  • Variants in MTNR1B influence fasting glucose levels

    • To identify previously unknown genetic loci associated with fasting glucose concentrations, we examined the leading association signals in ten genome-wide association scans involving a total of 36,610 individuals of European descent. Variants in the gene encoding melatonin receptor 1B (MTNR1B) were consistently associated with fasting glucose across all ten studies. The strongest signal was observed at rs10830963, where each G allele (frequency 0.30 in HapMap CEU) was associated with an increase of 0.07 (95% CI = 0.06-0.08) mmol/l in fasting glucose levels (P = 3.2 x 10(-50)) and reduced beta-cell function as measured by homeostasis model assessment (HOMA-B, P = 1.1 x 10(-15)). The same allele was associated with an increased risk of type 2 diabetes (odds ratio = 1.09 (1.05-1.12), per G allele P = 3.3 x 10(-7)) in a meta-analysis of 13 case-control studies totaling 18,236 cases and 64,453 controls. Our analyses also confirm previous associations of fasting glucose with variants at the G6PC2 (rs560887, P = 1.1 x 10(-57)) and GCK (rs4607517, P = 1.0 x 10(-25)) loci.

  • Sudden cardiac arrest associated with early repolarization

    • BACKGROUND: Early repolarization is a common electrocardiographic finding that is generally considered to be benign. Its potential to cause cardiac arrhythmias has been hypothesized from experimental studies, but it is not known whether there is a clinical association with sudden cardiac arrest.METHODS: We reviewed data from 206 case subjects at 22 centers who were resuscitated after cardiac arrest due to idiopathic ventricular fibrillation and assessed the prevalence of electrocardiographic early repolarization. The latter was defined as an elevation of the QRS-ST junction of at least 0.1 mV from baseline in the inferior or lateral lead, manifested as QRS slurring or notching. The control group comprised 412 subjects without heart disease who were matched for age, sex, race, and level of physical activity. Follow-up data that included the results of monitoring with an implantable defibrillator were obtained for all case subjects.RESULTS: Early repolarization was more frequent in case subjects with idiopathic ventricular fibrillation than in control subjects (31% vs. 5%, P<0.001). Among case subjects, those with early repolarization were more likely to be male and to have a history of syncope or sudden cardiac arrest during sleep than those without early repolarization. In eight subjects, the origin of ectopy that initiated ventricular arrhythmias was mapped to sites concordant with the localization of repolarization abnormalities. During a mean (+/-SD) follow-up of 61+/-50 months, defibrillator monitoring showed a higher incidence of recurrent ventricular fibrillation in case subjects with a repolarization abnormality than in those without such an abnormality (hazard ratio, 2.1; 95% confidence interval, 1.2 to 3.5; P=0.008).CONCLUSIONS: Among patients with a history of idiopathic ventricular fibrillation, there is an increased prevalence of early repolarization.

  • Meta-analysis of 28,141 individuals identifies common variants within five new loci that influence uric acid concentrations

    • Elevated serum uric acid levels cause gout and are a risk factor for cardiovascular disease and diabetes. To investigate the polygenetic basis of serum uric acid levels, we conducted a meta-analysis of genome-wide association scans from 14 studies totalling 28,141 participants of European descent, resulting in identification of 954 SNPs distributed across nine loci that exceeded the threshold of genome-wide significance, five of which are novel. Overall, the common variants associated with serum uric acid levels fall in the following nine regions: SLC2A9 (p = 5.2×10−201), ABCG2 (p = 3.1×10−26), SLC17A1 (p = 3.0×10−14), SLC22A11 (p = 6.7×10−14), SLC22A12 (p = 2.0×10−9), SLC16A9 (p = 1.1×10−8), GCKR (p = 1.4×10−9), LRRC16A (p = 8.5×10−9), and near PDZK1 (p = 2.7×10−9). Identified variants were analyzed for gender differences. We found that the minor allele for rs734553 in SLC2A9 has greater influence in lowering uric acid levels in women and the minor allele of rs2231142 in ABCG2 elevates uric acid levels more strongly in men compared to women. To further characterize the identified variants, we analyzed their association with a panel of metabolites. rs12356193 within SLC16A9 was associated with DL-carnitine (p = 4.0×10−26) and propionyl-L-carnitine (p = 5.0×10−8) concentrations, which in turn were associated with serum UA levels (p = 1.4×10−57 and p = 8.1×10−54, respectively), forming a triangle between SNP, metabolites, and UA levels. Taken together, these associations highlight additional pathways that are important in the regulation of serum uric acid levels and point toward novel potential targets for pharmacological intervention to prevent or treat hyperuricemia. In addition, these findings strongly support the hypothesis that transport proteins are key in regulating serum uric acid levels.

