- Objective To study the clinical and biochemical effects of a low starting dose for gold therapy in rheumatoid arthritis patients with a contact allergy to gold. Methods Serum cytokines were assayed before and 24 h after the first injection of gold sodium thiomalate (GSTM). Results Contact allergy to gold was found in 4 of 19 patients. Compared to gold-negative patients (starting dose: 10 mg GSTM), there was a larger increase in serum TNFalpha (p < 0.05), sTNF-R1 (NS), and IL-1 ra (p < 0.05) in gold-allergic patients. Conclusions Cytokines are released in blood by GSTM in RA patients with gold allergy. To minimize the risk of acute adverse reactions the starting dose of GSTM should be lowered to 5 mg. Alternatively, patients should be patch-tested before gold therapy; in test-positive cases, 5 mg is recommended as the first dose.

Interstitial laser hyperthermia of a rat liver adenocarcinoma

- The aim of this study was to compare interstitial laser thermotherapy with excision of a liver tumour. A dimethylhydrazine-induced adenocarcinoma was implanted into the left lateral lobe of the rat liver, and treatment was performed 8 days later. Rats were treated with resection of the tumour-bearing lobe or underwent interstitial laser thermotherapy, which was performed at a steady-state temperature of 46°C for 30 min, 3 mm from the tumour margin. The incidence and extent of intraperitoneal spread was smaller after laser thermotherapy than after resection, with no difference in local control. Using inoculation of tumour cell suspensions into the lateral and the median lobes of the liver simultaneously and treating the lateral lobe tumour only, we found that laser thermotherapy reduced take and growth of the untreated tumour in the median lobe indicating that laser thermotherapy may induce immunologic effects. It is concluded that interstitial laser thermotherapy reduces spread of liver tumour as compared to resection. It is suggested that this can be at least partly explained by a laser-induced immunologic effect.

- This paper outlines a joint industry-academia research project in the area of method engineering. Founded in practical experiences and emerging theoretical constructs, the project aims at developing theories, methods and tools to support the adaptation, integration and construction of method components for flexible configuration of system development methods. By explicating the possibilities of using method's inherent rationale, the possibilities to adopt rigorous methods (such as the Rational Unified Process) to comply with increasing demands of flexibility will be exploited. The paper also addresses the approach to technology transfer adopted in the project, viewing the project as existing in two different intellectual spheres, one academic and one industrial. The two spheres overlap in a space of conceptualization and interpretation shared by the practitioners and academic researchers involved. This way the project adopts an iterative process of reflection and application, generating knowledge directly applicable in industry as well as knowledge of theoretical and scientific importance.
