  • Results from the "Me & My Heart" (eMocial) Study: a Randomized Evaluation of a New Smartphone-Based Support Tool to Increase Therapy Adherence of Patients with Acute Coronary Syndrome

    • Purpose This study evaluated whether patient support, administered via an electronic device-based app, increased adherence to treatment and lifestyle changes in patients with acute coronary syndrome (ACS) treated with ticagrelor in routine clinical practice. Methods Patients (aged >= 18 years) with diagnosed ACS treated with ticagrelor co-administered with low-dose acetylsalicylic acid were randomized into an active group (with support tool app for medication intake reminders and motivational messages) and a control group (without support tool app), and observed for 48 weeks (ClinicalTrials.gov Identifier: NCT02615704). Patients were asked to complete the 36-item Short-Form Health Survey (SF-36) and Lifestyle Changes Questionnaire (LSQ), and were assessed for blood pressure and body mass index (BMI) at baseline (visit 1) and at the end of the study (visit 2). Medication adherence was measured using the Brilique Adherence Questionnaire (BAQ). Results Patients (N = 676) were randomized to an active (n = 342) or a control (n = 334) group. BAQ data were available for 174 patients in the active group and 174 patients in the control group. Over the 48-week period, mean (standard deviation) adherence for the active and control groups was 96.4% (13.2%) and 91.5% (23.1%), respectively (effect of app intervention, p < 0.05). There were no significant differences in blood pressure and BMI between visits. General improvements in SF-36 and LSQ scores were observed for both groups. Conclusion The patient support tool app was associated with significant improvements in patient-reported treatment adherence compared with a data collection app alone in patients prescribed ticagrelor for ACS.

  • Culture of human ovarian tissue in xeno-free conditions using laminin components of the human ovarian extracellular matrix

    • Purpose: Our purpose was to identify human ovarian extracellular matrix (ECM) components that would support in vitro culture of human ovarian tissue and be compatible with possible future clinical applications. We characterized ovarian expression of laminins and selected three laminin tripeptides for culture experiments to be compared with Matrigel, an undefined and animal-based mixture of ECM components. Methods: Expression of the 12 laminin genes was determined on transcript and protein levels using cortical tissue samples (n = 6), commercial ovary RNA (n = 1), follicular fluid granulosa cells (n = 20), and single-cell RNA-sequencing data. Laminin 221 (LN221), LN521, LN511, and their mixture were chosen for a 7-day culture experiment along with Matrigel using tissue from 17 patients. At the end of the culture, follicles were evaluated by scoring and counting from serial tissue sections, apoptosis measured using in situ TUNEL assay, proliferation by Ki67 staining, and endocrine function by quantifying steroids in culture media using UPLC-MS/MS. Results: Approximately half of the cells in ovarian cortex expressed at least one laminin gene. The overall most expressed laminin α-chains were LAMA2 and LAMA5, β-chains LAMB1 and LAMB2, and γ-chain LAMC1. In culture experiments, LN221 enhanced follicular survival compared with Matrigel (p < 0.001), whereas tissue cultured on LN521 had higher proportion of secondary follicles (p < 0.001). LN511 and mixture of laminins did not support the cultures leading to lower follicle densities and higher apoptosis. All cultures produced steroids and contained proliferating cells. Conclusions: LN221 and LN521 show promise in providing xeno-free growth substrates for human ovarian tissue cultures, which may help in further development of folliculogenesis in vitro for clinical practices. The system could also be used for identification of adverse effects of chemicals in ovaries.

