  • Hormone replacement therapy and hip fracture risk : population based case-control study

    • OBJECTIVE: To determine the relative risk of hip fracture associated with postmenopausal hormone replacement therapy including the effect of duration and recency of treatment, the addition of progestins, route of administration, and dose. DESIGN: Population based case-control study. Setting: Six counties in Sweden. SUBJECTS: 1327 women aged 50-81 years with hip fracture and 3262 randomly selected controls. MAIN OUTCOME MEASURE: Use of hormone replacement therapy. RESULTS: Compared with women who had never used hormone replacement therapy, current users had an odds ratio of 0.35 (95 % confidence interval 0.24 to 0.53) for hip fracture and former users had an odds ratio of 0.76 (0.57 to 1.01). For every year of therapy, the overall risk decreased by 6% (3% to 9%): 4% (1% to 8%) for regimens without progestin and 11% (6% to 16%) for those with progestin. Last use between one and five years previously, with a duration of use more than five years, was associated with an odds ratio of 0.27 (0.08 to 0.94). After five years without hormone replacement therapy the protective effect was substantially diminished (-7% to 48%). With current use, an initiation of therapy nine or more years after the menopause gave equally strong reduction in risk for hip fracture as an earlier start. Oestrogen treatment with skin patches gave similar risk estimates as oral regimens. CONCLUSIONS: Recent use of hormone replacement therapy is required for optimum fracture protection, but therapy can be started several years after the menopause. The protective effect increases with duration of use, and an oestrogen-sparing effect is achieved when progestins are included in the regimen.

  • Synthesis of theoretical and empirical experineces from nutrient and cyprinid reductions in Lake Ringsjön

    • The reduction in external phosphorus load to Lake Ringsjön during the 1980s, did not result in improved water transparency during the following ten-year period. Furthermore, a fish-kill in the Eastern Basin of the lake, in addition to a cyprinid reduction programme (biomanipulation; 1988–1992), in contrast to theory, did not lead to any increase in zooplankton biomass or size. This absence of response in the pelagic food chain may have been attributed to the increase in abundance of YOY (0+) fish, following the fish reduction programme. Despite the lack of effect on zooplankton, there was a decrease in phytoplankton biomass, a change in species composition and an increase in water transparency following biomanipulation. In 1989, one year after the fish-kill in Eastern Basin, the Secchi depth (summer mean) increased from 60 cm to 110 cm. In the following years, water transparency increased further, despite an increase in phosphorus loading. An unexpected effect of the biomanipulation was an increase in benthic invertebrate and staging waterfowl abundances, which occurred 2–4 years after fish reduction. Hence, the response in the benthic community following biomanipulation was considerably stronger than in the pelagic community. A likely explanation is that reduction in abundance of the benthic feeding fish species bream (Abramis brama), strongly affected the benthic invertebrate fauna. In this paper, we present what we believe happened in Lake Ringsjön, and which processes are likely to have been important at various stages of the restoration process.

  • Moral internalism : An Essay in Moral Psychology

    • An ancient but central divide in moral philosophy concerns the nature of opinions about what is morally wrong or what our moral duties are. Some philosophers argue that moral motivation is internal to moral opinions: that moral opinions consist of motivational states such as desires or emotions. This has often been seen as a threat to the possibility of rational argument and justification in morals. Other philosophers argue that moral motivation is external to moral opinion: moral opinions should be seen as beliefs about moral reality, beliefs which may or may not motivate depending on whether the person holding them cares about moral matters.In this essay it is argued that although the traditional case for the internalist position fails, the total available evidence and methodological considerations support an internalist theory formulated in terms of a relatively rich psychological model. It is shown how such a theory can explain not only the practical character of moral opinions and their connection to moral emotions but also phenomena that have been taken to suggest an externalist picture, such as the role of inference, inconsistency, argument and explanations in moral discussion, as well as cases of amoralism and psychological disturbance. In the end, it is concluded that externalist explanations of the same phenomena are methodologically inferior for postulating a more complicated psychology.

    • This dissertation studies public institutional arrangements on the Swedish salt market 1720-1862. Crucial issues are how an why these arrangements emerged and were changed as well as they were used. The arrangements were erected in order to realize economic and social goals. In this respect, the policy persued by the Swedish government differed from those of most other governments, where policies concerning salt primarily meant tax policies. Government policies in Sweden were firstly external and had a long-term orientation. The aim was to import as much salt as possible to Sweden. The government also pursued a short-term policy in order to cope with short-term shorages. Overall, these policies were successful. Contrary to what has been stated in earlier Swedish research, the protectionist shipping policies did not lead to shortages of salt and high saltprices. Instead, the Swedish shipping and freight manufacture was stimulated, and Sweden became independent of foreign merchant fleets. The mercantilist aim of building a large merchant fleet, independent of potential enemy nations, was achieved without any negative effects on prices or supply on the Swedish salt market.Shortages of salt were not caused by a generally low yearly supply of salt. On the contrary, Sweden consistently had a surplus of salt and re-exported every year salt to other countries. The shortages of salt was rather regional and temporary, due to unexpected shocks on the supply side (wars, buccaneering, shipwrecks, persistent head winds, crises of production) and on the demand side (abundant fishing, cattle diseases and forced slaughter etc.). Costly and slow transports and communication meant that unexpected shortages could not be solved through trade. To solve these problems different institutional arrangements were created. These arrangements emerged in an evolutionary process of institutional change characterized by significant random elements. Severe shortages caused large changes in relative prices which acted as triggering factors in the process of institutional formation.Three more elaborated institutional arrangements having welfare purpuses were established, but disappeared in a rather short time. In 1774 a system of State Salt Stores were established in all staple towns. This arrangement proved to be very efficient as an insurance system, and the Salt Stores were frequently used to avert temporary shortages until new cargos of salt had arrived. The active state policy was a prerequisite for the markets to function satisfactory under mercantilism. But public stockpiling came to an end because the need for this insurance services diminished. But new technologies in saltproduction and shipping, increases in security and improved communications, a more efficiently-functioning market structure caused a significant long run decline i saltprices in relation to other prices and wages. Consequently, the need for public arrangements on the saltmarket decreased.

  • Ultrasound Doppler Vector Tomography - Measurements of Directional Blood Flow

    • An experimental system has been developed to verify the possibility of detecting flow activity using a technique called ultrasound Doppler vector tomography. A tomography algorithm is used to reconstruct blood flow vector fields using data from computer-controlled ultrasound continuous-wave Doppler scanning equipment. The result is a picture in which the brightness variations represent the reconstructed values of the curl of the velocity field (del x v). Continuous ultrasound is transmitted into a region with flow activity and the Doppler-shifted signals are received. To obtain measurement data suited for fan beam tomography, the scanning is performed in a plane from points encircling the region. Reconstructions have been achieved using measurement data from two different flow phantoms. A comparison between the experimental results and simulations shows good conformity.
