  • Does income matter for troublesome neck pain? : A population-based study on risk and prognosis

    • Background Studies have shown associations between higher income and better health, but income has not been studied in relation to neck pain. The aims of this cohort study were to assess the sex-specific role of disposable income for onset and prognosis of neck pain in the general population and if economic stress influences such potential associations. Methods Two subcohorts were identified in the Stockholm Public Health Cohort with data from 2002. Cohort I (risk cohort) included persons without neck pain (n = 8348). Cohort II (prognostic cohort) included persons with occasional neck pain during the previous 6 months (n =0 523). Both cohorts were assessed for long duration troublesome neck pain (LDNP) in 2007. Individual income was defined as aggregated annual family income in 2002 with each family member assigned a weighted consumption share, based on salary, pensions and social benefits. LDNP in 2007 was defined as having had troublesome neck pain lasting for three or more consecutive months the previous 5 years. Association between income and LDNP, considering potential confounding, was investigated by multivariable logistic regression. Economic stress was tested as effect modifier between income and LDNP. Results In both cohorts, associations were found between lower income and a higher risk for LDNP. The results were similar between the sexes. Economic stress modified the associations in both cohorts. Conclusions Low income may be a risk as well as prognostic factor for developing LDNP. Furthermore, the results indicate that economic stress may be an underlying factor to consider when studying associations between income and neck pain.

    • Today, Social emotional learning (SEL) is common in pre-school and school both in Europe and North America. An overall issue in this study concerns the how borders between private and public are constructed and negotiated in schools and pre-schools where socio-emotional programs are practiced. A specific aspect, dealt with in this paper, concerns the processes of internalization, among students and children, of the normative set of values proclaimed by these programs. In this first preliminary analysis we operate within a foucouldian perspective. With this as a point of departure, abilities such as self-control, regulation of emotions and social behavior, and a discursive and social competence for presenting the self are constructed through the production of docile, obedient bodies.

    • Social emotional learning (SEL) is common in preschools and schools both in Europe and North America today. Programmes for socio-emotional training and the rise of what is labelled therapeutic education have dramatically increased during the first decade of the millennium. In this article, a manual-based programme used for SEL in a Swedish school context is analysed from perspectives rooted in childhood sociology and post-structural studies. The aim of this study is to analyse the discursive constructions of a context of risk and the instilment of specific corporeal regimes. The main issue concerns the meaning and use of the body in the discursive construction of the social competent child within this context of risk. The analysis shows that the socially competent child is shaped and cultivated through self-regulating techniques aiming at creating a docile body, a body that will be a good citizen, a pliant member of the social order. Social emotional learning (SEL) is common in preschools and schools both in Europe and North America today. Programmes for socio-emotional learning and what is labelled therapeutic education (Ecclestone & Hayes, 2007; Furedi, 2009) have dramatically increased during the first decade of the millennium. In Sweden, schools often justify their use of these programmes as a way of organizing their value-based education (von Br mssen, 2011), that is, as a way of realizing the democratic ambitions expressed in educational policy documents, such as the national curriculum.

  • Rapport från projektet Socioemotionella program i förskola och skola : förskolebarns, elevers och lärares erfarenheter

    • Projektet syftar till att belysa olika aktörsperspektiv på implementering av och arbetet med socioemotionella program i den kommunala grundskolan och förskolan. Studien består av fem delstudier av fem kommunala verksamheter där arbete med socioemotionella program har ägt rum. Studien har en etnografisk design. Resultatet visar att implementeringsprocessen har sett olika ut i de studerade verksamheterna gällande initiativ till arbetet (uppifrån och ned, nedifrån och upp eller både och). Gemensamt i samtliga verksamheter var dock att det manualbaserade arbetet infördes utan granskning eller diskussion, vare sig på central kommunal nivå eller ute i verksamheterna. Vidare visar resultatet att många lärare till en början välkomnade manualbaserade program för att få tillgång till verktyg för systematiskt arbete med socioemotionella frågor, men att de snart fann svårigheter att omsätta övningarna på ett meningsfullt och etiskt försvarbart sätt för barnen och eleverna i verksamheten. Lärare uttrycker också osäkerhet gällande vilken skillnad de anser att programmen gjort för barnen/eleverna. Många barn och elever var kritiska mot programmen; barnen i förskolan hade svårt att sätta ord på vad de lärde sig av programmen och eleverna på högstadiet var öppet kritiskta till programarbetet. Eleverna i år tre och fem uttryckte däremot i mer positiva ordalag om det socioemotionella arbetet de tagit del av; deras svar ligger ofta i linje med de motiveringar för programmet som finns i programmen själva och bland lärare. Dessa elever uppvisar därmed ha tillägnat sig en diskursiv kompetens i sitt tal om programarbetet. I rapporten diskuteras också vilka implikationer programarbetet medfört dels för lärarprofessionaliteten, dels för realiserandet av skolans normativa uppdrag.