    • We describe a novel nature-derived epoxy resin monomer (ERM) derived from the plant lignan pinoresinol. Epoxy resins are thermosetting materials in global usage owing to their excellent technical properties such as flexibility and durability. However, their adverse health effects are often not considered and affect users of epoxy resins worldwide. Components of epoxy resin systems are strong skin sensitizers and cause allergic contact dermatitis. The reported prevalence attributable to epoxy chemicals is between 11.7 and 12.5% of all cases of occupational allergic contact dermatitis. We are committed to developing epoxy resins with reduced allergenic effect, while maintaining their excellent properties. The novel ERM, pinoresinol diglycidyl ether (PinoDGE), was synthesized in one step from pinoresinol and epichlorohydrin in 88% yield. It was not classified as a skin sensitizer in the in vivo local lymph node assay, at concentrations up to 0.17 m, as it did not cause a stimulation index >3 compared to control. Pinoresinol diglycidyl ether reacted with the model peptide AcPHCKRM in a reactivity assay and was predicted to be a skin sensitizer in the KeratinoSens assay. Preliminary cross-linking studies indicate that it has promising properties compared to commercially used ERMs. Pinoresinol diglycidyl ether could be seen as a lead compound for further development of alternative ERMs with a better safety profile based on natural and renewable sources for construction of epoxy resin polymers.

  • Prediction of Adult Height by Machine Learning Technique

    • Context: Prediction of AH is frequently undertaken in the clinical setting. The commonly used methods are based on the assessment of skeletal maturation. Predictive algorithms generated by machine learning, which can already automatically drive cars and recognize spoken language, are the keys to unlocking data that can precisely inform the pediatrician for real-time decision making. Objective: To use machine learning (ML) to predict adult height (AH) based on growth measurements until age 6 years. Methods: Growth data from 1596 subjects (798 boys) aged 0-20 years from the longitudinal GrowUp 1974 Gothenburg cohort were utilized to train multiple ML regressors. Of these, 100 were used for model comparison, the rest was used for 5-fold cross-validation. The winning model, random forest (RF), was first validated on 684 additional subjects from the 1974 cohort. It was additionally validated using 1890 subjects from the GrowUp 1990 Gothenburg cohort and 145 subjects from the Edinburgh Longitudinal Growth Study cohort. Results: RF with 51 regression trees produced the most accurate predictions. The best predicting features were sex and height at age 3.4-6.0 years. Observed and predicted AHs were 173.98.9 cm and 173.9 +/- 7.7 cm, respectively, with prediction average error of -0.4 +/- 4.0 cm. Validation of prediction for 684 GrowUp 1974 children showed prediction accuracy r=0.87 between predicted and observed AH (R-2 = 0.75). When validated on the 1990 Gothenburg and Edinburgh cohorts (completely unseen by the learned RF model), the prediction accuracy was r = 0.88 in both cases (R-2 = 0.77). AH in short children was overpredicted and AH in tall children was underpredicted. Prediction absolute error correlated negatively with AH (P < .0001). Conclusion: We show successful, validated ML of AH using growth measurements before age 6 years. The most important features for prediction were sex, and height at age 3.4-6.0. Prediction errors result in over- or underestimates of AH for short and tall subjects, respectively. Prediction by ML can be generalized to other cohorts.

    • In a circular economy approach, edible filamentous fungi (single cell protein) can be cultivated on volatile fatty acids (VFAs) derived from anaerobic digestion (AD) of organic-rich waste streams. In this study, the effect of pH, concentration/distribution of VFAs, nutrient supplementation, and type of waste on Aspergillus oryzae cultivation on synthetic VFAs, and actual VFAs derived from AD of food waste and cow manure were investigated. The optimal pH for A. oryzae growth on VFAs were 6 and 7 with maximum acetic acid consumption rates of 0.09 g/L. h. The fungus could thrive on high concentrations of acetic (up to 9 g/L) yielding 0.29 g dry biomass/gVFAs(fed). In mixed VFAs cultures, A. oryzae primarily consumed caproic and acetic acids reaching a biomass yield of 0.26 g dry biomass/gVFAs(fed) (containing up to 41% protein). For waste-derived VFAs at pH 6, the fungus successfully consumed 81-100% of caproic, acetic, and butyric acids.

