    • The combination of melphalan and prednisone (MP) has been the standard treatment of multiple myeloma (MM). Since the introduction of novel agents, the clinical outcome in MM has improved. Several prospective studies with thalidomide combined with MP (MPT) compared to MP have been performed, most of them showing that MPT gives a better response rate and median overall survival (OS). Among 1843 MM patients admitted to 15 Swedish centres, we selected all patients treated with MP and MPT in 1(st) , 2(nd) , 3(rd) or 4(th) line of therapy, in total 888 patients treated with MP and 274 with MPT. Patients were evaluated for response rate, OS and TTNT. Multivariate Cox model analysis was made to adjust for different criteria at time for MM-diagnosis. The median OS from beginning of 1(st) line of treatment was 2.2/4.2 years after MP/MPT respectively, and in 2(nd) , 3(rd) and 4(th) line of treatment 1.8/2.9, 1.4/1.6 and 1.1/1.9 years (P<0.0001, 0.003, 0.74 and 0.235). The relative risk for death in the MPT group vs. the MP group was 0.61 (95% CI: 0.45-0.84) in 1(st) and 0.55 (0.38-0.83), p<0.01) in 2(nd) line. Treatment with MPT gave a significantly better overall survival rate after both 1(st) and 2(nd) line of therapy compared to treatment with MP only. This article is protected by copyright.   • Adaptive stabilized finite element framework for simulation of vocal fold turbulent fluid-structure interaction

    • As a step toward building a more complete model of voice production mechanics, we assess the feasibility of a fluid-structure simulation of the vocal fold mechanics in the Unicorn incompressible Unified Continuum framework. The Unicorn framework consists of conservation equations for mass and momentum, a phase function selecting solid or fluid constitutive laws, a convection equation for the phase function and moving mesh methods for tracking the interface, and discretization through an adaptive stabilized finite element method. The framework has been validated for turbulent flow for both low and high Reynolds numbers and has the following features: implicit turbulence modeling (turbulent dissipation only occurs through numerical stabilization), goal-oriented mesh adaptivity, strong, implicit fluid-structure coupling and good scaling on massively parallel computers. We have applied the framework for turbulent fluid-structure interaction simulation of vocal folds, and present initial results. Acoustic quantities have been extracted from the framework in the setting of an investigation of a configuration approximating an exhaust system with turbulent flow around a flexible triangular steel plate in a circular duct. We present some results of the investigation as well as results of the framework applied to other problems.

  • Plasma vitamin D and mortality in older men : a community-based prospective cohort study

    • Background: Vitamin D status is known to be important for bone health but may also affect the development of several chronic diseases, including cancer and cardiovascular diseases, which are 2 major causes of death. Objective: We aimed to examine how vitamin D status relates to overall and cause-specific mortality. Design: The Uppsala Longitudinal Study of Adult Men, a community-based cohort of elderly men (mean age at baseline: 71 y; n = 1194), was used to investigate the association between plasma 25-hydroxyvitamin D [25(OH)D] and mortality. Total plasma 25(OH)D was determined with HPLC atmospheric pressure chemical ionization mass spectrometry. Proportional hazards regression was used to compute hazard ratios (HRs). Results: During follow-up (median: 12.7 y), 584 (49%) participants died. There was a U-shaped association between vitamin D concentrations and total mortality. An approximately 50% higher total mortality rate was observed among men in the lowest 10% (<46 nmol/L) and the highest 5% (>98 nmol/L) of plasma 25(OH)D concentrations compared with intermediate concentrations. Cancer mortality was also higher at low plasma concentrations (multivariable-adjusted HR: 2.20; 95% CI: 1.44, 3.38) and at high concentrations (HR: 2.64; 95% CI: 1.46, 4.78). For cardiovascular death, only low (HR: 1.89; 95% CI: 1.21, 2.96) but not high (HR: 1.33; 95% CI: 0.69, 2.54) concentrations indicated higher risk. Conclusions: Both high and low concentrations of plasma 25(OH)D are associated with elevated risks of overall and cancer mortality. Low concentrations are associated with cardiovascular mortality.