  • Annexin A1 expression in blood mononuclear cells : a potential marker of glucocorticoid activity in patients with coronary artery disease

    • An imbalance between pro- and anti-inflammatory actions is believed to drive progression of atherosclerosis. Annexin A1 (AnxA1) is a key player in resolution of inflammation and a mediator of anti-inflammatory effects of glucocorticoids. Here, we investigated whether expression of AnxA1 in peripheral blood mononuclear cells (PBMCs) was altered in patients with coronary artery disease (CAD) and also related findings to glucocorticoid sensitivity ex vivo.We included 57 patients 6-12 months after acute coronary syndrome (ACS), 10 patients with ACS, and healthy controls. AnxA1 mRNA was measured in PBMCs and AnxA1 protein was assessed in monocytes and lymphocyte subsets by flow cytometry. In post-ACS patients and controls, glucocorticoid sensitivity was determined by measuring inhibitory effects of dexamethasone on LPS46 induced cytokine secretion.AnxA1 mRNA levels in PBMCs were higher in patients compared with controls, although most pronounced in ACS patients. AnxA1 protein was most abundant in the monocyte fraction. ACS patients exhibited the highest levels of cell surface-associated AnxA1 protein while levels in post-ACS patients and controls were similar. Ex vivo assays showed that PBMCs from post-ACS patients were more prone to release IL-6. Glucocorticoid sensitivity correlated with cell surface-associated AnxA1 protein in peripheral monocytes. Dexamethasone also induced upregulation of AnxA1 mRNA.AnxA1 expression in PBMCs is closely associated with glucocorticoid actions and cell surface associated AnxA1 appears to be a marker of glucocorticoid sensitivity. Although still speculative, a “normal” expression of cell surface-associated AnxA1 in post-ACS patients may suggest that glucocorticoid actions in vivo are insufficient to provide adequate anti-inflammatory effects in these patients.

  • Higher expression of annexin A1 in 1 CD56+ than in CD56-T cells : Potential implications for coronary artery disease

    • Background: Increased proportions of circulating proinflammatory CD56+ T cells have been reported in patients with coronary artery disease (CAD). Yet, little is known about regulation of these cells. In the present study, we investigated the expression and potential role of the glucocorticoid-mediated protein annexin A1 (AnxA1) in CD56+ and CD56-T cell subsets, with focus on CAD.Methods and Results: We included totally 52 healthy individuals, 28 patients with acute coronary syndrome (ACS) and 57 patients with a history of ACS. AnxA1 mRNA expression was assessed in peripheral blood mononuclear cells. AnxA1 protein expression (total and cell surface-associated) was measured by whole blood flow cytometry in circulating CD56+ and CD56- T cell subsets. Furthermore, inhibitory effects of dexamethasone and/or recombinant AnxA1 on cytokine secretion by CD56+ and CD56- T cells were explored in vitro. We found that CD56+ T cells (the majority CD8+), expressed higher levels of AnxA1 mRNA and protein than did CD56- T cells. When comparing CAD patients with healthy controls, significantly higher levels of cell surface-associated AnxA1 expression were seen in patients, most pronounced in ACS patients. In vitro, dexamethasone reduced cytokine secretion by CD56+ T cells, whereas AnxA1 alone had no effect, and AnxA1 combined with dexamethasone abolished the dexamethasone-induced suppressive effects.Conclusion: AnxA1 was expressed more abundantly in proinflammatory CD56+ T cells. Patients with ACS exhibited increased levels of cell surface-associated AnxA1, thus indicating increased activation of the AnxA1 pathway. Our data further suggested that AnxA1 might counteract glucocorticoid mediated anti-inflammatory effects in T cells.

