  • Fifteen-year survival in prostate cancer : a prospective, population-based study in Sweden

    • OBJECTIVE: To describe the natural history of initially untreated early-stage prostate cancer. A key secondary objective was to calculate long-term survival rates by stage, grade, and age at diagnosis. DESIGN: Prospective cohort study. SETTING: Population-based in 1 county of Sweden, without screening for prostate cancer. PATIENTS: A group of 642 patients with prostate cancer of any stage, consecutively diagnosed between 1977 and 1984 at a mean age of 72 years with complete follow-up to 1994. MAIN OUTCOME MEASURES: Proportion of patients who died from prostate cancer, and 15-year survival (with 95% confidence interval [CI]), corrected for causes of death other than prostate cancer. RESULTS: In the entire cohort, prostate cancer accounted for 201 (37%) of all 541 deaths. Among 300 patients with a diagnosis of localized disease (T0-T2), 33 (11%) died of prostate cancer. In this group, the corrected 15-year survival rate was similar in 223 patients with deferred treatment (81%; 95% CI, 72%-89%) and in 77 who received initial treatment (81%; 95% CI, 67%-95%). The corrected 15-year survival was 57% (95% CI, 45%-68%) in 183 patients with locally advanced cancer (T3-T4) and 6% (95% CI, 0%-12%) in those 159 who had distant metastases at the time of diagnosis. CONCLUSION: Patients with localized prostate cancer have a favorable outlook following watchful waiting, and the number of deaths potentially avoidable by radical initial treatment is limited. Without reliable prognostic indicators, an aggressive approach to all patients with early disease would entail substantial overtreatment. In contrast, patients with locally advanced or metastatic disease need trials of aggressive therapy to improve their poor prognosis.

  • Prostate Cancer Registration in Four Swedish Regions 1996 : Differences in Incidence, Age Structure and Management

    • Introduction: In 1996 registration of prostate cancer in four of the six Swedish regions was started to facilitate evaluation of geographical variations in incidence and treatment.Material and methods: For all cases of prostate cancer, personal identification number, tumour stage, tumour grade and primary treatment were registered.Results: In the four regions covered by the register, 3541 cases of prostate cancer were registered. Altogether there were 5795 cases of prostate cancer diagnosed in Sweden the same year. The age-standardized incidence varied from 89/100 000 to 169/100 000 among counties. The proportion of localized tumours correlated positively to the incidence (p < 0.05) and negatively to mean age at diagnosis (p < 0.01). There was also a significant positive correlation between the proportion of localized tumours and the percentage of patients given curative treatment. All registered variables showed large geographical variations, especially concerning percentage of T1c tumours, treatment of localized tumours and choice of palliative treatment.Conclusion: Diagnostic activity varied considerably among counties, resulting in large variation in age-standardized incidence. High incidence is associated with a larger proportion of localized tumours, which, in turn, is associated with early age at diagnosis. In counties where a policy of detecting tumours early is practised, curative treatment is also given more often. Treatment of localized tumours and preference for palliative treatment seem to depend on local traditions. The lack of cytological and histopathological standards makes geographical comparisons based on tumour grade impossible.

    • Anti-HCV prevalence in treated hemophiliacs, their heterosexual partners, intravenous drug addicts and homosexual men was studied. In hemophiliacs and many of the intravenous drug addicts, greater than or equal to 2 sera drawn 1-18 or 1-17 years apart were available. Anti-HCV testing was performed by ELISA (Ortho). Among patients with severe and moderate hemophilia A, 87% (98/112) were positive for anti-HCV at least once and among patients with severe and moderate hemophilia B, 83% (24/29) were positive for anti-HCV. Seroconversion to anti-HCV was observed in 21% of hemophilia patients. In hemophilia A, HCV infection generally occurred during the first years of life and in hemophilia B somewhat later. Loss of anti-HCV antibody was seen in 12% (17 patients). The rest, 54% (76 patients) were seropositive in first and last samples. All 12 tested spouses to anti-HCV positive men were anti-HCV negative. 80% of the drug addicts (137/172) were seropositive for anti-HCV. In those with greater than 1 serum tested, 8% were consistently negative and 68% consistently positive. 21% seroconverted to anti-HCV while 3% lost antibody. 10% (22/211) of homosexual men were anti-HCV positive. Intravenous transmission of HCV thus seemed highly efficient whereas sexual transmission was much less efficient.

  • Influence of the platinum-support interaction on the direct reduction of NOx under lean conditions

    • Catalysts containing Pt supported on SiC, Al2O3 and ZSM-5 were prepared and studied for NOx reduction by C3H6 in Oz excess under transient (temperature ramps) and steady-state conditions. The maximum NOx reduction activity in the heating ramp experiments was similar for Pt/SiC and Pt/ZSM-5, while Pt/Al2O3 showed higher maximum activity. Both N-2 and N2O formation was observed for all catalysts, although the respective amounts varied with the investigated system. Highest Nz selectivity was observed for Pt/Al2O3. When the NOx reduction activity was studied under steady-state conditions the activity of Pt/Al2O3 decreased substantially (mainly due to a loss in N-2 production). Pt/ZSM-5 became somewhat more selective towards Na production whereas the activity and selectivity of Pt/SiC remained at about the same values as far the heating ramp experiments. Adsorbed species on the surface of the different catalysts were investigated using in-situ FTIR in order to obtain information about the reaction mechanisms. The adsorption of species on Pt/SiC was negligible, while a number of absorption bands were observed for Pt/Al2O3 (N and C containing species, and -NCO) and Pt/ZSM-5 (HC).

  • Dual-broadband rotational cars measurements in an IC engine

    • This is the first report of pure rotational coherent anti-Stokes Raman spectroscopy (CARS) measurementsin an internal combustion (IC) engine. Single-shot, dual-broadband rotational CARS (DB-RCARS) spectra were recorded both prior to ignition and in the postcombustion gases. From these spectra, both temperature and relative oxygen concentrations were evaluated. The pressure was registered simultaneously with the CARS measurements in the spark-ignition engine burning natural gas and air. Prior to ignition, normally at temperatures below 1000 K and pressures below 2 MPa, a rotational CARS spectrum is very temperature sensitive, and the technique can be used for temperature measurements with high accuracy. Evaluated temperatures show a negligible dependence on uncertainties in parameters such as the nonresonant susceptibility of the gas and the slit width. Moreover, no collisional narrowing of the lines has to be taken into account. The relative standard deviation of evaluated temperatures and of relative oxygen concentrations from single-shot measurements were as low as 1, and 1.4-1.9%, respectively. In the postcombustion gases at temperatures above 2000 K and pressures above 1.5 MPa, the nonresonant CARS background gave a large contribution to the total spectrum. In this temperature and pressure range, the evaluated values of temperature and nonresonant susceptibility are not independent, and the nonresonant susceptibility had to be fixed at a precalculated value to get a reliable temperature evaluation. The avantages and disadvantages of rotational CARS in comparison with vibrational CARS for IC engine measurements are discussed.
