    • Deep learning convolutional neural networks (CNN) may facilitate melanoma detection, but data comparing a CNN's diagnostic performance to larger groups of dermatologists are lacking.Google's Inception v4 CNN architecture was trained and validated using dermoscopic images and corresponding diagnoses. In a comparative cross-sectional reader study a 100-image test-set was used (level-I: dermoscopy only; level-II: dermoscopy plus clinical information and images). Main outcome measures were sensitivity, specificity and area under the curve (AUC) of receiver operating characteristics (ROC) for diagnostic classification (dichotomous) of lesions by the CNN versus an international group of 58 dermatologists during level-I or -II of the reader study. Secondary end points included the dermatologists' diagnostic performance in their management decisions and differences in the diagnostic performance of dermatologists during level-I and -II of the reader study. Additionally, the CNN's performance was compared with the top-five algorithms of the 2016 International Symposium on Biomedical Imaging (ISBI) challenge.In level-I dermatologists achieved a mean (±standard deviation) sensitivity and specificity for lesion classification of 86.6% (±9.3%) and 71.3% (±11.2%), respectively. More clinical information (level-II) improved the sensitivity to 88.9% (±9.6%, P=0.19) and specificity to 75.7% (±11.7%, P<0.05). The CNN ROC curve revealed a higher specificity of 82.5% when compared with dermatologists in level-I (71.3%, P<0.01) and level-II (75.7%, P<0.01) at their sensitivities of 86.6% and 88.9%, respectively. The CNN ROC AUC was greater than the mean ROC area of dermatologists (0.86 versus 0.79, P<0.01). The CNN scored results close to the top three algorithms of the ISBI 2016 challenge.For the first time we compared a CNN's diagnostic performance with a large international group of 58 dermatologists, including 30 experts. Most dermatologists were outperformed by the CNN. Irrespective of any physicians' experience, they may benefit from assistance by a CNN's image classification.This study was registered at the German Clinical Trial Register (DRKS-Study-ID: DRKS00013570; https://www.drks.de/drks_web/).

  • Overdiagnosis of COPD in Subjects With Unobstructed Spirometry A BOLD Analysis

    • BACKGROUND: There are several reports on underdiagnosis of COPD, while little is known about COPD overdiagnosis and overtreatment. We describe the overdiagnosis and the prevalence of spirometrically defined false positive COPD, as well as their relationship with overtreatment across 23 population samples in 20 countries participating in the BOLD Study between 2003 and 2012.METHODS: A false positive diagnosis of COPD was considered when participants reported a doctor's diagnosis of COPD, but postbronchodilator spirometry was unobstructed (FEV1/FVC > LLN). Additional analyses were performed using the fixed ratio criterion (FEV1/FVC < 0.7).RESULTS: Among 16,177 participants, 919 (5.7%) reported a previous medical diagnosis of COPD. Postbronchodilator spirometry was unobstructed in 569 subjects (61.9%): false positive COPD. A similar rate of overdiagnosis was seen when using the fixed ratio criterion (55.3%). In a subgroup analysis excluding participants who reported a diagnosis of "chronic bronchitis" or "emphysema" (n = 220), 37.7% had no airflow limitation. The site-specific prevalence of false positive COPD varied greatly, from 1.9% in low- to middle-income countries to 4.9% in high-income countries. In multivariate analysis, overdiagnosis was more common among women, and was associated with higher education; former and current smoking; the presence of wheeze, cough, and phlegm; and concomitant medical diagnosis of asthma or heart disease. Among the subjects with false positive COPD, 45.7% reported current use of respiratory medication. Excluding patients with reported asthma, 34.4% of those with normal spirometry still used a respiratory medication.CONCLUSIONS: False positive COPD is frequent. This might expose nonobstructed subjects to possible adverse effects of respiratory medication.

