    • Background. Description of incidence, clinical signs, symptoms, and consequences of eclampsia in Scandinavia, and assessment of substandard care and potential preventability. Methods. A descriptive cohort study including all women giving birth in a 2-year period (mid-1998-mid-2000) in Scandinavia. Notifications of eclampsia cases were obtained from all obstetric units at 3-monthly intervals. All patient files were reviewed, and systematic audit was performed to identify potentially preventable cases by using predefined criteria. Main outcome measures. Signs and symptoms preceding the eclamptic seizure, the standard of medical care, maternal and perinatal morbidity, and mortality were all recorded. Potentially preventable cases through improved care and cases eligible for primary prophylactic magnesium sulfate (MgSO4) were estimated. Results. The incidence of eclampsia was 5.0/10 000 maternities (CI = 4.3-5.7/ 10 000). Eighty-six percent had a diagnosis of pre-eclampsia before the seizure. Nine of 10 had at least one physical complaint before the first seizure, severe headache being the most common symptom, occurring in two-thirds. Most seizures (90%) occurred after admission to hospital. By audit, 89 cases (42%) were classified as having received substandard care. Prophylactic use of magnesium sulfate might have reduced the number of eclampsia cases by 35 (17%). Conclusions. Eclampsia occurred mainly in hospital and the majority of women had symptoms heralding the seizure. In retrospect, nearly half of the cases were found potentially preventable by timely intervention, improved medical care, and systematic use of prophylactic treatment with MgSO4.

  • Psychological predictors of sport injuries among soccer players

    • Between 65 – 91 % of elite soccer players have at least one injury / year (Hägglund, 2007). Several researches have established models that specify psychological factors that could predict sport injuries. Two examples are Rogers and Landers (2005) stress – coping model and Williams and Andersen´s (1998) stress – injury model. The main purpose of the study was to single out significant psychological factors that could lead to an increased injury risk among soccer players. The participants were 152 male and female soccer players (m = 17, 6) studying at soccer high schools in southwest Sweden. Five questionnaires were used STAI, SAS, LESCA, ACSI – 28 and SSP. Continuously injury record was collected by athletic trainers at the school, during a period of six months. The result suggested that there are four significant predictors that in all could explain 23 % of the injuries. The main factors are life event stress, somatic trait anxiety, mistrust and negative coping. These findings are in unison with for example Williams and Andersen´s (1998) stress – injury model and should be considered by coaches when it comes to preventing sport injuries among their athletes.

    • This is a clinical and radiographic review of 87 hips in 79 patients 9 (7-11) years after acetabular revision with extensive use of morselized allograft firmly impacted into localized defects as well as the entire acetabular cavity, followed by insertion of an uncemented hydroxyapatite-coated cup with supplementary screw fixation. The 9-year survival rate for the acetabular implant was 90.5% (95% CI, 83.4%-97.6%) with revision for any reason as end point and 94% (95% CI, 89%-99.1%) with revision for aseptic loosening as end point. Acetabular revision with massive allograft impaction and hydroxyapatite-coated implants show very promising results. The limited contact between implant and host bone does not seem to compromise implant survival in the 7- to 11-year perspective.
