The diagnostic value of a change in bowel habit for colorectal cancer within different age groups

- Background Change in bowel habit as a sole alarm symptom for colorectal cancer is disputed. Objective We investigated the diagnostic value of change in bowel habit for colorectal cancer, particularly as a single symptom and within different age groups. Methods This retrospective cohort study examined colorectal cancer fast track referrals and outcomes across four Swedish hospitals (April 2016-May 2017). Entry criteria constituted one or more of three alarm features: anaemia, visible rectal bleeding, or change in bowel habit for more than 4 weeks in patients over 40 years of age. Patients were grouped as having only change in bowel habit, change in bowel habit plus anaemia/bleeding or anaemia/bleeding only. Results Of 628 patients, 22% were diagnosed with colorectal cancer. There were no cases of colorectal cancer in the only change in bowel habit group under 55 years, while this was 6% for 55-64 years, 8% for 65-74 years and 14% for 75 years and older. Among subjects under 55 years, 2% with anaemia/bleeding had colorectal cancer, this increased to 34% for 55 years and older (P < 0.0001). Change in bowel habit plus anaemia/bleeding gave a colorectal cancer prevalence of 16% in under 55 years and increased to 30% for 55 years and older (P = 0.07). Conclusion Change in bowel habit as the only alarm feature has a low diagnostic yield for colorectal cancer in patients under 55 years.

Challenges, tasks, and opportunities in modeling agent-based complex systems

- Humanity is facing many grand challenges at unprecedented rates, nearly everywhere, and at all levels. Yet virtually all these challenges can be traced back to the decision and behavior of autonomous agents that constitute the complex systems under such challenges. Agent-based modeling has been developed and employed to address such challenges for a few decades with great achievements and caveats. This article reviews the advances of ABM in social, ecological, and socio-ecological systems, compare ABM with other traditional, equation-based models, provide guidelines for ABM novice, modelers, and reviewers, and point out the challenges and impending tasks that need to be addressed for the ABM community. We further point out great opportunities arising from new forms of data, data science and artificial intelligence, showing that agent behavioral rules can be derived through data mining and machine learning. Towards the end, we call for a new science of Agent-based Complex Systems (ACS) that can pave an effective way to tackle the grand challenges.

- Agent-based modeling (ABM) has been widely used in numerous disciplines and practice domains, subject to many eulogies and criticisms. This article presents key advances and challenges in agent-based modeling over the last two decades and shows that understanding agents' behaviors is a major priority for various research fields. We demonstrate that artificial intelligence and data science will likely generate revolutionary impacts for science and technology towards understanding agent decisions and behaviors in complex systems. We propose an innovative approach that leverages reinforcement learning and convolutional neural networks to equip agents with the intelligence of self-learning their behavior rules directly from data. We call for further developments of ABM, especially modeling agent behaviors, in the light of data science and artificial intelligence.

- Purpose: Cervical cancer prevention for older women can be challenging since there are no specific guidelines for this group. This study aimed to determine the incidence of oncogenic HPV and HPV-related dysplasia in elderly women 5 years after being HPV negative.Methods: Invited women participated five years earlier in a study where self-sampling for HPV testing was applied, at this time, they were all HPV negative. The women were now, five years later invited to perform self-sampling for HPV testing. Women with a positive result performed a repeat HPV test. Those with a positive repeat HPV test were examined by colposcopy, biopsy and cytology.Results: Of the 804 invited women, 634 (76.9%) agreed to participate in the study and a self-sampling kit was sent to them. Of these, 99.6% (632/634) sent a sample to the HPV laboratory. The participation rate in each age group was 93.3% at age 65, 74.0% at age 70, 80.7% at age 75 and 64.6% at age 80. Overall 18 women (2.8%, 95% CI 3.2 to 6.0) were HPV positive in the first test and 8 (1.3%, 95% CI 0.6 to 2.6) in the second test. Sampling for the second test was done on average 5.4 months after the first test. Fifty per cent (4/8) of the women with a positive repeat test had dysplasia in histology.Conclusions: The incidence of HPV in previously HPV-negative elderly women was low. Among women who were HPV positive in a repeat test, there was a high prevalence of low grade dysplasia.

- Aquaglyceroporin 7 (AQP7) facilitates glycerol flux across the plasma membrane with a critical physiological role linked to metabolism, obesity, and associated diseases. Here, we present the single-particle cryo-EM structure of AQP7 determined at 2.55 Å resolution adopting two adhering tetramers, stabilized by extracellularly exposed loops, in a configuration like that of the well-characterized interaction of AQP0 tetramers. The central pore, in-between the four monomers, displays well-defined densities restricted by two leucine filters. Gas chromatography mass spectrometry (GC/MS) results show that the AQP7 sample contains glycerol 3-phosphate (Gro3P), which is compatible with the identified features in the central pore. AQP7 is shown to be highly expressed in human pancreatic α- and β- cells suggesting that the identified AQP7 octamer assembly, in addition to its function as glycerol channel, may serve as junction proteins within the endocrine pancreas.
