  • Gene expression profiling of periodontitis-affected gingival tissue by spatial transcriptomics

    • Periodontitis is a highly prevalent chronic inflammatory disease of the periodontium, leading ultimately to tooth loss. In order to characterize the gene expression of periodontitis-affected gingival tissue, we have here simultaneously quantified and localized gene expression in periodontal tissue using spatial transcriptomics, combining RNA sequencing with histological analysis. Our analyses revealed distinct clusters of gene expression, which were identified to correspond to epithelium, inflamed areas of connective tissue, and non-inflamed areas of connective tissue. Moreover, 92 genes were identified as significantly up-regulated in inflamed areas of the gingival connective tissue compared to non-inflamed tissue. Among these, immunoglobulin lambda-like polypeptide 5 (IGLL5), signal sequence receptor subunit 4 (SSR4), marginal zone B and B1 cell specific protein (MZB1), and X-box binding protein 1 (XBP1) were the four most highly up-regulated genes. These genes were also verified as significantly higher expressed in gingival tissue of patients with periodontitis compared to healthy controls, using reverse transcription quantitative polymerase chain reaction. Moreover, the protein expressions of up-regulated genes were verified in gingival biopsies by immunohistochemistry. In summary, in this study, we report distinct gene expression signatures within periodontitis-affected gingival tissue, as well as specific genes that are up-regulated in inflamed areas compared to non-inflamed areas of gingival tissue. The results obtained from this study may add novel information on the genes and cell types contributing to pathogenesis of the chronic inflammatory disease periodontitis.

  • The continuing advance and retreat of rural settlement in the northern inland of Sweden

    • In 1960, a range of leading rural geographers started a debate about population development and the “advance and retreat” of human settlement in sparsely populated rural areas, including in the inland north of Sweden. In what came to be known as the “Siljan Symposium,” they identified a number of key themes in relation to migration and human mobility that were thought to determine settlement patterns in the inland north, including: internal migration and urbanisation of populations; the role of simultaneous in- and out-migration in re-shaping settlement patterns; redistribution of rural populations through return migration and international migration; and changing preferences for settlement in different northern “zones” based on the methods for exploiting natural resources for agriculture, forestry, mining and energy production. This paper re-visits the main themes from the 1960 Siljan Symposium and examines Swedish register data to identify how migration patterns and the resulting “advance and retreat” of human settlement have changed across the inland of Västerbotten and Norrbotten. The results suggest that, while general urban-rural and regional- local settlement patterns appear to have been relatively consistent, new forms of migration (including internal, return and international) with different preferences for rural settlement emerging in different localities as a result of both persistent (mining, forestry, energy) and changing (tourism, lifestyle) values of natural resources. We also observe substantial differences in migration and urbanisation rates between Norrbotten and Västerbotten. The paper then discusses how the persistence and discontinuity of experiences over the past decades may provide insights into the potential future patterns of northern settlement.

  • Perlecan heparan sulfate deficiency impairs pulmonary vascular development and attenuates hypoxic pulmonary hypertension

    • Aims Excessive vascular cell proliferation is an important component of pulmonary hypertension (PH). Perlecan is the major heparan sulfate (HS) proteoglycan in the vascular extracellular matrix. It binds growth factors, including FGF2, and either restricts or promotes cell proliferation. In this study, we have explored the effects of perlecan HS deficiency on pulmonary vascular development and in hypoxia-induced PH. Methods and results In normoxia, Hspg2(Delta 3/Delta 3) mice, deficient in perlecan HS, had reduced pericytes and muscularization of intra-acinar vessels. Pulmonary angiography revealed a peripheral perfusion defect. Despite these abnormalities, right ventricular systolic pressure (RVSP) and myocardial mass remained normal. After 4 weeks of hypoxia, increases in the proportion of muscularized vessels, RVSP, and right ventricular hypertrophy were significantly less in Hspg2(Delta 3/Delta 3) compared with wild type. The early phase of hypoxia induced a significantly lower increase in fibroblast growth factor receptor-1 (FGFR1) protein level and receptor phosphorylation, and reduced pulmonary artery smooth muscle cell (PASMC) proliferation in Hspg2(Delta 3/Delta 3). At 4 weeks, FGF2 mRNA and protein were also significantly reduced in Hspg2(Delta 3/Delta 3) lungs. Ligand and carbohydrate engagement assay showed that perlecan HS is required for HS-FGF2-FGFR1 ternary complex formation. In vitro, proliferation assays showed that PASMC proliferation is reduced by selective FGFR1 inhibition. PASMC adhesion to fibronectin was higher in Hspg2(Delta 3/Delta 3) compared with wild type. Conclusions Perlecan HS chains are important for normal vascular arborization and recruitment of pericytes to pulmonary vessels. Perlecan HS deficiency also attenuates hypoxia-induced PH, where the underlying mechanisms involve impaired FGF2/FGFR1 interaction, inhibition of PASMC growth, and altered cell-matrix interactions.

    • © 2019 The European Pediatric Pulmonary Vascular Disease Network is a registered, non-profit organization that strives to define and develop effective, innovative diagnostic methods and treatment options in all forms of pediatric pulmonary hypertensive vascular disease, including pulmonary hypertension (PH) associated with bronchopulmonary dysplasia, PH associated with congenital heart disease (CHD), persistent PH of the newborn, and related cardiac dysfunction. The executive writing group members conducted searches of the PubMed/MEDLINE bibliographic database (1990–2018) and held face-to-face and web-based meetings. Ten section task forces voted on the updated recommendations, based on the 2016 executive summary. Clinical trials, meta-analyses, guidelines, and other articles that include pediatric data were searched using the term “pulmonary hypertension” and other keywords. Class of recommendation (COR) and level of evidence (LOE) were assigned based on European Society of Cardiology/American Heart Association definitions and on pediatric data only, or on adult studies that included >10% children or studies that enrolled adults with CHD. New definitions by the World Symposium on Pulmonary Hypertension 2018 were included. We generated 10 tables with graded recommendations (COR/LOE). The topics include diagnosis/monitoring, genetics/biomarkers, cardiac catheterization, echocardiography, cardiac magnetic resonance/chest computed tomography, associated forms of PH, intensive care unit/lung transplantation, and treatment of pediatric PH. For the first time, a set of specific recommendations on the management of PH in middle- and low-income regions was developed. Taken together, these executive, up-to-date guidelines provide a specific, comprehensive, detailed but practical framework for the optimal clinical care of children and young adults with PH.

  • Repopulating and revitalising rural Sweden? Re-examining immigration as a solution to rural decline

    • Increasing international immigration is often portrayed as a potential solution to persistent economic and population decline in rural areas. Based on longitudinal register data, this study examines the extent to which international migration has contributed to demographic and labour market changes in rural Sweden between 1990 and 2010. Results show that the urbanisation rate of international migrants is very high while their employment rate in rural areas remains comparatively low. Small positive changes are noticeable in the rate of higher education, self-employment and employment in new service-related industries among particular groups of immigrants. Immigrants to rural areas are a highly heterogeneous group in terms of their demographic and labour market characteristics, which should be considered when estimating the contributions of immigration to socio-economic development in rural areas. This study shows that, while international migration may dampen population decline in rural areas to some extent, particularly in the working-age groups, its potential to stimulate socio-economic revitalisation in rural areas needs to be questioned and examined from a more nuanced and longitudinal perspective.
