Contributions in this special issue cover a wide range of international cases and explore systemic perspectives on economic development placing institutional context and evolutionary dynamics at the core.

  • Results of a curtailed randomized controlled trial, evaluating the efficacy and safety of azimilide in patients with implantable cardioverter-defibrillators : The SHIELD-2 trial

    • BACKGROUND: Frequent hospital attendances in patients with implantable cardioverter-defibrillators (ICDs) result in significant morbidity and health care costs. Current drugs to reduce ICD shocks and hospital visits have limited efficacy and considerable toxicity. We evaluated the efficacy and safety of azimilide, a novel oral class III antiarrhythmic, for use in ICD patients.METHODS: A total of 240 patients were enrolled in a prospective, randomized, double-blind, placebo-controlled trial to evaluate the effect of oral azimilide 75 mg daily in ICD patients with previously documented ventricular tachycardia or ventricular fibrillation, and a left ventricular ejection fraction ≤40%. The primary outcome metric was the adjudicated time-to-first unplanned cardiovascular (CV) hospitalization, or CV emergency department (ED) visit, or CV death. The trial was prematurely discontinued due to withdrawal of study sponsorship.RESULTS: Azimilide demonstrated numerical but statistically nonsignificant reductions in the primary composite outcome (odds ratio [OR] 0.79, 95% CI 0.44-1.44), unplanned CV hospitalizations (OR 0.75, 95% CI 0.41-1.38), ED visits (OR 0.68, 95% CI 0.35-1.31), and all-cause shocks (OR 0.58, 95% CI 0.32-1.05). The incidence of adverse events was lower in the azimilide group. Neutropenia was not observed (absolute neutrophil count <1000 μ/L), and there was one possible torsade de pointes case that led to a successful ICD discharge.CONCLUSION: The SHIELD-2 trial was statistically underpowered due to early trial termination and did not meet its primary objective. Despite this limitation, azimilide showed promise as a safe and effective drug in reducing all-cause shocks, unplanned hospitalizations, and ED visits in ICD patients.

    • Signs in the linguistic landscape (LL) can be used to both inform the publicand to regulate their actions – functions that are highly relevant when communicatingabout risk. This chapter discusses the characteristics of the riskdiscourse carried by the linguistic landscape, more specifically the safetyinformation signage found onboard metro carriages in Stockholm andHelsinki. The analysis deals with both how such information is presentedon the multimodal signs and what kinds of risks and responsibilities aredepicted as part of this information. The results show that emplacement andthe visual design, more specifically the application of colours, are noteworthycharacteristics of risk discourse present in the LL. Few actual risks arespelled out in the signage and even if the readers can be given responsibilityto act in the face of risks, this information can be worded in different ways.Considering interpersonal speech functions, both direct commands andmore ambivalent, passive statements can be found, with some differences inpractices between the two cities. Overall, the situated nature of the safetyinformation signage points to the role of place semiotics in meaning makingwithin risk discourse, thus highlighting the value of considering the LL inorder to gain a better understanding of risk communication.

    • The second chapter starts the journey through the urban LLs of a selection of European cities with an endeavour to apply Umberto Eco’s concept of the model reader to LL studies. It explores language use and the intended audience of signs in Stockholm’s nature reserves, with a focus on the addressees. Simultaneously, it offers insights into an urban sphere that has not attracted much attention in previous studies of urban LLs. The case study, which is based on data collected from the fringes of Sweden’s capital city, opens up stimulating paths for the further study of both the sociolinguistic variation in urban LLs and the (perceived) use of different urban spaces.

    • Using the particle-rotor model in which one high-j quasiparticle is coupled to the core of triaxial shape, wobbling excitations are studied. The family of wobbling phonon excitations can be characterized by: (a) very similar intrinsic structure while collective rotation shows the wobbling feature; (b) strong B(E2;I-->I-1) values for Deltan(w)=1 transitions where n(w) expresses the number of wobbling phonons. For the Fermi level lying below the high-j shell with the most favorable triaxiality gammaapproximate to+20degrees, the wobbling phonon excitations may be more easily identified close to the yrast line, compared with the Fermi level lying around the middle of the shell with gammaapproximate to-30degrees. The spectroscopic study of the yrast states for the triaxial shape with -60degrees<γ<0 are illustrated by taking a representative example with gamma=-30degrees, in which a quantum number related with the special symmetry is introduced to help the physics understanding.