  • Quantitative Magnetic Resonance in Diffuse Neurological and Liver Disease

    • Introduction: Magnetic resonance (MR) imaging is one of the most important diagnostic tools in modern medicine. Compared to other imaging modalities, it provides superior soft tissue contrast of all parts of the body and it is considered to be safe for patients. Today almost all MR is performed in a nonquantitative manner, only comparing neighboring tissue in the search for pathology. It is possible to quantify MR-signals and relate them to their physical entities, but time consuming and complicated calibration procedures have prevented this being used in a practical manner for clinical routines. The aim of this work is to develop and improve quantification methods in MRspectroscopy (MRS) and MR-imaging (MRI). The techniques are intended to be applied to diffuse diseases, where conventional imaging methods are unable to perform accurate staging or to reveal metabolic changes associated with disease development.Methods: Proton (1H) MRS was used to characterize the white matter in the brain of multiple sclerosis (MS) patients. Phosphorus (31P) MRS was used to evaluate the energy metabolism in patients with diffuse liver disease. A new quantitative MRI (qMRI) method was invented for accurate, rapid and simultaneous quantification of B1, T1, T2, and proton density. A method for automatic assessment of visceral adipose tissue volume based on an in- and out-ofphase imaging protocol was developed. Finally, a method for quantification of the hepatobiliary uptake of liver specific T1 enhancing contrast agents was demonstrated on healthy subjects.Results: The 1H MRS investigations of white matter in MS-patients revealed a significant correlation between tissue concentrations of Glutamate and Creatine on the one hand and the disease progression rate on the other, as measured using the MSSS. High accuracy, both in vitro and in vivo, of the measured MR-parameters from the qMRI method was observed. 31P MRS showed lower concentrations of phosphodiesters, and a higher metabolic charge in patients with cirrhosis, compared to patients with mild fibrosis and to controls. The adipose tissue quantification method agreed with estimates obtained using manual segmentation, and enabled measurements which were insensitive to partial volume effects. The hepatobiliary uptake of Gd-EOB-DTPA and Gd-BOPTA was significantly correlated in healthy subjects.Conclusion: In this work, new methods for accurate quantification of MR parameters in diffuse diseases in the liver and the brain were demonstrated. Several applications were shown where quantitative MR improves the interpretation of observed signal changes in MRI and MRS in relation to underlying differences in physiology and pathophysiology.

  • Imaging astrocytosis with PET in Creutzfeldt-Jakob disease : case report with histopathological findings

    • In a previous study, patients with suspect Creutzfeldt-Jakob's disease (CJD) have been examined with Positron Emission Tomography (PET) combining N-[11C-methyl]-L-deuterodeprenyl (DED) and [(18)F] 2- fluorodeoxyglucose (FDG) in an attempt to detect astrocytosis and neuronal dysfunction, two of the hallmarks in CJD. Increased DED uptake with pronounced hypometabolism matching the areas with high DED retention was found in the fronto-parieto-occipital areas and cerebellum of patients with confirmed CJD. However, the temporal lobes did not present such a pattern. In 6 of the 15 examined patients the autopsy was performed, but a strict comparison between the PET results and the histopathology could not be done. Recently, one patient with suspect CJD was examined with PET using DED and FDG. The results of the examinations in this patient showed a pattern similar to that found in the brain of the CJD patients from the first study. The patient died shortly after the examination and an autopsy could be performed. The autopsy showed neuronal death, astrocytosis and spongiform changes in the brain. The diagnosis of definite sporadic CJD was established by the Western blot analysis, confirming the presence of the prion resistant protein (PrPres). The PET data demonstrated high DED uptake and extreme low glucose uptake in the left brain hemisphere whereas the right side was less affected. The autopsy was performed allowing the comparison between high DED uptake and the histopathological findings of reactive astrocytosis revealed by immunostaining with antibodies against glial fibrillary acid protein (GFAP). The results confirmed the presence of a pattern with high ratio DED/FDG, similar to that found in the previous study and revealing for the first time, a good correlation between high DED uptake and high density of reactive astrocytes as demonstrated by immunostaining.
