- Background Registries have shown that quality of care for acute coronary syndromes (ACS) often falls below the standards recommended in professional guidelines. Quality improvement (QI) is a strategy to improve standards of clinical care for patients, but the efficacy of QI for ACS has not been tested in randomized trials. Methods We undertook a prospective, cluster-randomized, multicenter, multinational study to evaluate the efficacy of a QI program for ACS. Participating centers collected data on consecutive admissions for non-ST-elevation ACS for 4 months before the QI intervention and 3 months after. Thirty-eight hospitals in France, Italy, Poland, Spain, and the United Kingdom were randomized to receive the QI program or not, 19 in each group. We measured 8 in-hospital quality indicators (risk stratification, coronary angiography, anticoagulation, beta-blockers, statins, angiotensin-converting enzyme inhibitors, and clopidogrel loading and maintenance) before and after the intervention and compared composite changes between the QI and non-QI groups. Results A total of 2604 patients were enrolled. The absolute overall change in use of quality indicators in the QI group was 8.5% compared with 0.8% in the non-QI group (odds ratio for achieving a quality indicator in QI versus non-QI 1.66, 95% CI 1.43-1.94; P < .001). The main changes were observed in the use of risk stratification and clopidogrel loading dose. Conclusions The QI strategy resulted in a significant improvement in the quality indicators measured. This type of QI intervention can lead to useful changes in health care practice for ACS in a wide range of settings.

- Background: Acute coronary syndromes, including myocardial infarction and unstable angina, are important causes of premature mortality, morbidity and hospital admissions. Acute coronary syndromes consume large amounts of health care resources, and have a major negative economic and social impact through days lost at work, support for disability, and coping with the psychological consequences of illness. Several registries have shown that evidence based treatments are under-utilised in this patient population, particularly in high-risk patients. There is evidence that systematic educational programmes can lead to improvement in the management of these patients. Since application of the results of important clinical trials and expert clinical guidelines into clinical practice leads to improved patient care and outcomes, we propose to test a quality improvement programme in a general group of hospitals in Europe. Methods/Design: This will be a multi-centre cluster-randomised study in 5 European countries: France, Spain, Poland, Italy and the UK. Thirty eight hospitals will be randomised to receive a quality improvement programme or no quality improvement programme. Centres will enter data for all eligible non-ST segment elevation acute coronary syndrome patients admitted to their hospital for a period of approximately 10 months onto the study database and the sample size is estimated at 2,000-4,000 patients. The primary outcome is a composite of eight measures to assess aggregate potential for improvement in the management and treatment of this patient population (risk stratification, early coronary angiography, anticoagulation, beta-blockers, statins, ACE-inhibitors, clopidogrel as a loading dose and at discharge). After the quality improvement programme, each of the eight measures will be compared between the two groups, correcting for cluster effect. Discussion: If we can demonstrate important improvements in the quality of patient care as a result of a quality improvement programme, this could lead to a greater acceptance that such programmes should be incorporated into routine health training for health professionals and hospital managers.

- This paper, drawing on an empirical study of academic researchers pursuing collective research projects in engineering-, natural- and medical sciences, address the dynamics of accommodation and resistance of project management technology such as tools, techniques and taxonomy. We ground our analysis of accommodation and resistance on three interconnected empirical questions. First, we want to question the means to manage, critically dissecting what skills and methods are in use for running complex research projects within university settings. Secondly, we want to question the authority or legitimacy to manage in the sense if, how and in what way project managers/coordinators have or may appropriate necessary legitimacy for applying such obtrusive management techniques such as project management prescribes. Third, we want to question the identity work (of the self and of the collective) of how potentially to mitigate ideas of freedom and independence with efficient coordination through project management.

- In Sweden, three of the four largest institutional research funders have the explicit goal to not only support research but also to stimulate economic growth. How this should be done in practice, however, is more uncertain. Often used theories, e.g. triple helix, national innovation systems and clusters, are all based on the notion that different skills and organizations should be combined in dynamic partnerships in order to shorten the time from research to application. In this dissertation, I present how an R&D program with the requirement to both produce valuable research results and to stimulate innovation was put into practice. The program is explored from multiple levels of analysis. Based on the case, it is showed how innovation policy acts as an organizing force on its environment and that it affects the organization of R&D activities. This dissertation identifies problems that arise and investigates how the process can be improved in order to meet the objectives of stimulating innovation and growth. By applying the ideas of Serres and Luhmann, using the term innovation in these instances can be regarded as a form of parasite, which affects the system as such. I argue that this can be a fruitful way of conceptualizing how we can strive to manage closed systems that are otherwise challenging to govern. We can develop the debate on how to stimulate innovation by shifting the perspective from a sequential top-down process in which policy is being implemented to have a planned effect (and where any deviation constitutes an unsuccessful implementation), to discuss how we can disrupt a system in a particular direction. Based on the empirical data, I explore and analyze a number of identified events that will be rewarding for professionals working with similar efforts or are planning further research in this area. Among other things, it is demonstrated how a process perspective can be a rewarding way to study not only structural aspects but also seize the dynamics of the system. I demonstrate that this perspective better allows us to understand which aspects are important for stimulating innovation. The dissertation further discusses the concept of dual technologies, intellectual property, applied research and the conflict between different levels of innovation.

What may also be seen is that practice-based studies are not yet a coherent area. However, it is more important that practice-based studies allow researchers to understand the organization less as an entity and more as a socially-accomplished task.Research limitations/implications – Several implications for research are offered, including the need for studies that emphasize the small details of organizing, and that practice-based studies are not restricted to a certain methodology but depend on what a particular paper tries to accomplish.Practical implications – With an ever-growing stream of research focusing on projects the guest editors argue that it is about time to look into the details of organizing. This could be accomplished through a number of ways but in this special issue it is proposed that approaching traditional areas with a conscious naivety when asking the questions may do it. For the practitioner, the special issue offers important insights into how things are done in practice, which may be used as a mirror or reflection upon their own practice.Originality/value – This editorial and special issue offer insights for any academic interested in understanding projects differently.
