  • Allele-specific regulation of MTTP expression influences the risk of ischemic heart disease.

    • Promoter polymorphisms in microsomal triglyceride transfer protein (MTTP) have been associated with decreased plasma lipids but an increased risk for ischemic heart disease (IHD), indicating that MTTP influences the susceptibility for IHD independent of plasma lipids. The objective of this study was to characterize the functional promoter polymorphism in MTTP predisposing to IHD and its underlying mechanism. Use of pyrosequencing technology revealed that presence of the minor alleles of the promoter polymorphisms -493G>T and -164T>C result in lower transcription of MTTP in vivo in the heart, liver, and macrophages. In vitro experiments indicated that the minor -164C allele mediates the lower gene expression and that C/EBP binds to the polymorphic region in an allele-specific manner. Furthermore, homozygous carriers of the -164C were found to have increased risk for IHD as shown in a case-control study including a total of 544 IHD patients and 544 healthy control subjects. We concluded that carriers of the minor -164C allele have lower expression of MTTP in the heart, mediated at least partly by the transcription factor CCAAT/enhancer binding protein, and that reduced concentration of MTTP in the myocardium may contribute to IHD upon ischemic damage.

  • Open Source Software for Model Driven Development : A Case Study

    • Model Driven Development (MDD) is widely used in the embedded systems domain, and many proprietary and Open Source tools exist that support MDD. The potential for sustainability of such tools needs to assessed prior to any organisational adoption. In this paper we report from a case study conducted in a consultancy company context aiming to investigate Open Source tools for MDD. For the company it was interesting to explore the two Open Source modelling tools Topcased and Papyrus for potential adoption. The focus for our case study is on assessing the health of the ecosystems for the two investigated Open Source projects by means of quantitative analysis of publically available data sources about Open Source projects. The health of ecosystems is an important prerequisite for a long term sustainable OSS (Open Source Software) tool-chain in the MDD area, which can aid strategic decision making for potential adoption within a company context. We have established details on the extent to which developers and users are active in two specific OSS ecosystems, and identified organisational influence for both ecosystems. We find that the investigated tools are promising regarding the health of their ecosystems, and a natural next step for the company would be to proceed with a pilot study in order to analyse the effectiveness of the investigated tools in company contexts.

  • Warfarin dose prediction in children using pharmacometric bridging : comparison with published pharmacogenetic dosing algorithms

    • PurposeNumerous studies have investigated causes of warfarin dose variability in adults whereas studies in children are limited both in numbers and size. Mechanism-based population modelling provides an opportunity to condense and propagate prior knowledge from one population to another. The main objectives with this study were to evaluate the predictive performance of a theoretically bridged adult warfarin model in children, and to compare accuracy in dose prediction relative to published warfarin algorithms for children.MethodAn adult population PK/PD-model for warfarin, with CYP2C9 and VKORC1 genotype, age and target INR as dose predictors, was bridged to children using allometric scaling methods. Its predictive properties were evaluated in an external dataset of children 0-18 years old, including comparison of dose prediction accuracy with three pharmacogenetics-based algorithms for children.ResultsOverall, the bridged model predicted INR response well in 64 warfarin treated Swedish children (median age 4.3 years), but with a tendency to over predict INR in children ≤ 2 years old. The bridged model predicted 20 of 49 children (41%) within ± 20% of actual maintenance dose (median age 7.2 years). In comparison the published dosing algorithms predicted 33-41% of the children within ± 20% of actual dose. Dose optimization with the bridged model based on up to three individual INR observations increased the proportion within ± 20% of actual dose to 70%.ConclusionA mechanism-based population model developed on adult data provides a promising first step towards more individualized warfarin therapy in children.

