  • Assessing Radiographic Response to 223Ra with an Automated Bone Scan Index in Metastatic Castration-Resistant Prostate Cancer Patients

    • For effective clinical management of patients being treated with 223Ra, there is a need for radiographic response biomarkers to minimize disease progression and to stratify patients for subsequent treatment options. The objective of this study was to evaluate an automated bone scan index (aBSI) as a quantitative assessment of bone scans for radiographic response in patients with metastatic castration-resistant prostate cancer (mCRPC). Methods: In a multicenter retrospective study, bone scans from patients with mCRPC treated with monthly injections of 223Ra were collected from 7 hospitals in Sweden. Patients with available bone scans before treatment with 223Ra and at treatment discontinuation were eligible for the study. The aBSI was generated at baseline and at treatment discontinuation. The Spearman rank correlation was used to correlate aBSI with the baseline covariates: alkaline phosphatase (ALP) and prostate-specific antigen (PSA). The Cox proportional-hazards model and Kaplan-Meier curve were used to evaluate the association of covariates at baseline and their change at treatment discontinuation with overall survival (OS). The concordance index (C-index) was used to evaluate the discriminating strength of covariates in predicting OS. Results: Bone scan images at baseline were available from 156 patients, and 67 patients had both a baseline and a treatment discontinuation bone scan (median, 5 doses; interquartile range, 3-6 doses). Baseline aBSI (median, 4.5; interquartile range, 2.4-6.5) was moderately correlated with ALP (r = 0.60, P < 0.0001) and with PSA (r = 0.38, P = 0.003). Among baseline covariates, aBSI (P = 0.01) and ALP (P = 0.001) were significantly associated with OS, whereas PSA values were not (P = 0.059). After treatment discontinuation, 36% (24/67), 80% (54/67), and 13% (9/67) of patients demonstrated a decline in aBSI, ALP, and PSA, respectively. As a continuous variable, the relative change in aBSI after treatment, compared with baseline, was significantly associated with OS (P < 0.0001), with a C-index of 0.67. Median OS in patients with both aBSI and ALP decline (median, 134 wk) was significantly longer than in patients with ALP decline only (median, 77 wk; P = 0.029). Conclusion: Both aBSI at baseline and its change at treatment discontinuation were significant parameters associated with OS. The study warrants prospective validation of aBSI as a quantitative imaging response biomarker to predict OS in patients with mCRPC treated with 223Ra.

  • Mammalian evolution of human cis-regulatory elements and transcription factor binding sites

    • Understanding the regulatory landscape of the human genome is a long-standing objective of modern biology. Using the reference-free alignment across 241 mammalian genomes produced by the Zoonomia Consortium, we charted evolutionary trajectories for 0.92 million human candidate cis-regulatory elements (cCREs) and 15.6 million human transcription factor binding sites (TFBSs). We identified 439,461 cCREs and 2,024,062 TFBSs under evolutionary constraint. Genes near constrained elements perform fundamental cellular processes, whereas genes near primate-specific elements are involved in environmental interaction, including odor perception and immune response. About 20% of TFBSs are transposable element-derived and exhibit intricate patterns of gains and losses during primate evolution whereas sequence variants associated with complex traits are enriched in constrained TFBSs. Our annotations illuminate the regulatory functions of the human genome.

    • Background:Chronic ruptures of the Achilles tendon may lead to symptomatic weakness, despite rehabilitation. Open reconstruction yields good outcome but has a high complication rate, notably wound problems. Endoscopically assisted free semitendinosus transfer restores ankle and preserves first metatarsophalangeal joint (MTPJ) function.Indications:The main indication for the procedure is symptomatic chronic rupture of the Achilles tendon with a palpable tendon gap.Technique Description:The procedure can be split into 4 stages: graft harvest, calcaneum and tunnel preparation, proximal graft attachment, and finally graft passage and screw insertion.Discussion/Conclusion:Following reconstruction, patients use a cast in full plantar flexion for 2 weeks, then a graduated walker for full weight-bearing.

  • Gender, age at onset, and duration of being ill as predictors for the long-term course and outcome of schizophrenia : an international multicenter study

    • Background The aim of the current study was to explore the effect of gender, age at onset, and duration on the long-term course of schizophrenia. Methods Twenty-nine centers from 25 countries representing all continents participated in the study that included 2358 patients aged 37.21 +/- 11.87 years with a DSM-IV or DSM-5 diagnosis of schizophrenia; the Positive and Negative Syndrome Scale as well as relevant clinicodemographic data were gathered. Analysis of variance and analysis of covariance were used, and the methodology corrected for the presence of potentially confounding effects. Results There was a 3-year later age at onset for females (P < .001) and lower rates of negative symptoms (P < .01) and higher depression/anxiety measures (P < .05) at some stages. The age at onset manifested a distribution with a single peak for both genders with a tendency of patients with younger onset having slower advancement through illness stages (P = .001). No significant effects were found concerning duration of illness. Discussion Our results confirmed a later onset and a possibly more benign course and outcome in females. Age at onset manifested a single peak in both genders, and surprisingly, earlier onset was related to a slower progression of the illness. No effect of duration has been detected. These results are partially in accord with the literature, but they also differ as a consequence of the different starting point of our methodology (a novel staging model), which in our opinion precluded the impact of confounding effects. Future research should focus on the therapeutic policy and implications of these results in more representative samples.

