A controlled randomized trial of budesonide versus prednisolone retention enemas in active distal ulcerative colitis

- Sixty-four patients with active distal ulcerative colitis participated in a multicentre, randomized, investigator-blind trial to compare the effect of budesonide enema, 2 mg/100 ml, with prednisolone disodium phosphate enema, 31.25 mg/100 ml. Budesonide is a new potent corticosteroid with a rapid first-pass elimination. The patients were treated for 4 weeks, and the efficacy of the drugs were evaluated by sigmoidoscopy, histology, and subjective symptoms after 2 and 4 weeks. After 4 weeks of treatment 16 of 31 patients (52%) receiving budesonide enema had healed endoscopically, compared with 8 of 33 (24%) (p = 0.045) receiving prednisolone enema. Budesonide was superior to prednisolone in terms of both significantly improved sigmoidoscopic and histologic scores and subjective symptoms evaluated by visual analogue scales. The patients receiving prednisolone had a significant depression of endogenous cortisol levels during the treatment period, but not the patients receiving budesonide. Budesonide enema seems to be a promising therapy for active distal ulcerative colitis and causes no adverse reactions

Birds doing it the octopus way : fright moulting and distraction of predators

- "Fright moulting" has been recorded in several orders of birds and interpreted as a way of escaping a predator when already caught. Two observations suggest that the behaviour may have another function, viz. when an aerial predator chases the prey. By shedding body feathers in the air the bird creates a cloud behind it; this might confuse the predator in the same way as the ink of octopuses confuse their predators.

Smoking habits in patients with inflammatory bowel disease. A case control study.

- In a case-control study there were significantly fewer smokers among patients with ulcerative colitis both at disease onset and at interview, the relative risk compared with non-smokers being 0.33 and 0.12, respectively. Among female patients with Crohn's disease there were significantly more smokers, the relative risk being 2.70 and 2.33, respectively. Of the ex-smokers with ulcerative colitis, two-thirds became ill after they stopped smoking, and most of these during the first years after stopping. Neither in ulcerative colitis nor in Crohn's disease could any relation be found between the localization of disease and smoking habits at the time of diagnosis. The findings in the present study support the hypothesis that smoking may influence the course of inflammatory bowel disease

- The effect of lymph diversion on plasma apolipoprotein A-I levels was studied. In lymph fistula rats apolipoprotein A-I levels in plasma stayed constant in spite of a loss of an equivalent of one-half plasma pool of apolipoprotein A-I per day through the lymph fistula. This indicates that synthesis of apolipoprotein A-I increases or that catabolism of apolipoprotein A-I decreases in a compensatory manner as intestinal apolipoprotein A-I is diverted. By using incorporation of [3H]leucine into newly synthesized apolipoprotein A-I it was shown that 2.6-times as much [3H]leucine was incorporated into apolipoprotein A-I in thoracic duct drained animals compared to controls. In experiments in which 125I-labeled HDL was injected intravenously into rats, it was shown that catabolism of HDL and apolipoprotein A-I was not decreased in lymph-drained rats. These data thus suggest that an increased synthesis of apolipoprotein A-I occurs when the intestinal contribution of apolipoprotein A-I diminishes. This is probably due to an increase in liver protein synthesis.

- The need for more reliable finite element analyses of materials with complex behaviour has led to the development of new constitutive theories. These theories often have a large number of parameters which must be determined from experiments. A method is described of fitting constitutive parameters through optimization. An application is made to the Prevost multisurface plasticity theory. Good agreement is obtained between theoretical results and experimental data in the case of a multi axially loaded hard metal powder.

The SSEM Model : A Brief Description

- This short paper presents the main features of a new minimal model of the Swedish economy that is currently being developed at the Stockholm School of Economics. Both the model and the results presented are to be regarded as preliminary, since the work on the model is still in progress. During the 1950's and 60's econometric model building was predominantly directed toward the construction of large-scale models. One may say that the general attitude was that sifficiently detailed and elaborated models could help solving most problems in policy analysis and forecasting. With the more unstable economics conditions of the 1970's many large-scale models ran into serious trouble. The size of the models made it difficult to see what went wrong. The need for econometric models that are more closely linked to recent work in macro theory has stimulated work with small-scale, ghighly aggregative econometric models with strong emphasis on their theoretical underpinnings. The work with the SSEM model can be said to have got its inspiration from this development. In structure and in specification, the model is similar to work initiated at the London School of Economics by A R Bergstrom et al. Examples of this type of model can be found in Bergstrom and Wymer (1974), Jonson (1976b) and Knight and Wymer (1978). Jonson has continued this work at the Reserve Bank of Australia, where he has been rsponsible fot the development of the RBA model of Australia.
