  • Primary lymph node Gastrinoma or occult duodenal microgastrinoma with lymph node Metastases in a MEN1 patient - The need for a systematic search for the primary tumor
  • 2008
  • Ingår i: American Journal of Surgical Pathology. - 0147-5185 .- 1532-0979. ; 32:7, s. 1101-1105
  • Tidskriftsartikel (refereegranskat)abstract
    • Gastrinoma tissue has been found frequently in lymph nodes located near the duodenum without a known primary tumor. Therefore, it has been suggested that a primary lymph node gastrinoma exists. We report on a 38-year-old woman suffering from multiple endocrine neoplasia type 1 (MEN 1) confirmed by menin gene mutation analysis. MEN1 disease started with primary hyperparathyroidism followed by Cushing disease, the detection of tumors of the pituitary, adrenal cortex, and the pancreas and also an elevated serum gastrin level. An octreotide scan revealed 4 tumors in the upper abdomen. A selective arterial calcium stimulation test located the source of the hypergastrinemia to the area of the gastroduodenal and the superior mesenteric arteries. Total pancreatoduodenectomy was performed and conventional histopathologic examination revealed a well-differentiated cystic neuroendocrine tumor of the pancreas expressing glucagon and accompanied by several microadenomas. In addition, 3 suprapancreatic lymph nodes with gastrin-positive endocrine tissue were found. None of the pancreatic microadenomas expressed gastrin and no duodenal endocrine tumor was found despite careful macroscopic examination. Only after complete embedding of the duodenal and pancreatic tissue in 65 paraffin blocks, 2 microgastrinomas (0.45 and 0.8 mm in diameter) were identified in the duodenum. It is concluded that duodenal gastrinomas that give rise to lymph node metastases may be so tiny that they are easily overlooked in a routine examination and that systematic tissue monitoring is required to identify them.

  • Sporadic versus hereditary gastrinomas of the duodenum and pancreas : distinct clinico-pathological and epidemiological features.
  • 2006
  • Ingår i: World Journal of Gastroenterology. - 1007-9327 .- 2219-2840. ; 12:34, s. 5440-6
  • Tidskriftsartikel (refereegranskat)abstract
    • Gastrinomas are defined as gastrin secreting tumors that are associated with Zollinger-Ellison syndrome (ZES). ZES is characterized by elevated fasting gastrin serum levels, positive secretin stimulation test and clinical symptoms such as recurrent peptic ulcer disease, gastroesophageal reflux disease and occasional diarrhea. Genetically, nonhereditary (sporadic) gastrinomas are distinguished from hereditary gastrinomas, which are associated with multiple endocrine neoplasia type 1 (MEN1) syndrome. In general, duodenal gastrinomas are small and solitary if they are sporadic and multiple as well as hereditary. The sporadic gastrinomas occur in the duodenum or in the pancreas while the hereditary gastrinomas almost all occur in the duodenum. Our series of 77 sporadic duodenal neuroendocrine tumors (NETs) includes 18 patients (23.4%) with gastrinomas and ZES. Of 535 sporadic NETs in the pancreas collected from the NET archives of the departments of pathology in Zurich, Switzerland, and Kiel, Germany, 24 patients (4.5%) suffered from sporadic pancreatic gastrinomas and ZES. These NETs have to be distinguished from tumors with immunohistochemical positivity for gastrin but without evidence of ZES. An additional 19 patients suffered from MEN1 and ZES. These patients showed exclusively duodenal gastrinomas, but not pancreatic gastrinomas. The prognosis of sporadic and MEN1-associated duodenal gastrinomas is better than that of pancreatic gastrinomas, since they progress slowly to liver metastasis. In summary, sporadic and MEN1-associated gastrinomas in the duodenum and pancreas show different clinico-pathological and genetic features. The incidence of sporadic duodenal gastrin-producing tumors is increasing, possibly due to optimized diagnostic procedures. In contrast, pancreatic MEN1-associated gastrinomas seem to be extremely rare. A considerable subset of tumors with immunohistochemical expression of gastrin but without evidence of ZES should be designated as functionally inactive NETs expressing gastrin, but not as gastrinomas.

