  • Safety and Outcome of Revascularization Treatment in Patients With Acute Ischemic Stroke and COVID-19: The Global COVID-19 Stroke Registry.

    • COVID-19-related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19.This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT).Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16-2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20-2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23-1.99), 24-hour mortality (OR 2.47; 95% CI 1.58-3.86), and 3-month mortality (OR 1.88; 95% CI 1.52-2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26-1.60).Patients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non-COVID-19 patients receiving treatment. Current available data do not allow direct conclusions to be drawn on the effectiveness of revascularization treatments in patients with COVID-19 or to establish different treatment recommendations in this subgroup of patients with ischemic stroke. Our findings can be taken into consideration for treatment decisions, patient monitoring, and establishing prognosis.The study was registered under ClinicalTrials.gov identifier NCT04895462.

    • In 2015, the Rockefeller Foundation-Lancet Commission launched a report introducing a novel approach called Planetary Health and proposed a concept, a strategy and a course of action. To discuss the concept of Planetary Health in the context of Europe, a conference entitled: "Europe That Protects: Safeguarding Our Planet, Safeguarding Our Health" was held in Helsinki in December 2019. The conference participants concluded with a need for action to support Planetary Health during the 2020s. The Helsinki Declaration emphasizes the urgency to act as scientific evidence shows that human activities are causing climate change, biodiversity loss, land degradation, overuse of natural resources and pollution. They threaten the health and safety of human kind.Global, regional, national, local and individual initiatives are called for and multidisciplinary and multisectorial actions and measures are needed. A framework for an action plan is suggested that can be modified for local needs. Accordingly, a shift from fragmented approaches to policy and practice towards systematic actions will promote human health and health of the planet. Systems thinking will feed into conserving nature and biodiversity, and into halting climate change.The Planetary Health paradigm ‒ the health of human civilization and the state of natural systems on which it depends ‒ must become the driver for all policies.

  • Comparison of Position Estimation Methods for the Rotating Equatorial Microphone

    • We present a prototype of a microphone that moves rapidly along the equator of a rigid spherical scatterer. Our prototype allows for up to 100 rotations per second. It will enable processing methods like beamforming or sound field decomposition that are conventionally performed using microphone arrays. Solutions that assume one or more moving microphones have already been proposed in the literature but have not been verified in practise. Most of these methods require precise knowledge of the instantaneous microphone position, for which no convenient practical solution exists. Recent advancements in microphone array processing enable employing simple microphone trajectories, and recent advancements in 3D printing, microcontrollers, and high-speed electric motors allow for the required control of the movement. This paper presents the design of our prototype and evaluates its performance. Of particular interest is the accuracy of the estimation of the microphone's instantaneous position. This paper demonstrates that monitoring the passing time instants of a photodiode that is integrated into the rotating sphere provides the highest precision and robustness.

  • Direction of Arrival Estimation Using the Rotating Equatorial Microphone

    • Introduction: Direction of arrival (DOA) estimation of sound sources is an essential task of sound field analysis which typically requires two or more microphones. In this study, we present an algorithm that allows for DOA estimation using the previously designed Rotating Equatorial Microphone prototype, which is a single microphone that moves rapidly along a circular trajectory, introducing DOA-dependent periodic distortions in the captured signal. Methods: Our algorithm compensates for the induced spectral distortions caused by the REM’s circular motion for multiple DOA candidates. Subsequently, the best DOA candidate is identified using two distortion metrics. We verify our approach through numerical simulations and practical experiments conducted in a low-reverberant environment. Results: The proposed approach localizes unknown single-frequency sources with a mean absolute error of 23 degrees and unknown wideband sources with a mean absolute error of 5.4 degrees in practice. Two sources are also localizable provided they are sufficiently separated in space. Conclusion: Whilst previous work only allowed for DOA estimation of a single monochromatic sound source with a known frequency, our DOA estimation algorithm enables localization of unknown and arbitrary sources with a single moving microphone.

  • Occurrence of No-Harm Incidents and Adverse Events in Hospitalized Patients with Ischemic Stroke or TIA : A Cohort Study Using Trigger Tool Methodology

    • Adverse events (AEs)-healthcare caused events leading to patient harm or even death-are common in healthcare. Although it is a frequently investigated topic, systematic knowledge on this phenomenon in stroke patients is limited. To determine cumulative incidence of no-harm incidents and AEs, including their severity and preventability, a cohort study using trigger tool methodology for retrospective record review was designed. The study was carried out in a stroke center at a university hospital in the German speaking part of Switzerland. Electronic records from 150 randomly selected patient admissions for transient ischemic attack (TIA) or ischemic stroke, with or without acute recanalization therapy, were used. In total, 170 events (108 AEs and 62 no-harm incidents) were identified, affecting 83 patients (55.3%; 95% CI 47 to 63.4), corresponding to an event rate of 113 events/100 admissions or 142 events/1000 patient days. The three most frequent AEs were ischemic strokes (n = 12, 7.1%), urinary tract infections (n = 11, 6.5%) and phlebitis (n = 10, 5.9%). The most frequent no-harm incidents were medication events (n = 37, 21.8%). Preventability ranged from 12.5% for allergic reactions to 100% for medication events and pressure ulcers. Most of the events found (142; 83.5%; 95% CI 76.9 to 88.6) occurred throughout the whole stroke care. The remaining 28 events (16.5%; 95% CI 11.4 to 23.1) were detected during stroke care but were related to care outside the stroke pathway. Trigger tool methodology allows detection of AEs and no-harm incidents, showing a frequent occurrence of both event types in stroke and TIA patients. Further investigations into events' relationships with organizational systems and processes will be needed, first to achieve a better understanding of these events' underlying mechanisms and risk factors, then to determine efforts needed to improve patient safety.

    • Current sound-based practices and systems developed in both academia and industry point to convergent research trends that bring together the field of sound and music Computing with that of the Internet of Things. This article proposes a vision for the emerging field of the Internet of Sounds (IoS), which stems from such disciplines. The IoS relates to the network of Sound Things, i.e., devices capable of sensing, acquiring, processing, actuating, and exchanging data serving the purpose of communicating sound-related information. In the IoS paradigm, which merges under a unique umbrella the emerging fields of the Internet of Musical Things and the Internet of Audio Things, heterogeneous devices dedicated to musical and nonmusical tasks can interact and cooperate with one another and with other things connected to the Internet to facilitate sound-based services and applications that are globally available to the users. We survey the state-of-the-art in this space, discuss the technological and nontechnological challenges ahead of us and propose a comprehensive research agenda for the field.
