    • A review of recent advances in high-energy ion beam analysis at the Lund Institute of Technology is presented. A nonvacuum specimen chamber allows chemical speciation using a combination of ion beam analysis and controlled heating. The development of a new versatile scanning proton microbeam based on a new dedicated accelerator, an achromatic triplet lens and an advanced specimen chamber is outlined together with the performance of a microVAX-II/VMEbus-based data acquisition system.

  • Relation between donor transaminase and recipient hepatitis non-A, non-B in Sweden

    • The relation between donor alanine aminotransferase (ALT) and recipient post-transfusion hepatitis (PTH) non-A, non-B was studied in patients tested before and 6 and 12 weeks after transfusion. The minimum ALT criterion for PTH was 105 IU/l (greater than 2.5 times the upper normal of 42 IU/l). In 8.8% of donors, ALT was greater than 42 IU/l, and in 2.3% ALT was greater than 63 IU/l, i.e., 1.5 times elevated. PTH non-A, non-B occurred in 14 of 742 recipients. The PTH incidence increased when donor ALT was above 63 IU/l (1.5 vs. 5.6%; p less than 0.05). However, if the confounding factor of volume variations was compensated for, elevated donor ALT and PTH were only statistically linked among recipients less than 70 years (p less than 0.02; Mantel-Haenszel test).

  • Bilaterality in slipped upper femoral epiphysis

    • The frequency of slipping and osteoarthritis of the contralateral hip was recorded in 260 patients with slipped upper femoral epiphysis between 1910 and 1960. Twenty-three of these patients (9%) had primary bilateral slipping, 32 (12%) had a contralateral slip diagnosed later during adolescence and a further 104 (40%) had signs of contralateral slipping at follow-up 16 to 66 years later, giving a total of 159 cases (61%) with bilateral slips. Of the 104 slippings diagnosed at follow-up, 25% showed osteoarthirits. it is concluded that, with a slipped epiphysis, prophylactic contralateral pinning should be performed to avoid slipping and to reduce the risk of osteoarthritis.

    • Early closure of the growth plate after fixing a slipped upper femoral epiphysis is often regarded as favourable since, if growth of the femoral neck continues, the metal may lose its grip on the epiphysis with the possibility of further slipping. However, in the young child early fusion may result in leg length discrepancy and limited motion. The hook-pin (Hansson 1982) permits continued growth of the femoral neck without the risk of further slipping, as illustrated in the following cases.

    • Using polarization-dependent angle-resolved photoemission we show that two dominating structures in the photoemission spectra are due to direct transitions from the uppermost two valence bands in silicon. The final-state band for these transitions at photon energies 10.2-21.2 eV is found to have free-electron-like dispersion. Our results imply that that threefold-symmetry emission often assigned to back-bond surface states on Si(111)2×1 is really due to bulk photoemission.

    • A solid phase radio-immunoassay (SPRIA) was developed for the detection of anti-HBc IgM. The assay proved sensitive and easy to perform and rheumatoid factor did not affect the test results. Anti-HBc IgM titres were followed in consecutive samples from 15 patients after uncomplicated acute hepatitis B. In the acute phase anti-HBc IgM titres ranged from 10(-5) to 10(-7) (mean 10(-6.4)). One year after onset of disease ten of the 15 had titres below 10(-4) and between two and three years after onset most patients had titres 10(-3). Anti-HBc IgM titres were determined in six episodes of acute hepatitis B, all HBsAg negative but anti-HBc positive in the first samples obtained (within 8 days) and developing anti-HBs during convalescence. Acute phase anti-HBc IgM titres in these patients ranged between 10(-5.5) and 10(-7) (mean 10(-6.5)) and were thus identical with HBsAg positive cases. When acute phase sera from 168 episodes of acute hepatitis primarily classified as non-A, non-B, were tested for anti-HBc IgM titres above 10(-5), sera from 13 episodes were positive and in seven of these hepatitis B diagnosis could be confirmed by rising anti-HBs titres in convalescence. Sera from four of the 13 patients contained HBeAg, which was thus demonstrated in the absence of HBsAg. The results show that testing for anti-HBc IgM is important for a true non-A, non-B diagnosis.

    • A strain of Escherichia coli originally isolated from urine of a patient with acute pyelonephritis was studied in detail for binding to glycosphingolipids. Bacteria labeled metabolically with [14C]glucose were layered over a glycolipid chromatogram and bound bacteria were detected by autoradiography. The detection was down to a few ng of glycolipid (pmol level) under these assay conditions. At a test level of 500 ng all glycolipids (more than a dozen molecular species analyzed) with Gal alpha 1----4Gal as an internal or terminal part bound the bacteria strongly while glycolipids known to lack this sequence were negative. Conformational analysis using hard sphere calculations including the exo-anomeric effect showed a bend in the saccharide chain at this disaccharide with a largely hydrophobic surface of the convex side, probably being part of the binding epitope. Mixtures of glycolipids isolated from a human ureter scraping and from urinary sediments bound bacteria in the 2- to 7-sugar interval. Thus, this infectious strain of E. coli recognizes glycolipids being present in epithelial cells lining the urinary tract.

  • Structural identification of two ten-sugar branched chain glycosphingolipids of blood group H type present in epithelial cells of rat small intestine.

    • Two novel blood group H-type decaglycosylceramides with a branched core saccharide have been identified in mixture in a fraction isolated from rat small intestine. They were present exclusively in the epithelial cells. The number and sequence of sugars were established by direct inlet mass spectrometry of the permethylated and LiAlH4-reduced permethylated derivatives. Gas-liquid chromatography of the products after degradation of the native and permethylated glycolipids gave the type of sugars and the binding positions. A di- and a trisaccharide were identified by mass spectrometry after degradation of the permethylated-reduced derivative. One trisaccharide had the structure (formula see text) and was therefore additional evidence for a branched structure. Treatment of the decaglycosylceramide fraction with alpha-L-fucosidase gave free fucose and an octaglycosylceramide identified by mass spectrometry. Proton NMR spectra of the permethylated and permethylated-reduced octa- and decaglycosylceramides provided evidence for the binding configurations and the localization of type 1 and type 2 sequences in the two branches. The 3-linked branch was homogeneous with a type 1 saccharide (Gal beta 1 leads to 3GlcNAc) but the 6-linked branch had both type 1 and type 2 (Gal beta 1 leads to 4GlcNAc) sequences. Two glycolipids with the following probable structures were therefore present, making up 60 and 40% of the mixture, respectively: (formula see text) The lipophilic part contained mainly trihydroxy 18:0 long chain base (phytosphingosine) and 16:0 to 24:0 nonhydroxy fatty acids.