  • Genome-wide association study identifies eight loci associated with blood pressure

    • Elevated blood pressure is a common, heritable cause of cardiovascular disease worldwide. To date, identification of common genetic variants influencing blood pressure has proven challenging. We tested 2.5 million genotyped and imputed SNPs for association with systolic and diastolic blood pressure in 34,433 subjects of European ancestry from the Global BPgen consortium and followed up findings with direct genotyping (N <= 71,225 European ancestry, N <= 12,889 Indian Asian ancestry) and in silico comparison (CHARGE consortium, N 29,136). We identified association between systolic or diastolic blood pressure and common variants in eight regions near the CYP17A1 (P = 7 x 10(-24)), CYP1A2 (P = 1 x 10(-23)), FGF5 (P = 1 x 10(-21)), SH2B3 (P = 3 x 10(-18)), MTHFR (P = 2 x 10(-13)), c10orf107 (P = 1 x 10(-9)), ZNF652 (P = 5 x 10(-9)) and PLCD3 (P = 1 x 10(-8)) genes. All variants associated with continuous blood pressure were associated with dichotomous hypertension. These associations between common variants and blood pressure and hypertension offer mechanistic insights into the regulation of blood pressure and may point to novel targets for interventions to prevent cardiovascular disease.

  • Precision electroweak measurements on the Z resonance

    • We report on the final electroweak measurements performed with data taken at the Z resonance by the experiments operating at the electron-positron colliders SLC and LEP. The data consist of 17 million Z decays accumulated by the ALEPH, DELPHI, L3 and OPAL experiments at LEP, and 600 thousand Z decays by the SLID experiment using a polarised beam at SLC. The measurements include cross-sections, forward-backward asymmetries and polarised asymmetries. The mass and width of the Z boson, m(Z) and Gamma(Z), and its couplings to fermions, for example the p parameter and the effective electroweak mixing angle for leptons, are precisely measured: m(Z) = 91.1875 +/- 0.0021 GeV, Gamma(Z) = 2.4952 +/- 0.0023 GeV, rho(l) = 1.0050 +/- 0.0010, sin(2)theta(eff)(lept) = 0.23153 +/- 0.00016. The number of light neutrino species is determined to be 2.9840 +/- 0.0082, in agreement with the three observed generations of fundamental fermions. The results are compared to the predictions of the Standard Model (SM). At the Z-pole, electroweak radiative corrections beyond the running of the QED and QCD coupling constants are observed with a significance of five standard deviations, and in agreement with the Standard Model. Of the many Z-pole measurements, the forward-backward asymmetry in b-quark production shows the largest difference with respect to its SM expectation, at the level of 2.8 standard deviations. Through radiative corrections evaluated in the framework of the Standard Model, the Z-pole data are also used to predict the mass of the top quark, m(t) = 173(+10)(+13) GeV, and the mass of the W boson, m(W) = 80.363 +/- 0.032 GeV. These indirect constraints are compared to the direct measurements, providing a stringent test of the SM. Using in addition the direct measurements of m(t) and m(W), the mass of the as yet unobserved SM Higgs boson is predicted with a relative uncertainty of about 50% and found to be less than 285 GeV at 95% confidence level.   • Methodology to translate policy assessment problems into scenarios: the example of the SEAMLESS integrated framework

