  • Is malnutrition a risk factor for incident urinary tract infection among older people in residential care facilities?

    • Background: Malnutrition and urinary tract infections (UTI) are common among older people living in residential care facilities.Objectives: To determine whether malnutrition is a risk factor for incident urinary tract infection in people aged ≥65 years living in residential care facilities.Design, Setting, and Participants: A prospective cohort study of people living in residential care facilities in northern Sweden (N=373). Data from the Frail Older People-Activity and Nutrition and Umeå Dementia and Exercise studies were used.Measurements: Malnutrition was assessed using the Mini Nutritional Assessment (MNA). Risk factors for UTI were explored using univariate and multivariate Cox proportional hazard regression analyses. Maximum follow-up time was 9 months.Results: The incidence of UTI was 460/1000 person-years; 85/276=30.8% of women and 16/97=16.5% of men contracted UTIs. History of UTI (hazard ratio [HR] 2.804, 95% confidence interval [CI] 1.824–4.311), heart failure (HR 2.101, 95% CI 1.368–3.225), hypertension (HR 1.656, 95% CI 1.095–2.504), and low Mini-Mental State Examination (MMSE) score (HR 0.937, 95% CI 0.892–0.985) were associated independently with higher risk of incident UTI in multivariate analyses. Malnutrition was not associated with UTI in the whole sample or in women; MNA score was associated with UTI in men in univariate analysis (HR 0.841, 95% CI 0.750–0.944).Conclusion: The incidence of UTI was high in residential care facilities and individuals with histories of UTI, heart failure, hypertension, or cognitive impairment were more likely to be affected. Malnutrition was not a risk factor for UTI in the whole sample or in women, but may constitute a risk for UTI among men.

  • Prevalence of dental caries and influencing factors, time trends over a 30-year period in an adult population : Epidemiological studies between 1983 and 2013 in the county of Dalarna, Sweden

    • Objective. The aim of this study was to investigate the prevalence of dental caries in an adult population using four different cross-sectional studies over a 30-year period and to assess its possible associations with socio-economic and socio-behavioral factors. Materials and methods. Four cross-sectional epidemiological studies were performed in the county of Dalarna, Sweden, in 1983, 2003, 2008, and 2013. Random samples of 1012–2243 individuals, aged 20–85 years, who answered a questionnaire about socio-economic and socio-behavioral factors, were radiographically and clinically examined. Results. The proportion of individuals with at least one decayed surface (DS) was 58% in 1983 and significantly lower, 34% in 2008 (p<0.05) and 33% in 2013; the mean number of DS was 2.0 in 1983 and 1.1 in 2013 in the age group 35 to 75 (p < 0.05). In the age group 85, the mean number of DS was 1.2 in 2008 and 2.4 in 2013. Adjusted for age and number of teeth, irregular dental visits, limited financial resources for dental care, smoking, education below university, male gender, daily medication, and single living were positively and statistically associated with manifest caries. Conclusion. The declining trend in the prevalence of manifest caries seems to be broken. In the oldest age group mean number of DS was higher in 2013 compared with 2008, indicating a possible beginning of an increase.  This needs special attention as this group increases in the population, retaining natural teeth high up in age. Manifest caries was found to be associated with socio-economic and socio-behavioral factors.

  • Customer orientation in a Swedish county council

    • PurposeThe use of the customer concept and ways of interacting with customers in support functions are relatively new areas of interest for public organizations. The aim of this study is to describe the development towards a stronger customer orientation in a support function in a Swedish county council from a management team perspective.Design/methodology/approachA revision of plans and annual reports and individual interviews with the members of a management team was done. The interviews were examined using qualitative content analysis.FindingsThe results are presented in one overarching theme: the double-edged customer concept, and three themes: meeting the customer’s needs, being the customer’s specialist, and developing in collaboration with the customer. The development of a customer orientation is illustrated as a spiral involving the concepts of understanding, wanting, and acting.Originality/valueThe paper contributes to new understanding of how customer orientation develops in the context of public organizations’ support functions.

    • My current research focuses on textiles and rites, especially woven textiles for funerals and moments of loss. What active role can a textile such as an infant-wrapping cloth or a funeral pall play in the mourning process? This article will describe the development and current questions that address 1) the infant-wrapping cloth – the textile that is used to dress, clothe, or cover the dead body with particular attention to the question of infant mortality and the material practices of care. 2) The funeral pall that is used at funerals, draped over the coffin or as a body cover at hospital viewing rooms. One example to be presented is Kortedalakrönika (‘The Chronicle of Kortedala’), a collaborative project, woven for a church in Gothenburg. My work is based in artistic practice but opens up several scientific and existential questions.

  • I ritens rum - om mötet mellan tyg och människa : In a Room of Rites – Cloth Meeting Human

    • The project In a Room of Rites – Cloth Meeting Human is an artistic research project on textiles, rites and death, where I create textile art in the form of a funeral pall (in the artwork: Kortedalakrönika (Kortedala Chronicle) and infant wrapping cloths (in the artwork: I sin linda/Wrapped in Cloth). The initial aim of the project is to explore what textiles can mean at farewells and in funeral rituals, as well as to reflect on the experiences I gain in my art making. Funeral textiles in the form of funeral palls is an old tradition that has gained renewed relevance during the last decade. Through my own work with funeral palls in recent years, ideas of new textiles within this context gradually emerged, one of these being a wrapping cloth intended for children who were stillborn, or who died during childbirth, and where I asked myself the questions: what should a wrapping cloth of this kind look like, and how should it be woven? By weaving, and reflecting on these questions, my practical knowledge of textile art-making grows and develops – in a context where an intertwining interaction takes place between human life and cloth. My exploring takes an inward direction towards an artistic act of doing, and takes a more outward direction by examining the textiles in their contexts – together with the people who use them – through an inquiry moving between material, room and action. The manufacturing of textiles through hand-weaving, collective making and industrial production, also creates possibilities for reflecting on other aspects of doing. Artistic processes and art works are described in greater depth, and in the form of a reflexive investigation in the first person, in order to present a background narrative to my experience. Reflecting, I discovered wrapping and covering to be two very central textile-related actions when someone dies. When cloth meets human in the covering or wrapping of the dead, the meeting that comes into being here is of an absolute nature, that is: a person dies and we cover his or her face. The cloth becomes a symbol of the separating process, as well as of the separating line or border. Wrapping can, however, be an action linking birth to death. We are born into textile material, into cloths – and leave, wrapped in cloths – whilst being accompanied by a gesture of an embracing nature. How the wrapping cloths that came into being are experienced when put into practice will be the next step.

    • Certain questions, and things that happen to us, leave a deep imprint in our lives. And this is what happened to me when I was asked to weave small wrapping cloths for children who die during pregnancy or delivery. These wrapping cloths had an important task to perform in a difficult situation where words maybe fail those involved. Could the weave talk and act as words — by touching it and looking at it? In an attempt to examine this question, a booklet was produced and the wrapping cloths were also shown in a number of exhibitions. Sharing them with others led to me having contact with parents who had experienced the loss of a child, as well as the staff in hospitals, and I was so glad to receive their support and share in their experience. Preparations for a clinical study to be launched also took place, and this study is now underway at different pregnancy and childbirth clinics. The work and research that was once carried out alone has now come to be a process that I share with students, colleagues and friends in a Weaving Research Group at the HDK — Academy of Design and Crafts, University of Gothenburg. To embrace, to hold and to wrap are the words that lead us — as we weave on the same loom, and encourage each other on by saying: let’s weave — and let us weave the loveliest we can.
