    • Malignant gastrointestinal stromal tumours (GIST) have a poor prognosis. Since these tumours are resistant to conventional radiation and chemotherapy, surgery has been the mainstay of treatment. However, surgery is usually inadequate for the treatment of malignant GIST. Imatinib, a KIT tyrosine kinase inhibitor, has recently been found to have a dramatic antitumour effect on GIST. In this centre-based study of 17 consecutive patients with high-risk or overtly malignant GIST, imatinib was used in three different settings - palliatively, adjuvantly, and neoadjuvantly. The treatment was found to be safe and particularly effective in tumours with activating mutations of exon 11 of the KIT gene. Clinical response to imatinib treatment correlated morphologically to tumour necrosis, hyalinisation, and reduced proliferative activity. The value of neoadjuvant imatinib treatment was illustrated in one case.

  • Perceived future as a life-span factor in chronic pain: Multidimensional scaling

    • Perceived future has been found to be a significant factor in chronic pain. In this explorative study, a new psychometric instrument, the Future scale was constructed to investigate how chronic pain patients perceive the future. By use of multidimensional scaling a two-dimensional conceptual space was constructed which showed how 107 patients clustered according to the main factors of the scale. Two dimensions were recognized: activity-passivity and preceived obstacles-coping, respectively. Further, the patients were compared to a sample from a normal population (N=104). Significant differences in perceived future related to age between these two groups were found.

    • Perceived (subjective) future has been found to be a significant factor in explaining the relationship between pain and pain-related distress. The present study is based on the assumption that chronic pain patients with the three psychological profiles introduced by Turk and Rudy in 1988 could also be found in a sample of chronic pain patients and if so, these profiles have different perspectives on the future. The Multidimensional Pain Inventory (MPI) and The Future Scale were used to collect data from 569 patients with heterogeneous non-malignant chronic pain. A cluster analysis was conducted, where the resulting clusters closely resembled the profiles labelled by Turk and Rudy as 'dysfunctional', 'interpersonally distressed' and 'adaptive coper'. The results indicated that patients with adaptive coper profile have a more positive perception, while those with an interpersonally distressed profile have a more negative perception of the future. With an increased duration of pain, the proportion of the adaptive coper category decreased linearly, while an opposite trend was noted for the interpersonally distressed category. These results may better enable profiled psychological interventions in clinical pain treatment, e.g. by providing patients with therapies focused on positive future orientation, resulting in increased motivation for health-seeking behaviour and better abilities to cope with pain.

  • Psychological distress and adaptation to chronic pain: symptomatology in dysfunctional, interpersonally distressed, and adaptive copers

    • Objectives: To investigate psychological symptomatology and distress in subgroups of chronic pain patients with different adaptation styles. Methods: Subjects were 660 patients with chronic musculoskeletal pain who were tested by the combined use of the two large and much used psychological inventories – the Multidimensional Pain Inventory and the Symptom Checklist-90-Revised. Results: The results showed significant differences between the three adaptation profiles, dysfunctional, interpersonally distressed, and adaptive copers. Adaptive copers were equally and less distressed than a sample from a normal population. Conclusion: This study calls attention to the risk of blind faith of the 'objectiveness’ of psychometric scales and of the use of them as the sole basis for designing treatments. More attention seems to be needed to get more information about the ‘healthy’ group of adaptive copers by extended clinical judgment.

  • Factors of sense of coherence in chronic pain

    • Dimensions of the Sense of Coherence (SOC) scale were investigated for chronic pain inpatients and a control group (nurse students) (in all, N=737). Different facets of the SOC scale were examined. The patients were categorized according to prototype profiles: dysfunctional, interpersonally distressed and adaptive copers. Four factors of SOC were analyzed by use of confirmatory factor analysis): potentiality, unpredictability, control of events, trust in self and others. Large group differences were found for ratings related to each of the four dimensions.