    • Aim To update the Swedish references for weight, weight-for-height and body mass index (BMI) considering the secular trend for height but not including that for weight. Methods Longitudinal measures of height and weight were obtained (0-18 years) from 1418 (698 girls) healthy children from the GrowUp 1990 Gothenburg cohort born at term to non-smoking mothers and Nordic parents. A total of 145 individuals with extreme BMI value vs GrowUp 1974 BMI SDS reference were excluded (0-2 years: +/- 4SDS, 2 < years: -3SDS, +2.3SDS). References were constructed using the LMS method. Results The updated weight reference became similar to the GrowUp 1974 Gothenburg reference: BMI increased rapidly up to lower levels in the 1990 cohort during infancy/early childhood, similar in both groups in late childhood/adolescence, despite lower values at +2SDS. Compared with the WHO weight standard, median and -2SDS weight values were higher for the 1990 cohort, whereas +2SDS values were lower, resulting in narrower normal range. Median values were greater and +/- 2SDS narrower for the 1990 vs the WHO weight-for-height reference. International Obesity Task force (IOTF) BMI lines for definitions for over- and underweight were added. Conclusion We present updated references for weight, weight-for-height and BMI, providing a healthy goal for weight development when monitoring growth within healthcare settings.

    • Electrochromic (EC) technology allows control of the transmission of visible light and solar radiation through thin-film devices. When applied to “smart” windows, EC technology can significantly diminish energy use for cooling and air conditioning of buildings and simultaneously provide good indoor comfort for the buildings’ occupants through reduced glare. EC “smart” windows are available on the market, but it is nevertheless important that their degradation under operating conditions be better understood and, ideally, prevented. In the present work, we investigated EC properties, voltammetric cycling durability, and potentiostatic rejuvenation of sputter-deposited WO3 thin films immersed in LiClO4–propylene carbonate electrolytes containing up to 3.0 wt% of ∼7-nm-diameter SiO2 nanoparticles. Adding about 1 wt% SiO2 led to a significant improvement in cycling durability in the commonly used potential range of 2.0–4.0 V vs. Li/Li+. Furthermore, X-ray photoemission spectroscopy indicated that O–Si bonds were associated with enhanced durability in the presence of SiO2 nanoparticles.

  • Electrochromism of nitrogen-doped tungsten oxide thin films

    • Tungsten-oxide-based thin films were prepared by reactive DC magnetron sputtering in the presence ofoxygen and nitrogen. Nitrogen contents up to 12 at.% were documented by Rutherford backscattering spectrometry and time-of-flight elastic recoil detection analysis. Optical and electrochemical measurements showed that films with up to 4 at.% of nitrogen were as transparent as undoped tungsten oxide films and displayed enhanced electrochromic properties manifested in an increase in the coloration efficiency by as much as 20%.

    • Materials that are able to vary their transparency and coloration reversibly when they are subjected to an electrical current or voltage are referred to as “electrochromic” (EC). High optical transmittance modulation and long service lifetime are apparent requirements for EC materials used in smart windows technology. An extended service lifetime is provided by the long-term durability of the materials. One important aspect of durability is the ability to sustain charge transport between the EC film and electrolyte, or between the two EC films in a device, for many hundreds or thousands of cycles without any significant changes in the performance such as optical modulation and inserted-extracted charge. The purpose of this study is to clarify the effects of the oxygen-argon gas flow ratio during sputter deposition on the durability of WO3 films. In this study, the oxygen to argon gas-flow ratio was modulated by setting the O2 flow rate to 7.5, 15.0, 22.5, and 45.0 ml min-1 and using a fixed Ar flow rate of 50 ml min-1. Thus, the oxygen to argon gas-flow ratio was varied from 0.15 to 0.90. The pressure in the sputter plasma was set as 30 mTorr and the sputter power was maintained at 200 W. For durability studies, cyclic voltammetry data were recorded for up to 500 cycles between 2.0 and 4.0 V versus Li/Li+ at a scan rate of 20 mV s-1. High oxygen to argon gas ratio was found to have a positive effect on the EC properties of the films. When the long-term performance of the films was examined, it was seen that all the samples displayed a slow decline of the colored-state transmittance due to ion accumulation in the host material. After 500 color-bleach cycles, the maximum optical transmittance modulation between colored and bleached states at a wavelength of 528 nm was 63.6% when the oxygen to argon gas-flow ratio was 0.90.
