    • Fifty-seven patients with MAGE-3-positive measurable metastatic cancer, most of them with melanoma, were vaccinated with escalating doses of a recombinant MAGE-3 protein combined with a fixed dose of the immunological adjuvant SBAS-2, which contained MPL and QS21. The immunisation schedule included 4 intramuscular (i.m.) injections at 3-week intervals. Patients whose tumour stabilised or regressed after 4 vaccinations received 2 additional vaccinations at 6-week intervals. The vaccine was generally well tolerated. Among the 33 melanoma patients who were evaluable for tumour response, we observed 2 partial responses, 2 mixed responses and 1 stabilisation. Time to progression in these 5 patients varied from 4 to 29 months. In addition, a partial response lasting 10 months was observed in 1 of the 3 metastatic bladder cancer patients included. None of the tumour responses described above involved visceral metastases. Immunological responses to the vaccine will be reported separately. ⌐ 2002 Elsevier Science Ltd. All rights reserved.

    • Background: Immunosuppression is documented in several malignant diseases, including breast cancer. Subsequently, future therapeutic concepts might include immunological approaches. However, detailed knowledge about tumor immunogenicity and host immunoreactivity, and how to assess these adequately, is still limited. We studied CD28 and CD3-ζ expression in sentinel node biopsies (SNB) from breast cancer patients to analyze tumor-related changes in T cell activity. Method: 25 women underwent surgery for primary breast cancer, including SNB. Frozen sections from 21 sentinel nodes could be analyzed with a double-staining technique. CD28 expression was studied in CD4+ and CD8+ T-lymphocyte subsets and compared with CD3-ζ expression in three specified nodal regions. Results: The degree of CD28 expression varied between the different lymph node areas. The lowest degree of CD28 expression was observed in CD4+ T-lymphocytes in the paracortex and germinal centers. Here, a good agreement with CD3-ζ expression was found. A higher CD28 expression was noted in CD4+ T-cells in the primary follicles, where concordance with CD3-ζ expression was weaker. The CD8+ T-lymphocyte subset displayed generally a higher degree of CD28 expression than the CD4+ subset. Conclusion: Sentinel lymph nodes from breast cancer patients displayed local immunosuppression of varying extent. In the areas with the lowest degree of CD28 expression an accordingly low CD3-ζ expression was found. The SNB might prove an important diagnostic tool for the evaluation of interactions between tumor and the host immune system, helping to select patients who might benefit from adjuvant immunotherapy.

  • On the extent of quantum noise limitation in digital chest radiography

    • The aim for this work was to study to what extent the detection of nodules is quantum noise limited, based on the combined results from a nodule-detection clinical trial and a Monte Carlo computational model of a digital chest imaging system. The Monte Carlo computer program computes measures of physical image quality such as image contrast, C and signalto-noise ratio, SNR for nodules of any size. A computed radiography (CR) imaging system used simulated. The patient anterior-posterior thickness was 25 cm and nodules with diameters between 1-40 mm were included. The image contrast and SNR was calculated for 1600 (40x40) positions in the chest image and averaged over five anatomical regions of interest (lateral pulmonary, retrocardial, hilar, lower- and upper mediastinal regions). Threshold contrasts for each region, Cth, corresponding to Az=0.80 for detecting a 10 mm nodule, were deduced from the clinical trial. A threshold is also used for the quantum noise signal-to-noise ratio, SNRth. The model computes the diameter of a disk-shaped object that is required to comply with the two criteria: SNR≥SNRth and C≥Cth. A system is said to be quantum noise limited when the nodule size required to fulfil both criteria is not limited by the contrast but by the SNR. The required nodule diameter is largest in the hilar region (25 mm) and smallest in the lateral pulmonary region (11 mm). When the threshold SNRth=25 is used, the lower mediastinal region is quantum noise limited already at low speed classes (S>100). The hilar region is never quantum noise limited at realistic speed classes (S<1000). The accuracy of this model will be tested in the future by more sophisticated modelling of anatomical background and noise in the SNR-expression.
