  • A validated decision model for treating the anaemia of myelodysplastic syndromes with erythropoietin + granulocyte colony-stimulating factor : Significant effects on quality of life

    • We have published previously a prototype of a decision model for anaemic patients with myelodysplastic syndromes (MDS), in which transfusion need and serum erythropoietin (S-Epo) were used to define three groups with different probabilities of erythroid response to treatment with granulocyte colony-stimulating factor (G-CSF) + Epo. S-Epo = 500 U/l and a transfusion need of < 2 units/month predicted a high probability of response to treatment, S-Epo > 500 U/l and =2 units/month for a poor response, whereas the presence of only one negative prognostic marker predicted an intermediate response. A total of 53 patients from a prospective study were included in our evaluation sample. Patients with good or intermediate probability of response were treated with G-CSF + Epo. The overall response rate was 42% with 28.3% achieving a complete and 13.2% a partial response to treatment. The response rates were 61% and 14% in the good and intermediate predictive groups respectively. The model retained a significant predictive value in the evaluation sample (P < 0.001). Median duration of response was 23 months. Scores for global health and quality of life (QOL) were significantly lower in MDS patients than in a reference population, and fatigue and dyspnoea was significantly more prominent. Global QOL improved in patients responding to treatment (P = 0.01). The validated decision model defined a subgroup of patients with a response rate of 61% (95% confidence interval 48-74%) to treatment with G-CSF + Epo. The majority of these patients have shown complete and durable responses.

  • Folate content in strawberries - Effects of storage, ripeness and cultivar

    • Strawberries are a good dietary source of folate and are of interest because of their importance as raw material for the Swedish food industry. The aim of this study was to quantify the total folate content in strawberries, after different conditions of storage, at different grades of ripeness and in different cultivars, using a strictly controlled RPBA-kit, optimised for food analysis. In the study fresh strawberries were stored imitating commercial conditions no losses of folate could be observed. Regarding ripeness, the highest folate concentration was found in unripe strawberries, which differed significantly from ripe and overripe. A significant variation (p<0.01) was found for six different cultivars. All results are given for both fresh and dry matter in order to exclude interference from possible variations in water content.

    • A conceptual schema can be viewed as a language to describe the phenomena in a system to be modelled, i.e. a set of derivation rules and integrity constraints as well as a set of event-rules describing the behaviour of an object system. In this paper, we investigate the relationship between the Lyee software requirements concepts with various constructs in conceptual modelling. Within our work we choose the Unified Modelling Language (UML) as a modelling notation for explaining conceptual models. The result obtained models a fully expressive set of UML and First Order Logic constructs mapped into Lyee concepts.

  • The role of simulation in support of Internet-based energy services

    • The connection of buildings to the Internet broadband network is becoming commonplace. The establishment of such an infrastructure enables the development of a range of new energy, environment and health-related services for people in their homes and workplaces. Telecommunications companies and utilities are actively developing examples of such services, and local authorities and health care providers are supporting trial deployments. Several of the services can be enabled or enhanced through the application of building modelling and simulation. This paper describes the infrastructure for the e-services under test within a European research project and shows the potential for simulation support for these services.

    • Aims The aim of the present analysis was to estimate the cost-effectiveness of intense insulin treatment after acute myocardial infarction in patients with diabetes mellitus based on the results of the Diabetes Mellitus Insulin Glucose Infusion in Acute Myocardial Infarction (DIGAMI) study. In this study 620 patients with diabetes mellitus and acute myocardial infarction were randomized to intense insulin treatment (insulin group) or to serve as controls given standard antidiabetic therapy. Mortality was significantly reduced in the insulin group. Methods and Results The cost-effectiveness ratio was estimated as the incremental cost per life-year and quality-adjusted life-year gained of intense insulin treatment. The incremental costs were estimated as the difference in health care costs and indirect costs (labour production) during the first year of follow-up plus the future costs of increased survival. The life-years gained were based on the 5-year long-term follow-up experience and an assumed annual 20% mortality risk for all patients thereafter. The health care costs were Euro 975 higher in the insulin group during the first year of follow-up, mainly due to a longer period of initial hospitalization related to the institution of multidose insulin. The estimated discounted gain in life-years of the insulin treatment was 0·94 years without and 0·66 with quality of life adjustment, respectively. The cost per life-year gained by intense insulin treatment was Euro 16900 and the cost per quality-adjusted life-year gained was Euro 24100. Thus the estimated cost-effectiveness ratios were relatively low. Conclusion The results of the DIGAMI study indicate that intense insulin treatment after an acute myocardial infarction in patients with diabetes mellitus has an acceptable level of cost-effectiveness.

    • One role of a business system is to provide a representation of a Universe of Discourse, which reflects its structure and behaviour. An equally important function of the system is to support communication within an organisation, by structuring and co-ordinating the actions performed by the organisation's agents. These two roles of a business system may be represented in terms of business and process models, i.e. separating the declarative aspects from the procedural control flow aspects of the system. Although this separation of concerns has many advantages, the differences in representation techniques and focus of the two model types constitute a problem in itself. Abstracting business semantics out of, for instance, technical messaging protocols poses severe problems for business analysts. The main contribution of this paper is a unified framework based on agent oriented concepts for facilitating analysis and integration of business models and process models in a systematic way. The approach suggested bridges the gap between the declarative and social/ economic aspects of a business model and the procedural and communicative aspects of a process model in a technology independent manner. We illustrate how our approach can simplify business and process models integration, process specification, process pattern interpretation and process choreography.

  • Influence of CD4 or CD8 deficiency on collagen-induced arthritis

    • The role of T cells in the mouse collagen-induced arthritis (CIA) model for rheumatoid arthritis is not clarified, and different results have been reported concerning the role of CD4 and CD8 T cells. To address this issue, we have investigated B10.Q mice deficient for CD4 or CD8. The mice lacking CD4 were found to be less susceptible to disease, but not completely resistant, whereas the CD8 deficiency had no significant impact on the disease. No difference in the development of late occurring relapses was noted. Interestingly, the CD4-deficient mice had a severely reduced response to the glycosylated form of the immunodominant type II collagen (CII) 256–270 peptide whereas the response to the non-glycosylated peptide was not significantly different. Furthermore, CD4-deficient mice had lower antibody responses to CII, explaining the lower disease susceptibility. In comparison with previously reported results, it is apparent that the lack of CD4 molecules has a different impact on CIA if present on different genetic backgrounds, findings that could possibly be related to the occurrence of different disease pathways of CIA in different mouse strains.
