- Introduction:The dispensing of drugs in medical care systems is, in most countries, strictly regulated, and nurses are responsible for distributing drugs to in-patients.Aim:To describe the perceptions of patients with rheumatic diseases regarding traditional drug dispensing during in-patient care and rehabilitation in a specialized rheumatological care unit.Method:Twenty in-patients who stayed in the Spenshult Hospital unit for 3-4 weeks and who were on continuous medication were chosen for the study. The phenomenographic approach was used for the collection and analysis of data.Findings:Three descriptive categories emerged - Relief, Active Participation and Dependence. These descriptive categories comprised: three perceptions for Relief (to experience security, to be served, to dare to bother), two for Active Participation (to rely on one's own ability, to search for knowledge) and two for Dependence (lack of independence, lack of information).Conclusion:The patients experienced relief due to the nurse assuming responsibility for the medication and its dispensation. Patients expressed a wish to be more active in the management of their medication, as they trusted their own ability. The patients articulated that they were dependent on the nurse to give them the correct medication and they also asked for more information about their medication.

Retinoids inhibit differentiation of hematopoietic osteoclast progenitors.

- Whether vitamin A promotes skeletal fragility, has no effect on fracture rate, or protects against bone loss is unclear. In the present study, effects of retinoids on osteoclast differentiation in cultured mouse bone marrow cells (BMCs), bone marrow macrophages (BMMs), spleen cells, and RAW264.7 cells were evaluated by analyzing osteoclast formation and expression of genes important in signal transduction and osteoclast function. All-trans-retinoic acid (ATRA) did not stimulate osteoclastogenesis in BMCs, but inhibited hormone and RANKL-induced gene expression and formation of osteoclasts. In BMMs, spleen cells, and RAW264.7 cells, osteoclast differentiation and formation stimulated by M-CSF/RANKL were inhibited (IC(50) = 0.3 nM) by ATRA. The effect was exerted at an early step of RANKL-induced differentiation. ATRA also abolished increases of the transcription factors c-Fos and NFAT2 stimulated by RANKL and suppressed down-regulation of the antiosteoclastogenic transcription factor MafB. By comparing effects of several compounds structurally related to ATRA, as well as by using receptor antagonists, evaluation pointed to inhibition being mediated by RARalpha, with no involvement of PPARbeta/delta. The results suggest that activation of RARalpha by retinoids in myeloid hematopoietic precursor cells decreases osteoclast formation by altering expression of the transcription factors c-Fos, NFAT2, and MafB.

Towards a deeper understanding of gender barriers in the IT business

- In this paper we analyze barriers for women to enter and take on professional roles in the IT business. It is argued that earlier research concerning these barriers only has scratched on the surface and to be able to take appropriate meassures a deeper understanding of the foundation the barriers rests upon is needed. In this paper concepts from gender research, the so called four relation types: power, production, emotion, and symbols; are introduced and used in analyzing the barriers identified in earlier research as well as in case studies of IT professionals and youths at a IT high school. This analyzes shows that gender regimes in our society, when it comes to the IT business are, more or less, hostile to women. It also shows that using these four concepts adds a new dimension to the discussion concerning the under-representation of women in IT business. By focusing on relations, measures can be designed to target specific problematic relations instead of targeting a problem that is a complex of two or more relations.

- Endothelin-1 (ET-1) is a potent endothelium-derived vasoconstrictor, which also stimulates insulin release. The aim of the present study was to evaluate whether exogenously administered ET-1 affected pancreatic islet blood flow in vivo in rats and the islet arteriolar reactivity in vitro in mice. Furthermore, we aimed to determine the ET-receptor subtype that was involved in such responses. When applying a microsphere technique for measurements of islet blood perfusion in vivo, we found that ET-1 (5 nmol/kg) consistently and markedly decreased total pancreatic and especially islet blood flow, despite having only minor effects on blood pressure. Neither endothelin A (ET(A)) receptor (BQ-123) nor endothelin-B (ET(B)) receptor (BQ-788) antagonists, alone or in combination, could prevent this reduction in blood flow. To avoid confounding interactions in vivo, we also examined the arteriolar vascular reactivity in isolated, perfused mouse islets. In the latter preparation, we demonstrated a dose-dependent constriction in response to ET-1. Administration of BQ-123 prevented this, whereas BQ-788 induced a right shift in the response. In conclusion, the pancreatic islet vasculature is highly sensitive to exogenous ET-1, which mediates its effect mainly through ET(A) receptors.

Health-related quality of life in significant others of patients dying from lung cancer

- This study compares health-related quality of life (HRQOL) in significant others of patients dying from lung cancer, with a general population sample. Further, it explores the course of HRQOL from diagnosis (T1), at a time point close to the patient's death M), and six months after the patient's death (T3). The group comparisons at T1 showed that the significant others scored significantly lower on the scales in the mental domain compared with a general population sample. These results were the same at T3, when the significant others also scored lower on most of the scales in the physical and social domains. In the longitudinal analyses, there were significant changes in four scales, and three patterns of change were identified: a decrease-increase pattern for 'self-rated health' and 'positive affect'; a constant decrease pattern for 'family functioning'; and a decrease-stable pattern for 'satisfaction with family functioning'. Thus, living with inoperable lung cancer in the family and then facing the death of a family member affects most of the HRQOL dimensions.

Delineating diversity and specificity of an IgE-encoding transcriptome

- IgE is a minor component of the humoral immune response but one that is of extensive medical importance as it is a major mediator of type 1 hypersensitivity or allergy. Allergy affects up to 30% of the population in the industrialized countries, consequently affecting the quality of life of millions of people. In this study we sought to investigate the IgE repertoire and its evolution as it occurs in grass pollen allergic subjects. For this purpose, a human IgE library was constructed by combining the IgE heavy chain genes with kappa and lambda light chain genes, which were isolated from peripheral blood B-cells of an individual with timothy allergy. The library was screened, using phage display, against a panel of six different timothy allergens. This procedure allowed us to delineate the specificity of more than 25% of the IgE-producing transcripts in this allergic individual. Apart from providing valuable insights into the diversity and specificity of allergy-inducing repertoires, we have established a range of antibodies that can aid us in the quest to define ways how human IgE antibodies recognize grass pollen allergens, which in turn can provide important clues in the design of new allergy vaccines.
