in PL than in C (P<0.05). A significant correlation between the myonuclear number and the cross-sectional area was observed. Since myonuclei in mature muscle fibres are not able to divide, we suggest that the incorporation of satellite cell nuclei into muscle fibres resulted in the maintenance of a constant nuclear to cytoplasmic ratio. The presence of small diameter fibres expressing markers for early myogenesis indicates the formation of new muscle fibres.

  • Occupational exposures as risk factors for oral cancer evaluated in a Swedish case-control study.

    • In a case-control study with 410 cases and 410 matched controls from the period 1980-1989 we investigated the role of occupations and occupational exposures as risk factors for squamous cell oral cancer. All subjects received a questionnaire. The response rates were 96% and 91% for cases and controls, respectively. A significantly increased risk was found for pulp industry workers, odds ratio (OR) 4.0 and wood or product workers, OR 5.5. Exposure to chemicals such as phenoxyacetic acids yielded an OR of 1.7. The positive findings in this study may be explained by exposure to chemicals, but need to be verified in further studies.

    • We analysed the presence of hepatitis C virus (HCV) antibodies in 566 patients undergoing liver biopsy. While over 20% of the patients were anti-HCV positive according to ELISA, only 13.8% had HCV antibodies when tested with a four-antigen recombinant immunoblot assay (RIBA 2). At the time of inclusion in the study, most patients were asymptomatic, irrespective of whether they were HCV-positive. Histological findings in anti-HCV-positive patients were chronic persistent hepatitis, chronic active hepatitis or cirrhosis in greater than 75% of cases. Only four of the patients who were anti-HCV-positive according to the RIBA 2 had autoimmune chronic active hepatitis. Risk behaviour could be identified in the majority of cases. Community-acquired sporadic cases were rare (12%). Of the 153 patients who died during follow-up, 23 subjects were anti-HCV positive. Although age- and sex-adjusted survival was not shorter in anti-HCV-positive patients than in anti-HCV-negatives, the risk of hepatocellular cancer was higher (P = 0.01). We conclude that HCV infection is associated with chronic liver disease, even when critical evidence of viral aetiology is slight. Truly sporadic cases are rare. Patients infected with HCV are at increased risk of developing hepatocellular cancer.