  • Trajectory optimization for multiple UAVs acting as wireless relays

    • This paper proposes a novel wireless relay selection scheme involving multiple mobile Unmanned Aerial Vehicles (UAVs) to support communicating ground users. The goal is to optimize the transmit power levels and trajectories of the relaying UAVs in order to maximize the data rate transmission of the ground users which are suffering from the absence of direct link. Assuming that each UAV is initially characterized by a predefined trajectory for a primary task, we propose to modify it whenever it is needed and the energy and trajectory boundaries constraints allow. We propose to solve this problem using an iterative two steps solution; first, a Mixed Integer Linear Programming (MILP) problem is formulated to optimally determine the users- UAVs associations and the UAVs' corresponding transmit power levels. In the second step, an efficient algorithm based on a recursive shrink-and-realign process is proposed to optimize the UAV trajectories. The performance of the proposed method shows advantages in terms of average throughput compared to the predefined trajectories solution.

    • Inter-annual climatic variability over a large portion of sub-Saharan Africa is under the influence of the El Niño-Southern Oscillation (ENSO). Extreme variability in climate is a threat to rural livelihoods in sub-Saharan Africa, yet the role of ENSO in the balance between supply and demand of net primary productivity (NPP) over this region is unclear. Here, we analyze the impact of ENSO on this balance in a spatially explicit framework using gridded population data from the WorldPop project, satellite-derived data on NPP supply, and statistical data from the United Nations. Our analyses demonstrate that between 2000 and 2013 fluctuations in the supply of NPP associated with moderate ENSO events average ± 2.8 g C m−2 yr.−1 across sub-Saharan drylands. The greatest sensitivity is in arid Southern Africa where a + 1 °C change in the Niño-3.4 sea surface temperature index is associated with a mean change in NPP supply of −6.6 g C m−2 yr.−1. Concurrently, the population-driven trend in NPP demand averages 3.5 g C m−2 yr.−1 over the entire region with densely populated urban areas exhibiting the highest mean demand for NPP. Our findings highlight the importance of accounting for the role ENSO plays in modulating the balance between supply and demand of NPP in sub-Saharan drylands. An important implication of these findings is that increase in NPP demand for socio-economic metabolism must be taken into account within the context of climate-modulated supply.

    • The magnetic behavior of Zn substituted Cu-Zn spinel ferrites having chemical formula Cu1-xZnxFe2O4 (x = 0.7, 0.8, 0.9 and 1.0) has been studied by SQUID magnetometry, by means of magnetic hysteresis, field-cooled (FC) and zero-field-cooled (ZFC) magnetization, memory effect and low field ac susceptibility measurements. These measurements suggest that the ferrimagnetic phase of the x <= 0.8 samples is gradually turned into a spin glass (x >= 0.9). The compound with x = 0.9 exhibits the typical dynamical behavior of spin glasses, with indication of aging, rejuvenation and memory effects. The evolution of the magnetic properties of Cu-Zn spinel ferrites with substitution of Zn for Cu is discussed.

  • The Ly alpha Reference Sample. VIII. Characterizing Ly alpha Scattering in Nearby Galaxies

    • We examine the dust geometry and Ly alpha scattering in the galaxies of the Lyman Alpha Reference Sample (LARS), a set of 14 nearby (0.02 < z < 0.2) Ly alpha-emitting and starbursting systems with Hubble Space Telescope Ly alpha, H alpha, and H beta imaging. We find that the global dust properties determined by line ratios are consistent with other studies, with some of the LARS galaxies exhibiting clumpy dust media, while others of them show significantly lower Ly alpha emission compared to their Balmer decrement. With the LARS imaging, we present Ly alpha/H alpha and H alpha/H beta maps with spatial resolutions as low as similar to 40 pc, and use these data to show that in most galaxies, the dust geometry is best modeled by three distinct regions: a central core where dust acts as a screen, an annulus where dust is distributed in clumps, and an outer envelope where Ly alpha photons only scatter. We show that the dust that affects the escape of Ly alpha is more restricted to the galaxies' central regions, while the larger Ly alpha halos are generated by scattering at large radii. We present an empirical modeling technique to quantify how much Ly alpha scatters in the halo, and find that this characteristic scattering distance correlates with the measured size of the Ly alpha halo. We note that there exists a slight anti-correlation between the scattering distance of Ly alpha and global dust properties.
