  • Predictors for work ability and disability in men and women with low-back or neck problems

    • In many countries back and neck problems are the most common reason for work incapacity. Numerous studies have looked for reliable predictors of return to work (RTW) or not return to work (NRTW) for those incapacitated, but with few exceptions their predictive and detective ability has been limited. We aimed to perform a comparative study of various commonly used health measures ability to predict RTW or NRTW in a cohort of men and women sick-listed for more than 28 days due to low-back pain (LBP) or neck pain (NP). A cohort of 1,575 men and women sick-listed more than 28 days due to back or neck problems was asked to answer ten different health measures (scales) about various aspects of health status (health related quality of life, pain, back function and depressivity) at four occasions during a 2-year period. A statistical diagnostic test was used to study how well the subject's values on these scales would serve as a predictor for work resumption. There was an improvement from day 28 up until 2 years, reflected by the different scales, more pronounced for LBP than for NP patients and for men with LBP compared with women with LBP. For all separately tested scales EuroQol (EQ-5D) had the highest overall ability to predict RTW or NRTW irrespective of gender, diagnoses or duration of the problems. When RTW or NRTW were predicted in a cohort of sick-listed low-back or neck patients, EQ-5D had outstanding properties in this respect irrespective of gender, diagnosis or elapsed time during this 2-year study.

    • LKAB Malmberget is a Swedish mining site located at Malmberget, Sweden. Seven boiler centers are located on north part of Malmberget. There are no connections in between these boiler centers, meaning that it is a decentralized heating system. The heat generated is to heat up buildings and for mine ventilation mainly during the cold periods. The heat is mainly provided from electric and oil boilers. However, most boilers under use are over 20 years old, and it is time to retrofit the boiler system and infrastructure. The purpose of this work is to design and optimize the heating system by the integrated concept. An optimization model based on the mixed integer linear programming (MlLP) has been developed to minimize the total heat production cost, including operation cost and investment cost. The model can be used to calculate how a given heat demand can be satisfied at the lowest possible cost. Four different technical options have been considered in a new centralized heating system. On the basis of data input and assumptions, modeling results indicate that a lower cost could be achieved when a waste heat recovery boiler is installed at the old pelletization plant to recover sensible heat from flue gas. This will lead to a remarkable electricity saving (around 70% saving) compared to the reference case. It has also been noticed that in the optimized cases, oil boilers should not be operated due to a higher price compared to electricity, and by estimation an annual reduction of 3000 ton CO2 and 4.5 ton NOx could be achieved.

  • Adequate iodine nutrition in Sweden: a cross-sectional national study of urinary iodine concentration in school-age children.

    • Background/Objectives:Sweden has a long-standing salt iodization program; however, its effects on iodine intake have never been monitored on a national level. The objective of this study was to evaluate iodine nutrition in the Swedish population by measuring the urinary iodine concentration (UIC) in a national sample of Swedish school-age (6-12 years of age) children.Subjects/Methods:A stratified probability proportionate to size cluster sampling method was used to obtain a representative national sample of school-age children from 30 clusters. Spot urine samples were collected for UIC analysis using a modified Sandell-Kolthoff method.Results:The median UIC of the children (n=857) was 125 mug/l (range 11-757 mug/l). The proportion of children with a UIC <100 mug/l was 30.0% and the proportion of children with a UIC <50 and >300 mug/l was 5.5 and 3.0%, respectively.Conclusions:The iodine nutritional status of the Swedish population is adequate. Iodized table salt remains the main dietary source of iodine in Swedish diet. Recommendations to reduce total salt intake in the population urge increased use of iodized salt in the production of processed foods. Pregnant and lactating women with high iodine requirements may still be at risk for low iodine intake.     • Mg-chelatase catalyzes the insertion of Mg2+ into protoporphyrin IX at the first committed step of the chlorophyll biosynthetic pathway. It consists of three subunits: I, D, and H. The I subunit belongs to the AAA protein superfamily ( ATPases associated with various cellular activities) that is known to form hexameric ring structures in an ATP-dependant fashion. Dominant mutations in the I subunit revealed that it functions in a cooperative manner. We demonstrated that the D subunit forms ATP-independent oligomeric structures and should also be classified as an AAA protein. Furthermore, we addressed the question of cooperativity of the D subunit with barley (Hordeum vulgare) mutant analyses. The recessive behavior in vivo was explained by the absence of mutant proteins in the barley cell. Analogous mutations in Rhodobacter capsulatus and the resulting D proteins were studied in vitro. Mixtures of wild-type and mutant R. capsulatus D subunits showed a lower activity compared with wild-type subunits alone. Thus, the mutant D subunits displayed dominant behavior in vitro, revealing cooperativity between the D subunits in the oligomeric state. We propose a model where the D oligomer forms a platform for the stepwise assembly of the I subunits. The cooperative behavior suggests that the D oligomer takes an active part in the conformational dynamics between the subunits of the enzyme.

