    • Gabapentin is used in analgesic treatment of neuropathic pain, and large interindividual variation has been observed in the pharmacokinetics (PK) of the drug. The aim of this study was to develop a population PK model for gabapentin appropriate for monitoring patients with neuropathic pain and for individualizing their dose regimens. Steady-state serum concentrations of gabapentin, distributed over a dosage interval, were obtained from 16 adult patients. Data were analyzed with an iterative 2-stage Bayesian and a nonparametric adaptive grid algorithm (NPAG) (USC*PACK) and with nonlinear mixed effects modeling (NONMEM). Compartmental population models for gabapentin PK were developed in NPAG and NONMEM using creatinine clearance and body weight as covariates. Bioavailability was included in the models as a function of dose by using a hyperbolic function derived from data previously reported in the literature. The mean population parameter estimates from the final NPAG model predicted individual serum concentrations reasonably well. The models developed in NONMEM provided additional information about the relevance of the various possible covariates and also allowed for further evaluation by simulation from the model. The population PK model may be utilized in the MM-USCPACK monitoring software (MM: multiple model dosage design) for predicting and achieving individually optimized steady-state serum concentrations of gabapentin.

  • Adaptive spatio-temporal filtering of disturbed ECGs: a multi-channel approach to heartbeat detection in smart clothing

    • Intermittent disturbances are common in ECG signals recorded with smart clothing: this is mainly because of displacement of the electrodes over the skin. We evaluated a novel adaptive method for spatio-temporal filtering for heartbeat detection in noisy multi-channel ECGs including short signal interruptions in single channels. Using multi-channel database recordings (12-channel ECGs from 10 healthy subjects), the results showed that multi-channel spatio-temporal filtering outperformed regular independent component analysis. We also recorded seven channels of ECG using a T-shirt with textile electrodes. Ten healthy subjects performed different sequences during a 10-min recording: resting, standing, flexing breast muscles, walking and pushups. Using adaptive multi-channel filtering, the sensitivity and precision was above 97% in nine subjects. Adaptive multi-channel spatio-temporal filtering can be used to detect heartbeats in ECGs with high noise levels. One application is heartbeat detection in noisy ECG recordings obtained by integrated textile electrodes in smart clothing.

    • Mining operations at Maarsaetter in 1877-81 resulted in increased metal loading to a small lake, notably as sulphidic tailings. The event is taken as an opportunity to study the present environmental impact of a historical single major metal release. Lake water and four sediment cores were sampled and analysed for principal and trace elements in solid and aqueous phases as well as general hydrochemical conditions. Chronologies were determined from super(206)Pb/ super(207)Pb ratios and historical records.Ordinary sedimentation after the event has lead to that the tailings are found as a distinct layer at a depth of 18-22 cm in the sediment. The layer is characterized by elevated metal concentrations in the solid and pore water phases, respectively, circum neutral pH and sulphate concentrations below detection. Geochemical modelling indicated a preference for carbonate equilibrium in the waste while sulphides prevailed above it. It is concluded that the growth of the ordinary sediment on top of the waste has lead to a physicochemical barrier that seals of the waste from the overlying sediment. Chemical or physical rupture of the barrier will release the metals to downstream regions.According to the chronologies at least three sources have contributed to the present elevated levels of metals, in additions to the release of tailings. Copper from a historical blast furnace prior to the event at Maarsaetter, transport from mineralized parts of the watershed and release of contaminated water from present mining operations maintain elevated levels of notably zinc, silver, cadmium and lead. At present less than 10% of the lead content at the sediment/water interface comes from atmospheric deposition. Increased levels of antimony and thallium were not observed prior to ca 1950.

    • We present new results for the inner valence levels of clusters of the three inert gases Ar, Kr and Xe based on photoelectron spectroscopy studies. The inner valence levels are compared to the localised core levels and to the delocalised outer valence levels. This comparison shows a gradual change from a relatively localised behaviour for Ar inner valence 3s, over the intermediate case of Kr inner valence 4s, to a more delocalised behaviour for Xe inner valence 5s. This change correlates well with the ratio between the orbital sizes and the interatomic distances. The Kr4s intermediate case is found to exhibit characteristics of both localised and delocalised behaviour.

    • Dynamic contrast-enhanced magnetic resonance imaging (MRI) is a promising tool in the evaluation of tumor physiology. From rapidly acquired images and a model for contrast agent pharmacokinetics, physiological parameters are derived. One pharmacokinetic model, the tissue homogeneity model, enables estimation of both blood flow and vessel permeability together with parameters that describe blood volume and extracellular extravascular volume fraction. However, studies have shown that parameter estimation with this model is unstable. Therefore, several initial guesses are needed for accurate estimates, which makes the estimation slow. In this study a new estimation algorithm for the tissue homogeneity model, based on Fourier domain calculations, was derived and implemented as a Matlab program. The algorithm was tested with Monte-Carlo simulations and the results were compared to an existing method that uses the adiabatic approximation. The algorithm was also tested on data from a metastasis in the brain. The comparison showed that the new algorithm gave more accurate results on the 2.5th and 97.5th percentile levels, for instance the error in blood volume was reduced by 21%. In addition, the time needed for the computations was reduced with a factor 25. It was concluded that the new algorithm can be used to speed up parameter estimation while accuracy can be gained at the same time.
