  • Reliability of a questionnaire assessing experiences of adolescents in orthodontic treatment

    • OBJECTIVE: To evaluate the reliability of a questionnaire that assessed the expectations and experiences of adolescent patients about orthodontic treatment. MATERIALS AND METHODS: The study included two groups of patients: 30 consecutive patients (19 girls and 11 boys, mean age 14.6 years, SD 2.3 years) naive to orthodontic treatment, and 30 consecutive adolescent patients (17 girls and 13 boys, mean age 15.1 years, SD 2.0 years) in active orthodontic treatment with fixed appliances in both jaws. A questionnaire comprising 46 items was developed, based upon focus group interviews and previous established questionnaires. The questionnaire covered the following domains: Treatment motivation; treatment expectations; pain and discomfort from teeth, jaws, and face; functional jaw impairment; and questionnaire validity. Internal consistency as well as temporal stability with the test-retest method was investigated. RESULTS: A majority of the questions exhibited acceptable test-retest reliability, and composite scores yielded excellent reliability for all domains. Internal consistency was acceptable and good face validity was found for all domains. CONCLUSION: The questionnaire can be recommended for use in the assessment of expectations and experiences of orthodontic treatment.

  • Mandibular jaw movement capacity in 10–17-yr-old children and adolescents : normative values and the influence of gender, age, and temporomandibular disorders

    • The aims of this study were, first, to report normative values for jaw movements in Caucasian children and adolescents (maximum opening, laterotrusion, and protrusion) and, second, to investigate the influence of age, gender, and temporomandibular disorders (TMD) on jaw movement capacity. The population-based study included 1,011 randomly selected German children and adolescents, aged 10-17 yr. Case histories, as well as mandibular movements and the presence of TMD, were assessed according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). The mean +/- standard deviation for maximum opening was 50.6 +/- 6.4 mm, for laterotrusion to the right was 10.2 +/- 2.2 mm, for laterotrusion to the left was 10.6 +/- 2.3 mm, and for protrusion was 8.2 +/- 2.5 mm. Bivariable (t-test) and multivariable (linear regression) analyses showed that normative values for jaw opening capacity were influenced by age and gender. No influence on jaw movement capacity was seen when TMD were present. Based on the distribution of the measurements in the population (lowest decentile), the mandibular mobility of subjects with a maximum opening of < 43 mm and laterotrusive movements < 8 mm or protrusive movements < 5 mm might be considered as being limited. However, these limitations do not necessarily require treatment.

  • Comparison of clinical signs in patients with episodic and chronic tension-type headache attributed to temporomandibular disorders. Preliminary results

    • Aim: This study assessed (1) the sensitivity at trigeminal and non-trigeminal site in TMD patients with episodic (ETTH) and chronic tension type headache (CTTH) compared with healthy controls and (2) the relationship between familiar headache in the temples and provocation tests of the masticatory system in TMD patients with ETTH and CTTH. Materials and Methods: 153 TMD pain subjects and 84 healthy controls were enrolled in the study. Inclusion criterion for the TMD subjects was a RDC/TMD pain diagnosis (group I and III) and ETTH or CTTH diagnosed according to IHS criteria. Assessment variables were mandibular range of motion, masticatory muscle and TMJ tenderness, referred pain, pressure pain threshold (PPT), and provocation tests of the jaw. Results: PPT differed significantly between controls and TMD (ETTH and CTTH) patients at trigeminal and non-trigeminal sites (p < 0.001). Number of painful muscle sites increased significantly with headache frequency (p < 0.001). CTTH patients had significantly more sites with referred pain in the temples than ETTH patients (p < 0.03). Overall, 31% of ETTH and 57% of CTTH patients reported familiar headache in one or more of the provocation tests. Conclusion: TMD patients with ETTH and CTTH had increased sensitivity at trigeminal and non-trigeminal sites and more muscle tenderness compared with controls, which suggests involvement of peripheral and central sensitization. Overall, one-third of ETTH and over half of CTTH patients exhibited a causal temporal relationship with familiar headache in the temples following provocation tests.