    • Nanoparticles of controlled size, well defined shape, pure phase and of clean surfaces are ideal model systems to investigate surface/interfacial reactions. In this study we have explored the possibility of synthesizing TiO2 nanoparticles in the size range of 7–20 nm under well controlled experimental conditions. A simple method based on the hydrolysis of TiCl4 was used to obtain particles having surfaces free from organics. Stable dispersions of TiO2 nanoparticles of various sizes were obtained by optimizing the reaction/dialysis time and temperature. The synthesized TiO2 particles were found to be predominantly of anatase phase and narrow particle size distributions were obtained. The TiO2 particles were characterized with respect to their phase, size and shape by X-ray diffraction (XRD) and transmission electron microscopy (TEM), respectively. Particle size distribution in a colloidal dispersion was obtained by the electrospray scanning mobility particle sizer (ES-SMPS) method and compared with an average particle size determined from dynamic light scattering (DLS). The average particle sizes obtained by the DLS and ES-SMPS methods were in good agreement, while a primary particle size of 4 nm was found in X-ray diffraction irrespective of the particle size in solution. Early stages of the nucleation process were monitored by the ES-SMPS method. These results show that small particles of 4–5 nm are initially formed and it is highly likely that large particles are formed due to aggregation of primary particles.

  • Improvement in the hospital organisation of CPR training and outcome after cardiac arrest in Sweden during a 10-year period

    • Aim To describe (a) changes in the organisation of training in cardiopulmonary resuscitation (CPR) and the treatment of cardiac arrest in hospital in Sweden and (b) the clinical achievement, i.e. survival and cerebral function, among survivors after in-hospital cardiac arrest (IHCA) in Sweden. Methods Aspects of CPR training among health care providers (HCPs) and treatment of IHCA in Sweden were evaluated in 3 national surveys (1999, 2003 and 2008). Patients with IHCA are recorded in a National Register covering two thirds of Swedish hospitals. Results The proportion of hospitals with a CPR coordinator increased from 45% in 1999 to 93% in 2008. The majority of co-ordinators are nurses. The proportions of hospitals with local guidelines for acceptable delays from cardiac arrest to the start of CPR and defibrillation increased from 48% in 1999 to 88% in 2008. The proportion of hospitals using local defibrillation outside intensive care units prior to arrival of rescue team increased from 55% in 1999 to 86% in 2008. During the past 4 years in Sweden, survival to hospital discharge has been 29%. Among survivors, 93% have a cerebral performance category (CPC) score of I or II, indicating acceptable cerebral function. Conclusion During the last 10 years, there was a marked improvement in CPR training and treatment of IHCA in Sweden. During the past 4 years, survival after IHCA is high and the majority of survivors have acceptable cerebral function.

    • Passive leg raising (PLR), to augment the artificial circulation, was deleted from cardiopulmonary resuscitation (CPR) guidelines in 1992. Increases in end-tidal carbon dioxide (P(ET)CO(2)) during CPR have been associated with increased pulmonary blood flow reflecting cardiac output. Measurements of P(ET)CO(2) after PLR might therefore increase our understanding of its potential value in CPR. We also observed the alteration in P(ET)CO(2) in relation to the return of spontaneous circulation (ROSC) and no ROSC.

