  • Increased water transport in PDMS silicone films by addition of excipients

    • The development of new adhesive wound care products intended for an application over a prolonged time requires good water transporting properties of the adhesive for the maintenance of a suitable environment around the wound. The ability of polydimethylsiloxane (PDMS)-based silicone films to transport water has led to its use in skin pressure-sensitive adhesives and it would be advantageous to find ways for controlling or increasing water transport across PDMS films in order to be able to develop improved skin adhesives. In this study we present a way to increase water transport in such films by the addition of hydrophilic excipients. Three hydrophilic additives, highly water-soluble sucrose and the two superabsorbent polymers (SAP) Carbopol® and Pemulen™, were investigated. The effect of the excipients was characterized by water transport studies, swelling tests, scanning electron microscopy imaging and confocal microscopy. The cross-linked polymers, primarily Pemulen™, were efficient water transport enhancers, whereas sucrose did not show any effect. The effect of the additives seemed to correlate with their water binding capacity. For SAPs the formation of a percolating structure by swollen polymer was also suggested, which enhances water penetration by the higher volume fraction of areas with a higher diffusion constant (swollen SAP), leading to a faster transport through the entire film. © 2011 Acta Materialia Inc.     • Carboxymethyl‐hexanoyl chitosan (CHC) has the ability to self‐assemble into nanocapsules in anaqueous solution and it has recently shown potentialin numerous biomedical applications. Here weinvestigate the protein loading efficiency and release,as well as the structural properties of CHC proteinnanocarriers. Bovine serum albumin (BSA) or itschromophore labelled version, fluorescein‐BSA, wasused as a model protein and the loading wasperformed with a simple mixing of pre‐formednanocapsules and protein. Dynamic light scatteringand zeta potential analysis revealed that proteinloaded nanocarriers with high positive zeta potentialwere formed. The protein loaded nanocarriersdisplayed a loading efficiency of 75% and a very slowprotein release. In summary, our results highlight thepotential of CHC as a protein nanocarrier, but alsoindicate that protein‐CHC interactions need to beconsidered in protein containing CHC formulationswhere protein release is not the main function.

    • The technique in Fontan surgery has developed from the lateral tunnel (LT) toward the extracardiac conduit (EC) used to reduce long-term complications such as atrial arrhythmia and sinus node dysfunction. Heart rate variability (HRV) examines cardiac nervous activity controlling the sinus node. This study aimed to investigate HRV in a cohort of children with univentricular hearts, focusing on the relation between HRV and surgical procedure. For 112 children with Fontan circulation, HRV was analyzed using power spectral analysis. Spectral power was determined in three regions: very-low-frequency (VLF), low-frequency (LF), and high-frequency (HF) regions. Patients were compared with 66 healthy controls subject. Patients with LT were compared with patients who had EC. The children with Fontan circulation showed a significantly reduced HRV including total power (P < 0.0001), VLF (P < 0.0001), LF (P < 0.0001), and HF (P = 0.001) compared with the control subjects. The LT and EC patients did not differ significantly. Reduced HRV was found in both the LT and EC patients. In terms of HRV reduction, EC was not superior to LT.

  • Fracture strength of low translucent and high translucent monolithic zirconia crowns with different thicknesses

    • Objective. The aim of the study was to examine the fracture strength of fully anatomical monolithic high translucent Y-TZP, yttria-stabilized tetragonal zirconia polycrystal, crowns and to compare them with monolithic low translucent Y-TZP crowns in different thicknesses. Materials and methods. 80 standardized crowns were made out of a master model resembling a first mandibular molar made in composite material; 40 crowns made of Lava Zirconia by 3M ESPE, a monolithic low translucent Y-TZP material, and 40 crowns made of Lava Plus by 3M ESPE, a monolithic high translucent Y-TZP material. In each group ten crowns of the thicknesses 1.0 mm, 0.7 mm, 0.5 mm and 0.3 mm were made. All crowns underwent thermocycling and preload to simulate aging and normal wear. Finally the specimens were placed in a testing jig and underwent load to fracture. Results. No significant difference was found when comparing the two materials. When comparing the materials in each thickness a significant difference was found in the 0.5 mm group where the high translucent YTZ-P had a higher mean fracture strength value. A significant difference was seen between the different thicknesses within both groups. The strength increased with added thicknesses. Conclusions. The low translucent and high translucent monolithic YTZ-P perform equally well. The use of high translucent monolithic Y-TZP could result in more aesthetic results and the minimal invasive preparation could lead to a reduction in biological complications. This could be beneficial for the patient since the results suggest that even a thin restoration sustains reasonable load in an in vitro environment. Clinical studies are needed to confirm this suggestion.

  • Hypoxic regulation of secreted proteoglycans in macrophages.

    • Macrophages are prominent in hypoxic areas of atherosclerotic lesions, and their secreted proteoglycans (PG), such as versican, can modulate the retention of lipoproteins and the activity of enzymes, cytokines, and growth factors involved in atherogenesis. In this study, we report the effects of hypoxia on PG secreted by human monocyte-derived macrophages (HMDM) and the potential regulation by the transcription factor hypoxia-inducible factor (HIF-1alpha and HIF-2alpha). We found that versican co-localized with HIF-1alpha in macrophage-rich areas in human advanced atherosclerotic lesions. Versican and perlecan mRNA expression increased after exposure to 0.5% O(2) (hypoxia) compared with 21% O(2) (control cells). Using precursors to GAG biosynthesis combined with immunoabsorption with a versican antibody an increased versican synthesis was detected at hypoxia. Furthermore, siRNA knockdown of HIF-1alpha and HIF-2alpha in THP-1 cells showed that the hypoxic induction of versican and perlecan mRNA expression involved HIF signaling. Versican expression was co-regulated by HIF-1alpha and HIF-2alpha but expression of perlecan was influenced only by HIF-1alpha and not by HIF-2alpha knockdown. The results show that oxygen concentration is an important modulator of PG expression in macrophages. This may be a novel component of the complex role of macrophages in atherosclerosis.

  • Rethinking User Involvement in the Front End of Healthcare Innovation

    • The purpose of this paper is to share insights from an empirical study within the Swedish healthcare system, based on a case study of the largest business unit of a Swedish medical technology company and its key customer segment. The study involving a Swedish medical technology company and three Swedish hospitals indicates need to move away from an archetypical, largely homogeneous view of the user, towards a more flexible, heterogeneous user perspective. Following from that insight the paper outlines a preliminary model aimed at improving the conditions of the firm for user involvement in the front end of healthcare innovation. In the proposed model, different users are brought to the foreground in different parts of the front-end innovation process. The research performed aimed at putting the business unit in a more beneficial position to involve users in the front end of innovation by increasing their understanding of a) which user types they actually have access to, b) which roles that different user types may take in the various stages of the innovation front-end.
