  • Duodenal levodopa infusion monotherapy vs oral polypharmacy in advanced Parkinson disease

    • Objectives: To compare daytime intraduodenal levodopa/carbidopa infusion as monotherapy with individually optimized conventional combination therapies in patients with advanced Parkinson disease (PD) for motor fluctuations and quality of life (QoL). Methods: Twenty-four patients with motor fluctuations and dyskinesia were studied in a randomized crossover design to compare individualized conventional treatment and intraduodenal infusion of a levodopa/ carbidopa gel for 3 + 3 weeks. Video scoring of motor function was assessed by blinded assessors on a global Treatment Response Scale from -3 to 0 to +3 (from severe "off" to "on" to "on" with severe dyskinesia). Patient self-assessment of motor performance and QoL was done using an electronic diary. Results: Median percentage of ratings in a functional "on" interval (-1 to +1) was increased from 81 to 100% by infusion therapy (p < 0.01). This improvement was accompanied by a decrease in "off" state (p < 0.01) and no increase in dyskinesia. Median Unified Parkinson's Disease Rating Scale score decreased from 53 to 35 in favor of infusion (p < 0.05). QoL was improved, using the two instruments: Parkinson's Disease Questionnaire-39 and 15D Quality of Life Instrument (p < 0.01). Adverse events were similar for both treatment strategies. Conclusions: Continuous intraduodenal infusion of the levodopa/carbidopa enteral gel as monotherapy is safe and clinically superior to a number of individually optimized combinations of conventional oral and subcutaneous medications in patients with motor fluctuations. Intraduodenal infusion of levodopa offers an important alternative in treating patients with advanced Parkinson disease.

  • Pied Flycatchers Ficedula hypoleuca travelling from Africa to breed in Europe: differential effects of winter and migration conditions on breeding date

    • In most bird species there is only a short time window available for optimal breeding due to variation in ecological conditions in a seasonal environment. Long-distance migrants must travel before they start breeding, and conditions at the wintering grounds and during migration may affect travelling speed and hence arrival and breeding dates. These effects are to a large extent determined by climate variables such as rainfall and temperature, and need to be identified to predict how well species can adapt to climate change. In this paper we analyse effects of vegetation growth on the wintering grounds and sites en route on the annual timing of breeding of 17 populations of Pied Flycatchers Ficedula hypoleuca studied between 1982–2000. Timing of breeding was largely correlated with local spring temperatures, supplemented by striking effects of African vegetation and NAO. Populations differed in the effects of vegetation growth on the wintering grounds, and on their northern African staging grounds, as well as ecological conditions in Europe as measured by the winter NAO. In general, early breeding populations (low altitude, western European populations) bred earlier in years with more vegetation in the Northern Sahel zone, as well as in Northern Africa. In contrast, late breeding populations (high altitude and northern and eastern populations) advanced their breeding dates when circumstances in Europe were more advanced (high NAO). Thus, timing of breeding in most Pied Flycatcher populations not only depends upon local circumstances, but also on conditions encountered during travelling, and these effects differ across populations dependent on the timing of travelling and breeding.

  • A 10-GW pulsed power supply for HPM sources

    • A research activity involving the detailed consideration of novel high-voltage transformers for pulsed-power applications has recently begun at Loughborough University. Although the main goal is the demonstration of a compact and lightweight unit employing magnetic insulation under vacuum conditions, the initial stage of the work is directed toward the development of a conventional air-cored transformer as a main component in a compact power supply for high power microwave sources. In cooperation with the Swedish Defence Research Agency (FOI), the power supply has been tested with a microwave source of the vircator type. The power generator for the system uses a 70-kJ/25-kV capacitor bank and an exploding wire array to generate a 150-kV voltage pulse in the primary circuit of a transformer. A pressurized SF6 spark gap in the secondary circuit sharpens the high-voltage output so that pulses approaching 500 kV and with a rise time well below 100 ns are generated on a 20-Omega high-power resistors With the peak power in excess of 10 GW. Experimental results are presented from final testing of the system, where a reflex triode vircator replaces the 20-Omega resistor. Measurements of the microwave emission using free-field sensors are presented, and comments are made with the microwave emission from the same vircator when powered by a Marx generator at FOI.

    • In the modified SX-700 monochromator installed at beam line I411, the focusing before the exit slit is achieved by a plane elliptical mirror. Like in the original SX-700 design, the resolution is mainly limited by the slope errors of the focusing mirror. A simple method to improve the resolution is to replace the focusing mirror by one with smaller slope errors. The effects of replacing the refocusing mirror by similar but more accurate in shape are studied both by carrying out ray tracing simulations and by analytical approach.