    • Aeolian mineral dust archived in Antarctic ice cores represents a key proxy for Quaternary climate evolution. The longest and most detailed dust and climate sequences from polar ice are provided today by the Vostok and by the EPICA-Dome C (EDC) ice cores. Here we investigate the geographic provenance of dust windborne to East Antarctica during Early and Middle Pleistocene glacial ages using strontium and neodymium isotopes as tracers. The isotopic signature of Antarctic dust points towards a dominant South American origin during Marine Isotopic Stage (MIS) 8, 10, 12, and back to MIS 16 and 20 as deduced from EDC core. Data provide evidence for a persistent overall westerly circulation pattern allowing efficient transfer of dust from South America to the interior of Antarctica over the last 800 kyr. Some small but significant dissimilarity between old and recent glacial ages suggests a slightly reduced Patagonian contribution during ancient glaciations.

    • Background: Childhood pre-B acute lymphoblastic leukemia (ALL) is a bone marrow (BM) derived disease, which often disseminates out of the BM cavity, where malignant cells to a variable degree can be found circulating in the peripheral blood (PB). Normal pre-B cells are absolutely dependent on BM stroma for survival and differentiation. It is not known whether transformed pre-B ALL cells retain any of this dependence, which possibly could impact on drug sensitivity or MRD measurements. Results: Pre-B ALL cells, highly purified by a novel method using surface expression of CD19 and immunoglobulin light chains, from BM and PB show a very high degree of similarity in gene expression patterns, with differential expression of vascular endothelial growth factor (VEGF) as a notable exception. In addition, the cell sorting procedure revealed that in 2 out of five investigated patients, a significant fraction of the malignant cells had matured beyond the pre-B cell stage. Conclusion: The transition of ALL cells from the BM into the circulation does not demand, or result in, major changes of gene expression pattern. This might indicate an independence of BM stroma on the part of transformed pre-B cells, which contrasts with that of their normal counterparts. © 2008 Hansson et al, licensee BioMed Central Ltd.

  • The beneficial effect of water vapour on the oxidation at 600 and 700 °C of a MoSi2-based composite

    • The oxidation characteristics of a MoSi2-based composite in O2 and O2+10% H2O at 600 and 700 °C were investigated. The effects of temperature and water vapour on oxidation were examined. The oxidation kinetics were studied using a thermobalance and furnace exposure, while the morphologies and compositions of the oxides were examined using XRD, ESEM/EDX, and SEM/EDX. We propose that oxidation proceeds by the initial formation of MoO3 crystals and amorphous SiO2 on the surface. The MoO3 is then evaporated; as volatile (MoO3)3 species in O2 and additional MoO2(OH)2 species in O2+10% H2O, which results in a porous, Mo-depleted oxide. However, the pores in the Mo-depleted SiO2 scale heal, and a protective crystalline scale is established eventually. The vapour pressures of the abovementioned volatile species increase with temperature and/or water vapour content in the atmosphere, which leads to accelerated Mo depletion from the oxide scale. A shorter time elapses before the oxide scale is transformed into the relatively Mo-free protective SiO2 scale, which results in less oxide being formed. Thus, the formed scale becomes thinner in O2+10% H2O than in O2. Thereby the Mo removal is beneficial when water vapour is added to the exposure atmosphere.

    • OBJECTIVES: X-ray fluorescence (XRF) is a non-invasive method for determining the iodine content of the thyroid gland in vivo. In spite of the obvious clinical value of such a method in situations of iodine deficiency or iodine overload, the method has not so far been widely used. The objective was to investigate the applicability of the XRF method in a larger number of subjects. DESIGN AND SUBJECTS: The study comprised 37 individuals, aged 60-65 years, who had spent their entire life with iodine supplementation through iodinated table salt. Individuals with (previous) thyroid disease were excluded. The individual thyroid function had previously been evaluated by measurements of thyroid-related hormones, thyroid volume and 131-Iodine (131I) uptake which indicated a sufficient iodine intake of the population in the area. Iodine in the right thyroid lobe in each subject was examined using XRF. RESULTS: The mean thyroid iodine concentration was 0.4 mg mL(-1), corresponding to a mean total iodine content of 5.2 mg (range 0.9-20.2). There was a pronounced difference between individuals. No correlation was found between iodine concentration and 131I uptake or thyroid volume. Neither was iodine content and 131I uptake correlated. CONCLUSIONS: In a population living under iodine-sufficient conditions, a large variation of iodine stored in the thyroid is compatible with euthyroidism. Determination of the iodine pool by XRF investigation is feasible in a clinical setting and the method offers a unique possibility to study the intrathyroidal iodine pool in subjects with thyroid disease. The low radiation dose enables the use of the method in pregnant women and also in young individuals.