  • On input-revolving deterministic and nondeterministic finite automata
  • 2009
  • Ingår i: Information and Computation. - 0890-5401 .- 1090-2651. ; 207, s. 1140-1155
  • Tidskriftsartikel (refereegranskat)abstract
    • We introduce and investigate input-revolving finite automata, which are (nondeterministic) finite state automata with additional ability to shift the remaining part of the input. Three different modes of shifting are considered, namely revolving to the left, revolving to the right, and circular-interchanging. We investigate the computational capacities of these three types of automata and their deterministic variants, comparing any of the six classes of automata with each other and with further classes of well-known automata. In particular, it is shown that nondeterminism is better than determinism, that is, for all three modes of shifting there is a language accepted by the nondeterministic model but not accepted by any determinstic automaton of the same type. Concerning the closure properties most of the deterministic language families studied are not closed under standard operations. For example, we show that the family of languages accepted by deterministic right-revolving finite automata is an anti-AFL which is not closed under reversal and intersection.

  • A ""tabletop"" electrostatic ion storage ring : Mini-Ring
  • 2008
  • Ingår i: Review of Scientific Instruments. - : AIP Publishing. - 0034-6748 .- 1089-7623. ; 79:7, s. 75109-
  • Tidskriftsartikel (refereegranskat)abstract
    • We report on the design, construction, and commissioning of a novel electrostatic ion storage ring of small dimensions-in the following referred to as ""Mini-Ring."" Mini-Ring consists of four horizontal parallel-plate deflectors and two conical electrostatic mirrors. Ions are injected through the two deflectors on the injection side and off axis with respect to the conical mirrors which face each other. The first injection deflector, originally at zero voltage, is switched to its set value such that the ions after one turn follow stable trajectories of lengths of roughly 30 cm. This design reduces the number of electrodes necessary to guide the ion beam through the ring in stable orbits. The six elements (deflectors and mirrors) are placed on a common grounded plate-the tabletop. Here, we present the design, ion trajectory simulations, and results of the first test experiments demonstrating the successful room-temperature operation of Mini-Ring at background pressures of 10(-6)-10(-7) mbar.

  • Endocranial Morphology and Phylogeny of Palaeozoic Gnathostomes (Jawed Vertebrates)
  • 2008
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • Gnathostomes, or jawed vertebrates, make up the overwhelming majority of modern vertebrate diversity. Among living vertebrates, they comprise the chondrichthyans (“cartilaginous fishes” such as sharks, skates, rays, chimaeras) and the osteichthyans (“bony fishes” or bony vertebrates, inclusive of tetrapods). Gnathostomes appear to have originated in the early Palaeozoic Era, but their early fossil record is fairly scant. The best fossils appear first in the Late Silurian and Devonian periods. Much of gnathostome diversity owes to unique adaptations in the internal skeleton of their head (the endocranium). The endocranium is composed of the braincase, jaws, hyoid arch, and branchial arches, which sometimes fossilise when they are composed of bone or calcified cartilage. The purpose of this thesis is to describe and compare the fossilised cranial endoskeletons of a variety of different Palaeozoic gnathostomes. The objective is to test current conceptions of gnathostome interrelationships (i.e. phylogeny) and infer aspects of key morphological transformations that took place during the evolution of Palaeozoic members of this group. Two key areas are examined: the morphology and interrelationships of Palaeozoic gnathostomes and the morphology of the visceral arches in sarcopterygian fishes. New data on the visceral arches are described from the stem tetrapods Panderichthys and rhizodontids. These provide insight into the sequence of character acquisition leading to the tetrapod middle ear. Panderichthys shows key features of the tetrapod middle ear chamber were established prior to the origin fo digited limbs. New morphological data are described from the “acanthodian” fish Ptomacanthus. Ptomacanthus provides only the second example of a well-preserved braincase from any member of this group. It shows dramatic differences from that of its counterpart, Acanthodes, providing new evidence for acanthodian paraphyly. New interpretations of basal gnathostome and osteichthyan phylogeny are presented, challenging or enriching existing views of these problems.