    • BACKGROUND/OBJECTIVES:Ductal adenocarcinoma in the head of the pancreas (PDAC) is usually unresectable at the time of diagnosis due to the involvement of the peripancreatic vessels. Various preoperative classification algorithms have been developed to describe the relationship of the tumor to these vessels, but most of them lack a surgically based approach. We present a CT-based classification algorithm for PDAC based on surgical resectability principles with a focus on interobserver variability.METHODS:Thirty patients with PDAC undergoing pancreaticoduodenectomy were examined by using a standard CT protocol. Nine radiologists, representing three different levels of expertise, evaluated the CT examinations and the tumors were classified into four categories (A-D) according to the proposed system. For the interobserver agreement, the Intraclass Correlation Coefficient (ICC) was estimated.RESULTS:The overall ICC was 0.94 and the ICCs among the trainees, experienced radiologists, and experts were 0.85, 0.76, and 0.92, respectively. All tumors classified as category A1 showed no signs of vascular invasion at surgery. In category A2, 40% of the tumors had corresponding infiltration and required resection of the superior mesenteric vein/portal vein (SMV/PV). One of two tumors in category B2 and two of three in category C required SMV/PV resection. All six patients in category D had both arterial and venous involvement.CONCLUSION:There is almost perfect agreement among radiologists with different levels of expertise in regards to the local staging of PDAC. For tumors in a more advanced preoperative category, an increased risk for vascular involvement was noticed at surgery.

    • Many organisations have long-term maintenance requirements for their digital assets, and it is often the case that those assets outlive the tools that were used to generate them. In many usage scenarios such assets are kept in proprietary file formats, thus increasing the risks for the organisation. In this paper we report from a case study aiming to analyse tool support for data curation and long-term maintenance of documents in standardised XML-based document formats. Our investigation includes analysis of both Open Source and proprietary tools using a rich resource of corporate documents initially created in a proprietary document format. Specifically, we investigate a number of tools implementing ODF and OfficeOpen XML in order to establish their support for data curation and long-term maintenance of documents. We found that support for data curation by migration from the investigated proprietary document format to the two standardised XML-based document formats do not satisfy the organisation’s expectations. Our study reports on a range of problems associated with the use of proprietary file formats and provides a basis for strategic consideration of these issues.

    • The article presents and analyses the editorial ideals and practices of the Enlightenment press with regard to their participatory elements. The decline of these ideals and practices, during the era of the establishment of the modern press in the 1820s, 1830s and 1840s, is discussed with regard to its advocates’ self-legitimating ambitions. These professional efforts were carried out in various ways, including either simply ignoring their precursors or emphasizing the non-participatory qualities of their own enterprise. The great influence of these modern editors’ narratives on both past and present is tracked up to the present day.

    • Current techniques for modeling software architecture do not provide sufficient support for modeling architectural design rules. This is a problem in the context of model-driven development in which it is assumed that major design artifacts are represented as formal or semi-formal models. This article addresses this problem by presenting an approach to modeling architectural design rules in UML at the abstraction level of the meaning of the rules. The high abstraction level and the use of UML makes the rules both amenable to automation and easy to understand for both architects and developers, which is crucial to deployment in an organization. To provide a proof-of-concept, a tool was developed that validates a system model against the architectural rules in a separate UML model. To demonstrate the feasibility of the approach, the architectural design rules of an existing live industrial-strength system were modeled according to the approach.

    • An important purpose of architectural design is to ensure that the system meets its quality requirements by defining a set of system wide design decisions. An important part of these design decisions is the set of architectural design rules that shall be followed by developers in the detailed design. The state of practice is to define these rules in natural language and to use manual reviews to enforce them. This way of transferring the rules to the developers is however error prone and requires a lot of effort from the architects since natural language is ambiguous and open for different interpretations and rule following have to be checked with manual reviews. This paper reports from an action case study where a novel approach for architectural modeling and automated conformance checking has been investigated regarding its ability to better communicate architectural design decisions to the developers. The findings indicate that the novel approach is significantly more effective than the state of practice. The findings also show that an important reason for this is that using a tool for conformance checking allows the developers to learn the rules by experimenting.
