- Patients operated on for complex congenital heart malformations need continuous follow-up. We present a male patient born in 1948 with Fallot's anomaly. A total correction was performed when he was 21 years old. Twenty-eight years after the operation, at routine follow-up, he presented with a significant left-to-right shunt because of a new ventricular septal defect. During the operation we found the original patch to be fractured with a central perforation. The patient received a new patch and has been without any clinical symptoms since. © 2006 by The Society of Thoracic Surgeons.

- This study has explored the possibility and potential of using a thermal active mass system for reducing the temperature raise and to increase the thermal comfort in an office room. The Controlled Active Mass (CAM) will be used as a heat sink is aimed to absorb heat from the room in order to increase the thermal comfort. Full-scale measurements have been used to evaluate the temperature, velocity and thermal comfort values, PMV, PPD and the equivalent temperature (Teq). The measurements showed that although the CAM size is relatively small, compared to the office room and the temperature difference between the room air and the temperature of the CAM is rather moderate the influence on the thermal comfort and indoor environment is quite apparent. Especially when the surfaces of the CAM were black the thermal comfort increases. The black CAM gave a larger impact on the indoor air since it was able to absorb more heat from the room surfaces than the polished CAM did. The CAM does not cause any uncomfortable draft and since it’s a low-exergy system it has a large potential for the future.

- De senaste åren har kulturens ekonomi kommit alltmer i fokus. Dessutom har en mängd offentliga satsningar på de kreativa näringarna eller upplevelseindustrin påbörjats, satsningar som har mötts av antingen hejarop eller avståndstaganden. Den här artikeln vill mana till eftertanke: att varken hurra för mycket eller gå förbi för snabbt. Konceptet kreativa näringar har blivit alltför spritt och starkt för att inte tas på allvar och bli föremål för en djupare diskussion. Artikeln presenterar en bakgrund till hur kulturen förändras och varför liksom till vilka ansatser som märks i policydiskussionen. Den presenterar också en modell för att förklara olika perspektiv på kreativa näringar och argumenterar för att kulturpolitiken måste förändras och involvera marknaden.

Neuron specific enolase and S-100B as predictors of outcome after cardiac arrest and induced hypothermia.

- AIM: To assess the prognostic value of repetitive serum samples of neuron specific enolase (NSE) and S-100B in cardiac arrest patients treated with hypothermia. METHODS: In a three-centre study, comatose patients after cardiac arrest were treated with hypothermia at 33 degrees C for 24h, regardless of cause or the initial rhythm. Serum samples were collected at 2, 24, 48 and 72h after the arrest and analysed for NSE and S-100B in a non-blinded way. The cerebral performance categories scale (CPC) was used as the outcome measure; a best CPC of 1-2 during 6 months was regarded as a good outcome, a best CPC of 3-5 a poor outcome. RESULTS: One centre was omitted in the NSE analysis due to missing 24 and 48h samples. Two partially overlapping groups were studied, the NSE group (n=102) and the S-100B group (n=107). NSE at 48h >28mug/l (specificity 100%, sensitivity 67%) and S-100B >0.51mug/l at 24h (specificity 96%, sensitivity 62%) correlated with a poor outcome, and so did a rise in NSE of >2mug/l between 24 and 48h (odds ratio 9.8, CI 3.5-27.7). A majority of missing samples (n=123) were from the 2h sampling time (n=56) due to referral from other hospitals or inter-hospital transfer. CONCLUSION: NSE was a better marker than S-100B for predicting outcome after cardiac arrest and induced hypothermia. NSE above 28mug/l at 48h and a rise in NSE of more than 2mug/l between 24 and 48h were markers for a poor outcome.