    • Scenario-based approaches in environmental and policy assessment studies are increasingly applied within integrated assessment and modelling frameworks. The SEAMLESS project develops such an integrated framework (SEAMLESS-IF) aiming to assess, ex-ante, impacts of alternative agro-environmental policies on the sustainability of agricultural systems. A particular challenge in this context is the consistent translation of a wide range of policy questions into scenarios that a modelling framework can assess. The present work defines a methodology for scenario-development in integrated policy assessment with specific emphasis on SEAMLESS-IF. After a general overview on scenario concepts for integrated policy assessment the adopted scenario concept and its development procedure is presented. They allow building integrated scenarios capturing the range of drivers of the assessed agricultural system in a consistent way across temporal and spatial scales. Then focus is on the particular procedures to translate the policy assessment questions into scenario parameters and to implement these parameters into SEAMLESS-IF. Two examples targeted at European and regional level combining integrated assessments of policy changes and technological innovations are considered to illustrate the SEAMLESS scenario concept. We conclude that the proposed methodology to translate policy assessment problems into scenarios effectively supports integrated assessment in SEAMLESS-IF or even in other modelling frameworks.   • AURKA F31I polymorphism and breast cancer risk in BRCA1 and BRCA2 mutation carriers: A consortium of investigators of modifiers of BRCA1/2 study

    • The AURKA oncogene is associated with abnormal chromosome segregation and aneuploidy and predisposition to cancer. Amplification of AURKA has been detected at higher frequency in tumors from BRCA1 and BRCA2 mutation carriers than in sporadic breast tumors, suggesting that overexpression of AURKA and inactivation of BRCA1 and BRCA2 cooperate during tumor development and progression. The F31I polymorphism in AURKA has been associated with breast cancer risk in the homozygous state in prior studies. We evaluated whether the AURKA F31I polymorphism modifies breast cancer risk in BRCA1 and BRCA2 mutation carriers from the Consortium of Investigators of Modifiers of BRCA1/2. Consortium of Investigators of Modifiers of BRCA1/2 was established to provide sufficient statistical power through increased numbers of mutation carriers to identify polymorphisms that act as modifiers of cancer risk and can refine breast cancer risk estimates in BRCA1 and BRCA2 mutation carriers. A total of 4,935 BRCA1 and 2,241 BRCA2 mutation carriers and 11 individuals carrying both BRCA1 and BRCA2 mutations was genotyped for F31I. Overall, homozygosity for the 311 allele was not significantly associated with breast cancer risk in BRCA1 and BRCA2 carriers combined [hazard ratio (HR), 0.91; 95% confidence interval (95% CI), 0.77-1.061. Similarly, no significant association was seen in BRCA1 (HR, 0.90; 95% Cl, 0.75-1.08) or BRCA2 carriers (HR, 0.93; 95% CI, 0.67-1.29) or when assessing the modifying effects of either bilateral prophylactic oophorectomy or menopausal status of BRCA1 and BRCA2 carriers. In summary, the F31I polymorphism in AURKA is not associated with a modified risk of breast cancer in BRCA1 and BRCA2 carriers.

    • BACKGROUND: The treatment of ischaemic stroke with neuroprotective drugs has been unsuccessful, and whether these compounds can be used to reduce disability after recurrent stroke is unknown. The putative neuroprotective effects of antiplatelet compounds and the angiotensin II receptor antagonist telmisartan were investigated in the Prevention Regimen for Effectively Avoiding Second Strokes (PRoFESS) trial.METHODS: Patients who had had an ischaemic stroke were randomly assigned in a two by two factorial design to receive either 25 mg aspirin (ASA) and 200 mg extended-release dipyridamole (ER-DP) twice a day or 75 mg clopidogrel once a day, and either 80 mg telmisartan or placebo once per day. The predefined endpoints for this substudy were disability after a recurrent stroke, assessed with the modified Rankin scale (mRS) and Barthel index at 3 months, and cognitive function, assessed with the mini-mental state examination (MMSE) score at 4 weeks after randomisation and at the penultimate visit. Analysis was by intention to treat. The study was registered with ClinicalTrials.gov, number NCT00153062.FINDINGS: 20,332 patients (mean age 66 years) were randomised and followed-up for a median of 2.4 years. Recurrent strokes occurred in 916 (9%) patients randomly assigned to ASA with ER-DP and 898 (9%) patients randomly assigned to clopidogrel; 880 (9%) patients randomly assigned to telmisartan and 934 (9%) patients given placebo had recurrent strokes. mRS scores were not statistically different in patients with recurrent stroke who were treated with ASA and ER-DP versus clopidogrel (p=0.38), or with telmisartan versus placebo (p=0.61). There was no significant difference in the proportion of patients with recurrent stroke with a good outcome, as measured with the Barthel index, across all treatment groups. Additionally, there was no significant difference in the median MMSE scores, the percentage of patients with an MMSE score of 24 points or less, the percentage of patients with a drop in MMSE score of 3 points or more between 1 month and the penultimate visit, and the number of patients with dementia among the treatment groups. There were no significant differences in the proportion of patients with cognitive impairment or dementia among the treatment groups.INTERPRETATION: Disability due to recurrent stroke and cognitive decline in patients with ischaemic stroke were not different between the two antiplatelet regimens and were not affected by the preventive use of telmisartan.