  • Associations between common genetic variants and income provide insights about the socioeconomic health gradient

    • We conducted a genome-wide association study (GWAS) on income among individuals of European descent and leveraged the results to investigate the socio-economic health gradient (N=668,288). We found 162 genomic loci associated with a common genetic factor underlying various income measures, all with small effect sizes. Our GWAS-derived polygenic index captures 1 - 4% of income variance, with only one-fourth attributed to direct genetic effects. A phenome-wide association study using this polygenic index showed reduced risks for a broad spectrum of diseases, including hypertension, obesity, type 2 diabetes, coronary atherosclerosis, depression, asthma, and back pain. The income factor showed a substantial genetic correlation (0.92, s.e. = .006) with educational attainment (EA). Accounting for EA's genetic overlap with income revealed that the remaining genetic signal for higher income related to better mental health but reduced physical health benefits and increased participation in risky behaviours such as drinking and smoking.

    • Alloys of AlCoCrFeMnxNi (x = 0.0, 0.04, 0.08, 0.12 and 0.16) have been synthesized through arc-melting and gas atomisation (x = 0.0 and 0.16) to investigate the effect of Mn additions to AlCoCrFeNi. Here, the structure, magnetic properties and the thermal stability of the alloys is presented. Electron microscopy confirmed the elemental composition and revealed the microstructure to consist of two spinodally decomposed phases. Rietveld analysis of standard powder X-ray diffraction showed the arc-melted samples consisted of two phases, a B2 phase and a bcc phase while the gas atomised powders consisted of a singlephased B2 structure. Magnetic measurements revealed an increase in the saturation magnetisation at room temperature by 68% for AlCoCrFeMnNi compared to AlCoCrFeNi. The thermal stability of the alloys was investigated using magnetometry, differential scanning calorimetry and in-situ X-ray diffraction, which showed that an increase in Mn content adversely effected the thermal stability of the alloy.

    • The proton local coordination environments and vibrational dynamics associated with the two order of magnitude change in proton conductivity in hydrated forms of hexagonal and cubic structured BaTi1-xScxO3Hx (0.16 < x < 0.7) were investigated using optical spectroscopy, neutron spectroscopy, and first-principles calculations. Whereas the cubic structure compositions display a single proton site, we show that protons occupy three distinct sites in compositions exhibiting the hexagonal structure. The principal site is characterized by interoctahedral hydrogen bonds, while two additional low occupancy sites are similar to those in the cubic structure, with classic intraoctahedral geometry. Furthermore, the proton hydrogen bond strength increases with decreasing scandium doping level. We infer from this that the stronger, more energetic hydrogen bonds in the hexagonal structure, resulting from proton sites with lower symmetry (lower multiplicity), are predominantly responsible for the significant reduction in macroscopic conductivity between cubic and hexagonal BaTi1-xScxO3Hx materials, rather than simply the absolute number of protons. Our findings are highly relevant to the field, clarifying the advantages of high-symmetry structures with high-multiplicity proton sites to favorable properties in ceramic proton-conducting oxides.

  • Influence of nano-VC on the structural and magnetic properties of MnAlC-alloy

    • Alloys of Mn55Al45C2 with additions of VC nano-particles have been synthesized and their properties evaluated. The Mn55Al45C2(VC)(x) (x=0.25, 0.5 and 1) alloys have been prepared by induction melting resulting in a high content of the ferromagnetic tau -phase (>94 wt.%). Powder X-ray diffraction indicates that nano-VC can be dissolved in the alloy matrix up to 1 at.%. On the other side, metallography investigations by scanning electron microscopy and scanning transmission electron microscope show inclusions of the nanosized additives in the microstructure. The effect of nano-VC on the grain and twin boundaries has been studied by electron backscattering diffraction. The magnetization has been measured by magnetometry up to 9 T while the domain structure has been studied using both magnetic force microscopy as well as Kerr-microscopy. For nano-VC contents above 0.25 at.%, a clear increase of the coercive force is observed, from 57 to 71 kA/m. The optimum appears to be for 0.5 at.% nano-VC which shows a 25% increase in coercive force without losing any saturation magnetization. This independent increase in coercivity is believed to originate from the nano-VC reducing the overall magnetic domain size. Overall, we observe that addition of nano-VC could be an interesting route to increase the coercive force of MnAl, without sacrificing saturation magnetization.

  • Keck/NIRC2 L'-Band Imaging of Jovian-Mass Accreting Protoplanets around PDS 70

    • We present L'-band imaging of the PDS 70 planetary system with Keck/NIRC2 using the new infrared pyramid wave front sensor. We detected both PDS 70 b and c in our images, as well as the front rim of the circumstellar disk. After subtracting off a model of the disk, we measured the astrometry and photometry of both planets. Placing priors based on the dynamics of the system, we estimated PDS 70 b to have a semimajor axis of au and PDS 70 c to have a semimajor axis of au (95% credible interval). We fit the spectral energy distribution (SED) of both planets. For PDS 70 b, we were able to place better constraints on the red half of its SED than previous studies and inferred the radius of the photosphere to be 2–3 R Jup. The SED of PDS 70 c is less well constrained, with a range of total luminosities spanning an order of magnitude. With our inferred radii and luminosities, we used evolutionary models of accreting protoplanets to derive a mass of PDS 70 b between 2 and 4 M Jup and a mean mass accretion rate between 3 × 10−7 and 8 × 10−7 M Jup/yr. For PDS 70 c, we computed a mass between 1 and 3 M Jup and mean mass accretion rate between 1 × 10−7 and 5 × 10−7 M Jup/yr. The mass accretion rates imply dust accretion timescales short enough to hide strong molecular absorption features in both planets' SEDs.