    • Abstract in Undetermined Packaging is of particular importance to retailers, since it can be considered an integral part of the product and the first point of contact with the brand (Rundh 2005). Over 73% of interviewed consumers rely on packaging to aid their purchasing decisions (Wells et al. 2007), and retailers are the ‘gatekeepers’ to the consumers (Dobson et al. 2003) via the retail stores, where the packaging of a product is what meets the eyes of consumers. Young (2008:26) simply states, “The package is the product”, and packaging “combines the ‘4 Ps’ of marketing: the package contains the product, packages convey messages about product attributes to consumers as part of public relations, and often its price, while also carrying promotions”, making it an integral part of the product (Hawkes 2010:297). Hence, innovations in packaging and packaging systems in the food sector are intimately connected with the contained products; success or failure can be due to either or both aspects. And the success rate of food products is low: 80 to 90% of all launched products fail within the first year, in the USA (Rudolph 1995), with similar figures in other countries. This might be due to shortcomings in the methodology to develop (Stewart-Knox & Mitchell 2003) or that the right business model to “capture value from innovations” has not been designed (Teece 2010:183). And “value exists only if the consumer perceives it as such” (Burt 1989:29). Today, many retailers control the supply chain from producers to consumers (Fernie & Sparks 2009), have expanded their range of differentiated private labels (Burt & Sparks 2002) and increasingly compete with manufacturers’ brands, including in Sweden (Beckeman & Olsson 2011). This has resulted in increased demands for more flexible production to meet a greater variety of packaging sizes, products, recipes and delivery on demand, without increasing The importance of packaging innovations in the Swedish food sector 11 194 Chapter 11 the costs, and consequently smaller order sizes and varying designs (Van Donk 2001; Van Donk et al. 2008). The real breakthrough for packaged food in Sweden came with the introduction of frozen food in 1945 and self-service stores in 1947, both of which demanded packaging (Beckeman 2006). These changes initiated efficient supply chains, which together with a value perspective have become a necessity for the different requirements of various food products (Fisher 1997; Gustafsson et al. 2006). Food and beverages range from dry products to liquids, requiring distribution/storage temperatures from ambient, via refrigerated to frozen. Hence, product demands on packaging vary. The broader background to this chapter can be found in a doctoral thesis (Beckeman 2011) based on interviews with retailers, food manufacturers and packaging suppliers active in Sweden. To our knowledge, no similar investigation of the Swedish food sector of today has been carried out. The purpose was to investigate how the three groups of actors view innovations in their own area, their roles and the roles of other actors in the chain; i.e. if there is a gap of opinions about innovations among them. This chapter summarises the results from interviewing packaging suppliers based on the following research questions: • How do innovative Swedish packaging suppliers define innovations, and how do they regard their own role in food innovations? • What is the nature of the collaboration among packaging suppliers and other actors in the supply chain regarding food innovations? ‘Consumer’ is defined as the end consumer of a food product, whereas a ‘customer’ can be a food manufacturer, a retailer or the next link in the packaging supply chain, as packaging suppliers cannot be defined as one homogenous group. They can be material producers, packaging converters, packaging machinery suppliers and other relevant suppliers (Paine 2002), and can work as partners, sub-suppliers and/or competitors with each other, depending on the situation and the demands. In this mixture of packaging suppliers, some are considered more innovative and successful than others, as previously suggested by interviewed retailers (Beckeman & Olsson 2011) and food manufacturers (Beckeman et al., in press) and are the focus of this study. This chapter is organised as follows: it starts by summarising literature on packaging and packaging functions and related to food innovations, continues with methodology, including framework for analysis, which is followed by results and analysis, and ends with conclusions.

  • A Unique Role of the Cholera Toxin A1-DD Adjuvant for Long-Term Plasma and Memory B Cell Development

    • Adjuvants have traditionally been appreciated for their immunoenhancing effects, whereas their impact on immunological memory has largely been neglected. In this paper, we have compared three mechanistically distinct adjuvants: aluminum salts (Alum), Ribi (monophosphoryl lipid A), and the cholera toxin A1 fusion protein CTA1-DD. Their influence on long-term memory development was dramatically different. Whereas a single immunization i.p. with 4-hydroxy-3-nitrophenyl acetyl (NP)-chicken γ-globulin and adjuvant stimulated serum anti-NP IgG titers that were comparable at 5 wk, CTA1-DD–adjuvanted responses were maintained for >16 mo with a half-life of anti-NP IgG ∼36 wk, but <15 wk after Ribi or Alum. A CTA1-DD dose-dependent increase in germinal center (GC) size and numbers was found, with >60% of splenic B cell follicles hosting GC at an optimal CTA1-DD dose. Roughly 7% of these GC were NP specific. This GC-promoting effect correlated well with the persistence of long-term plasma cells in the bone marrow and memory B cells in the spleen. CTA1-DD also facilitated increased somatic hypermutation and affinity maturation of NP-specific IgG Abs in a dose-dependent fashion, hence arguing that large GC not only promotes higher Ab titers but also high-quality Ab production. Adoptive transfer of splenic CD80+, but not CD80−, B cells, at 1 y after immunization demonstrated functional long-term anti-NP IgG and IgM memory cells. To our knowledge, this is the first report to specifically compare and document that adjuvants can differ considerably in their support of long-term immune responses. Differential effects on the GC reaction appear to be the basis for these differences.