  • A methodology for enhanced flexibility of integrated assessment in agriculture

    • Agriculture is interrelated with the socio-economic and natural environment and faces increasingly the problem of managing its multiple functions in a sustainable way. Growing emphasis is on adequate policies that can support both agriculture and sustainable development. Integrated Assessment and Modelling (IAM) can provide insight into the potential impacts of policy changes. An increasing number of Integrated Assessment (IA) models are being developed, but these are mainly monolithic and are targeted to answer specific problems. Approaches that allow flexible IA for a range of issues and functions are scarce. Recently, a methodology for policy support in agriculture has been developed that attempts to overcome some of the limitations of earlier IA models. The proposed framework (SEAMLESS-IF) integrates relationships and processes across disciplines and scales and combines quantitative analysis with qualitative judgments and experiences. It builds on the concept of systems analysis and attempts to enable flexible coupling of models and tools. The present paper aims to describe progress in improving flexibility of IAM achieved with the methodology developed for SEAMLESS-IF. A brief literature review identifying limitations in the flexibility of IAM is followed by a description of the progress achieved with SEAMLESS-IF. Two example applications are used to illustrate relevant capabilities of SEAMLESS-IF. The examples refer to (i) the impacts on European agriculture of changes in world trade regulations and (ii) regional impacts of the EU Nitrates Directive in combination with agro-management changes. We show that improving the flexibility of IAM requires flexibility in model linking but also a generic set up of all IA steps. This includes problem and scenario definition, the selection and specification of indicators and the indicator framework, the structuring of the database, and the visualization of results. Very important is the flexibility to integrate, select and link models, data and indicators depending on the application. Technical coupling and reusability of model components is greatly improved through adequate software architecture (SEAMLESS-IF uses OpenMI). The use of ontology strongly supports conceptual consistency of model linkages. However, the scientific basis for linking models across disciplines and scales is still weak and requires specific attention in future research. We conclude that the proposed framework significantly advances flexibility in IAM and that it is a good basis to further improve integrated modelling for policy impact assessment in agriculture.   • Localized sources maintaining atrial fibrillation organized by prior ablation

    • Background— Endocardial mapping of localized sources drivingatrial fibrillation (AF) in humans has not been reported.Methods and Results— Fifty patients with AF organizedby prior pulmonary vein and linear ablation were studied. AFwas considered organized if mapping during AF showed irregularbut discrete atrial complexes exhibiting consistent activationsequences for >75% of the time using a 20-pole catheter with5 radiating spines covering 3.5-cm diameter or sequential conventionalmapping. A site or region centrifugally activating the remainingatrial tissue defined a source. During AF with a cycle lengthof 211±32 ms, activation mapping identified 1 to 3 sourcesat the origin of atrial wavefronts in 38 patients (76%) predominantlyin the left atrium, including the coronary sinus region. Electrogramsat the earliest area varied from discrete centrifugal activationto an activity spanning 75% to 100% of the cycle length in 42%of cases, the latter indicating complex local conduction ora reentrant circuit. A gradient of cycle length (>20 ms)to the surrounding atrium was observed in 28%. Local radiofrequencyablation prolonged AF cycle length by 28±22 ms and eitherterminated AF or changed activation sequence to another organizedrhythm. In 4 patients, the driving source was isolated, surroundedby the atrium in sinus rhythm, and still firing at high frequency(228±31 ms) either permanently or in bursts.Conclusions— AF associated with consistent atrial activationsequences after prior ablation emanates mostly from localizedsources that can be mapped and ablated. Some sources harborelectrograms suggesting the presence of localized reentry.
