  • Adaptation of a Palliative Care Approach in the Context of Intensive Care: A Systematic Literature Review

    • Background Palliative care is assumed to be an integral part of care in Intensive Care Units (ICU), yet, research still indicates lack knowledge about how to adapt, integrate and adopt palliative care in curative focused contexts such as in intensive care. Aim To identify factors influencing the integration of a palliative care approach in the context of ICU. Method A systematic review based on quantitative and qualitative studies using a thematic synthesis of the articles was performed. The following search term were used: Implementation, palliative care and ICU. In total 1843 articles were screened, and 25 articles were eligible for this review. Result: The findings show that the influencing factors for integrating a palliative care approach are multifaceted. Themes were identified; facilitators and barriers for implementation and adaptation on organizational level (facilitating policies and environmental support, care pathways, prognostication , multidisciplinary team, training and education) and interpersonal level (communication conflict, emotional and spiritual support, person-centred care and patient and family involvement). Conclusion To adopt a palliative care approach into the ICU setting, the integration process requires adaption of organizational support and personal beliefs and attitudes within the entire multi-professional team including the manager as well as patient and family.

  • ‘Fit and Re-Orientation’: Unpacking Layers of Carceral Design Heritage in Contemporary Design of Special Care Homes for Youth, and Its Impact on Well-Being

    • Abstract: This paper aims to critically discuss and analyze the carceral (prisonlike) layers of the particular design heritage that form the contemporary interior design of rooms in institutions that serves as correctional facilities; such as prisons, psychiatric hospitals and so-called special youth homes. Moreover, it aims to shed a light on the impact carceral design may have on well-being for people who inhabit these spaces. The main focus of this paper will be on special youth homes that are run by The Swedish National Board of Institutional (Statens Institutionsstyrelse, SiS) and provide compulsory care and rehabilitation for children and adolescents with severe psychosocial problems, substance abuse and/or criminal behavior. The special youth homes are considered to be neither prisons nor jails. Nevertheless, some are actually located in former prisons, and the majority are built for incarceration. To analyze the layers of carceral design the concept of ‘Fit and Re-Orientation’ is used. This concept stems from a phenomenological viewpoint developed by Sarah Ahmed; “Queer phenomenology”. To elucidate incarcerated young people’s lived experiences of carceral design and the impact it has on their wellbeing in everyday life, examples of empirical data are brought in. It is further discussed how the physical environment ‘Fits or Re-Orientates’ them in the process of (re)habilitation. Finally, the authors suggest a model for how the dialectic relationship between the incarcerated person and the physical environment can be understood and why this relationship becomes counterproductive for (re)habilitating care processes and well-being.

  • The Meaning of Comfort in the Intensive Care Unit

    • Providing comfort in an intensive care unit (ICU) setting is often related to pain relief and end-of-life care; environmental factors are often neglected, despite the major role of the environment on the patients' well-being and comfort. The aim of this article was to explore the meanings of comfort from a theoretical and empirical perspective to increase the understanding of what comfort means in ICU settings. A lexical analysis and serials of workshops were performed, and data were analyzed using a qualitative content analysis. The findings from the theoretical analysis show that comfort has a broad range of synonyms related to both subjective experiences and objective and physical qualities. The findings from the empirical part reveal 4 themes: comfort in relation to nature, comfort in relation to situation and people, comfort in relation to place, and comfort in relation to objects and material. Materiality, functionality, memory, culture, and history stipulate comfort. It is challenging to discern what comfort is when it comes to function and emotions. We also found that comfort is closely linked to nature and well-being. © 2019 Wolters Kluwer Health, Inc. All rights reserved.

The outcome of care for patients sentenced to forensic psychiatric care is of importance not only for the patient but also for society, in preventing new crimes. In recent years, a person-centered perspective is influencing the care, recognizing the design of the physical environment as a therapeutic resource. To capture the complexity of patients’ experience of the physical environment, a qualitative approach is needed. The aim of this study was to investigate the meanings of the patient room as a place and space in forensic psychiatric in-patient care from the patients’ perspective. An explorative qualitative design was chosen, data were collected by photovoice; a combination of photographs, taken by the patients, followed by interviews. Eleven (N=11) patients were interviewed. The interviews were analysed by a thematic analysis method. Four themes emerged from the data revealing the meanings of the patient room as a place and space: (i) striving towards normality; (ii) being anchored and protected; (iii) being at-home and homeness; and (iv) being in communion and meaningfulness. The findings show that the physical environment has a say in patients’ basic needs and a role in maintaining normality. Substandard reveals a lack of respect and dignity towards this patient group. Involving patients in the design process of new facilities can be a way to make progress.

  • Room Design - A Phenomenological-Hermeneutical Study: A Factor in Creating a Caring Environment

    • Medical technology has progressed tremendously over the last few decades, but the same development cannot be seen in the design of these intensive care unit environments. Authors report results of a study of evidence-based room design, emphasizing the impact on conveying a caring attitude to patients. Ten nonparticipant observations were conducted in patient rooms with 2 different designs, followed by interviews. The data were analyzed using a phenomenological-hermeneutical approach. The results did not reveal that it was obvious that redesigned spaces resulted in a more caring attitude. The meanings of caring displayed during nursing activities were interpreted by interpreting gazes. Some of the nursing staff had an instrumental gaze, interpreted as caring with a task-orientated approach, while others communicated their caring with an attentive and attuned gaze, where the needs of the patients regulated the working shift. The study findings indicated that caring may not be perceived when nurses use a task-oriented approach. However, when nurses practice a person-centered approach, using an attentive and attuned gaze, caring is conveyed. Caring in intensive care contexts needs to be assisted by a supportive environment design that cultivates the caring approach. © 2019 Wolters Kluwer Health, Inc. All rights reserved.
