  • Long-term safety and survival outcomes from the Scandinavian Breast Group 2004-1 randomized phase II trial of tailored dose-dense adjuvant chemotherapy for early breast cancer

    • Although adjuvant polychemotherapy improves outcomes for early breast cancer, the significant variability in terms of pharmacokinetics results in differences in efficacy and both short and long-term toxicities. Retrospective studies support the use of dose tailoring according to the hematologic nadirs. The SBG 2004-1 trial was a randomized feasibility phase II study which assessed tailored dose-dense epirubicin and cyclophosphamide (EC) followed by docetaxel (T) (group A), the same regimen with fixed doses (group B) and the TAC regimen (group C). Women aged 18-65 years, ECOG PS 0-1 with at least one positive axillary lymph node were randomized 1:1:1. The primary endpoint of the study was the safety and feasibility of the treatment. Toxicity was graded according to CTC-AE version 3.0. The design and short-term toxicity have been previously published. Here, we report safety and efficacy data after 10 years of follow-up. A total of 124 patients were included in the study. After a median follow-up of 10.3 years, the probability for 10-year survival was 78.5, 75.1, and 63.4% and for relapse free survival 64.1, 71.0, and 59.5% for groups A, B, and C, respectively. There were no cases of clinically diagnosed cardiotoxicity or hematologic malignancies. No patient was lost to follow-up. In this randomized phase II trial, tailored dose adjuvant chemotherapy was feasible, without an increased risk for long-term adverse events after a median follow-up of 10 years.

  • Validation of SmartPill® wireless motility capsule for gastrointestinaltransit time : Intra-subject variability, software accuracy and comparison with video capsule endoscopy

    • BACKGROUND: There is interest in ultimately combining endoscopy and motility assessments. Gastric emptying (GET), small bowel (SBTT), colon (CTT) and whole gut transit (WGTT) times are conveniently obtained by SmartPill® wireless motility capsule (WMC) that records luminal pH, temperature and pressure. Reproducibility within same subjects and accuracy of software derived times (MotiliGI® ) were investigated for diagnostic application. GET and SBTT were separately measured using video capsule endoscopy (VCE). The aim of this investigation was to assess same subject reproducibility of WMC, accuracy of software derived transit times and relate to Pillcam® SB (small bowel) VCE motility data.METHODS: Seventy three healthy adults ingested a 260 kcal mixed meal followed by WMC tests. Food intake was permitted after 6 hours. Regional transit data was obtained for GET, SBTT and CTT, the sum yielding WGTT. Nineteen subjects repeated WMC tests 2 or 4 weeks later; a separate 70 underwent VCE while fasted.KEY RESULTS: Visually derived data from WMC yielded GET 3.46±0.27, SBTT 5.15±0.21, CTT 20.76±1.19 and WGTT 29.53±1.28 hours (mean±SEM). Pearson's correlation coefficients (r) against software derived results were: GET 0.78 (P<.0001), SBTT 0.28 (P<.05), CTT 0.96 (P<.0001), WGTT 0.99 (P<.0001). VCE yielded lower GET (0.71±0.08 hours) and SBTT (4.15±0.13 hours).CONCLUSIONS AND INFERENCES: GET, SBTT, CTT and WGTT obtained by WMC are commensurate with literature values, including by other methods. Visually and software derived transit times have strongest correlations for CTT and WGTT. WMC yields longer GET and SBTT than VCE, perhaps due to meal related effects on motility.

  • Improved refining energy efficiency in thermo-mechanical pulping by means of collimated wood chipping – from solid mechanics to full scale evaluation

    • The wood chipping process was never optimized with regard to high yield pulping processes as thermomechanical pulping (TMP) and chemithermomechanical pulping (CTMP). It is generally believed that wood chips for pulping should be produced in such a way that the degree of damage is minimized and that the chip dimensional distribution should be as narrow as possible. Since the TMP and CTMP processes were developed in the 60-ies and 70-ies, compression screw as well as roll nip equipment have been developed to pretreat wood chips as a way to reduce refining energy consumption to given fiber and pulp properties and also in order to improve impregnation. The general conclusions are that a combination of shear and compression in the tangential or radial direction of the wood initiates cracks that later in the refiner will enhance and optimize fiber separation and also fiber property development. The idea with the collimated chipping technology is to utilize the wood chipper as a tool, combining cutting of wood logs to wood chips with a pretreatment of the chips by creating cracks that would enhance fiber separation, fiber surface development as well as chip-impregnation. In this case the compression is performed in the wood fiber direction, in which direction wood actually is weakest when it comes to compression induced cracking. The maximization of the amount of cracks in wood-chips is performed by optimizing the knife angle (or spout angle) in the chipper, to what we call collimated chipping (according to a patent owned by CCT AB). This presentation describes a theoretical background and two demonstration scale studies performed by SCA Forest Products at their Ortviken mill. One conclusion drawn, based on two-month test period with three weeks of collimated chipping, was that specific refining energy reduction was around 100 kWh/adt of the 1400 kWh/adt used in primary stage double disc refining. Most probably the potential is higher if the whole system is optimized. Tests were performed at constant production rate and energy was reduced by reducing power to constant freeness, leading to similar tensile and light scattering levels. The deliberately increased forces created in the wood chipper by means of an optimized (increased) edge angle caused more problems with knife holder equipment than normally, as well as increased vibrations. These problems will have to be solved for future long-term implementation of the technique.

  • The NIMO Scandinavian Study : A Prospective Observational Study of Iron Isomaltoside Treatment in Patients with Iron Deficiency

    • Background. Intravenous iron allows for efficient and well-tolerated treatment in iron deficiency and is routinely used in diseases of the gastrointestinal tract. Objective. The aims of this study were to determine the probability of relapse of iron deficiency over time and to investigate treatment routine, effectiveness, and safety of iron isomaltoside. Methods. A total of 282 patients treated with iron isomaltoside were observed for two treatments or a minimum of one year. Results. Out of 282 patients, 82 had Crohn's disease and 67 had ulcerative colitis. Another 133 patients had chronic blood loss, malabsorption, or malignancy. Patients who received an iron isomaltoside dose above 1000 mg had a 65% lower probability of needing retreatment compared with those given 1000 mg. A clinically significant treatment response was shown, but in 71/191 (37%) of patients, anaemia was not corrected. The mean dose given was 1100 mg, lower than the calculated total iron need of 1481 mg. Adverse drug reactions were reported in 4% of patients. Conclusion. Iron isomaltoside is effective with a good safety profile, and high doses reduce the need for retreatment over time. Several patients were anaemic after treatment, indicating that doses were inadequate for full iron correction.

    • A feldspar-porphyric Svecofennian rhyolite associated with Mn-mineralisations at Ultevis, west of Jokkmokk, northern Sweden, has been dated to 1872 ± 9 Ma. This figure also gives a maximum age of deposition of the stratigraphically overlaying sedimentary rock sequence (the Snavva–Sjöfall group). The position of Mn-Fe-Ba-As-F-U-(W-Mo) mineralisations is discussed in relation to the transition from volcanic to sedimentary environments in the Tjåmotis–Ultevis area.

    • The Swedish Strategic Innovation Agenda for Geoenergy develops a strategy for highlighting the need for geoenergy research, development and innovation in relation to the business community and authorities. Sweden has a strong global position in the field. Geothermal energy is the third­largest renewable energy source in Sweden, along with wind power. Currently, approximately 18 TWh of renewable heat per year is supplied to household, industrial and commercial buildings. In addition, there is 1­2 TWh of cooling. There is a great need for a long­term strategy and financial plan for geoenergy research and innovation in Sweden, in order to maintain and further develop research groups and the country's leading position in the field. Today, Swedish research in this field comprises about 14 full­time positions at universities, institutes and companies. This is little compared to the existing use of geothermal energy, about 19 TWh, which is worth about 15 billion SEK in consumer sales. It is also little in relation to possible future contributions to sustainable development in Sweden and abroad. The agenda is divided into short independent chapters that are tailored to different target groups/ applications. Each chapter consists of an introduction to the current situation, how geoenergy can strategically contribute to a sustainable society, and what actions/needs are a priority.

    • Epitaxial Ge with low dislocation density is grown on a low temperature grown Ge seed layer on Si substrate by reduced pressure chemical vapor deposition. The surface topography measured by AFM shows that the strain relaxation occurred through pit formation which resulted in freezing the defects at Ge/Si interface. Moreover a lower threading dislocation density compared to conventional strain relaxed Ge buffers on Si was observed. We show that by growing the first layer at temperatures below 300 °C a surface roughness below 1 nm can be achieved together with carrier mobility enhancement. The different defects densities revealed from SECCO and Iodine etching shows that the defects types have been changed and SECCO is not always trustable.

    • A low temperature (T-max = 350 degrees C) process for Germanium (Ge) on insulator (GOI) substrate fabrication with thicknesses of less than 25 nm is reported in this paper. The process is based on a single step epitaxial growth of a Ge/SiGe/Ge stack on Si, room temperature wafer bonding and an etch-back process using Si0.5Ge0.5 as an etch-stop layer. GOI substrates with surface roughness below 0.5 nm, 0.15% tensile strain, thickness nonuniformity of less than 3 nm and residual p-type doping of less than 1016 cm(-3) were fabricated. Ge pFETs are fabricated (T-max = 600 degrees C) on the GOI wafer with 70% yield. The devices exhibit a negative threshold voltage of -0.18 V and 60% higher mobility than the SOI pFET reference devices.
