    • Detta är den nionde utgåvan av den samlade översikten över fisk- och kräftdjursbeståndens status i våra vatten. Kunskap om fiskbestånden och miljön är en förutsättning för att utnyttjandet av fiskresurserna skall bli bärkraftigt. För svenska vattenområden beskrivs miljöutvecklingen i ett ekosystemsperspektiv, dels för att tydliggöra fiskens ekologiska roll och beskriva yttre miljöfaktorer som påverkar fiskbestånden, dels för att belysa fiskets effekter på miljön.Fiskbestånd och miljö i hav och sötvatten är utarbetad av Sveriges lantbruksuniversitet (SLU), Institutionen för akvatiska resurser (SLU Aqua), på uppdrag av Havs- och vattenmyndigheten. Rapporten sammanfattar utveckling och beståndsstatus för de kommersiellt viktigaste fisk- och kräftdjursarterna i våra vatten. Bedömningar och förvaltningsråd är baserade på Internationella Havsforskningsrådets (ICES) rådgivning, SLU Aquas nationella och regionala provfiskedata, samt yrkesfiskets rapportering.

  • Sickness absence among cancer patients in the pre-diagnostic and the post-diagnostic phases of five common forms of cancer

    • Purpose The purpose of this study was to observe sickness absence before and after the cancer diagnosis among cancer patients with five common forms of cancer.Methods Using cohort data, we observed sick leave in the pre- and post-diagnostic phase among patients with colon, rectal, breast, prostate, or lung cancer (n = 2,738). We also identified reference subjects without cancer (total n = 12,246) who were individually matched for age and gender for each specific cancer cohort in order to compare sickness absence between patients with a specific form of cancer and the background population without cancer.Results Lung cancer patients had the highest increase in sick days both pre- and post-diagnosis and prostate cancer patients had the lowest increase. Irrespective of the form of cancer, cancer patients had significantly more sick days in the post-diagnostic phase compared to their reference subjects, ranging from 5 (prostate cancer) to 12 times the amount of sick days (colon and lung cancer). One year post-diagnosis, less than half of the cancer patients were on sick leave, except for lung cancer patients where 63% were still on sick leave.Conclusion Sick leave among cancer patients seems related not only to the cancer diagnosis and its treatment but also to the prodromal illness in the pre-diagnostic phase, especially for forms of cancer with heavier symptom burden such as colon and lung cancer. Although cancer results in substantial increase in sick leave, it is important to acknowledge that a major part of cancer patients return to work within 1 year after the cancer diagnosis.

    • In an article in this journal (2:2013) we identified several important shortcomings of War Studies at the Swedish National Defence College (SNDC) during the period of 2007–2012.In this article, we conclude that the authors of a responding article (4:2013), Senior Professors Stefan Axberg and Berndt Brehmer not only continue to focus on irrelevant aspects of asubject outside their expertise, but also neglect to recognize their own responsibility regarding the causes of the shortcomings. Furthermore, we find that their recommendations are incommensurable with the view presented by the Swedish Armed Forces and the current development of War Studies at the SNDC.

    • In the large majority of product development projects, computer-based design analyses are performed to assess the feasibility of potential technical solutions. As a first step to bring about a deeper understanding of the interactions between the engineering design and the design analysis activities, a survey has been performed in industry. The results of the survey cover: the use of design analysis within product development, the interactions of engineering design along the design analysis process, and the treatment of uncertainties and errors connected to the design analysis activities.

    • The Swedish Reference Group for Antibiotics (SRGA) has carried out a risk-benefit analysis of aminoglycoside treatment based on clinical efficacy, antibacterial spectrum, and synergistic effect with beta-lactam antibiotics, endotoxin release, toxicity, and side effects. In addition, SRGA has considered optimal dosage schedules and advice on serum concentration monitoring, with respect to variability in volume of drug distribution and renal clearance. SRGA recommends that aminoglycoside therapy should be considered in the following situations: (1) progressive severe sepsis and septic shock, in combination with broad-spectrum beta-lactam antibiotics, (2) sepsis without shock, in combination with broad-spectrum beta-lactam antibiotics if the infection is suspected to be caused by multi-resistant Gram-negative pathogens, (3) pyelonephritis, in combination with a beta-lactam or quinolone until culture and susceptibility results are obtained, or as monotherapy if a serious allergy to beta-lactam or quinolone antibiotics exists, (4) serious infections caused by multi-resistant Gram-negative bacteria when other alternatives are lacking, and (5) endocarditis caused by difficult-to-treat pathogens when monotherapy with beta-lactam antibiotics is not sufficient. Amikacin is generally more active against extended-spectrum beta-lactamase (ESBL)-producing and quinolone-resistant Escherichia coli than other aminoglycosides, making it a better option in cases of suspected infection caused by multidrug-resistant Enterobacteriaceae. Based on their resistance data, local drug committees should decide on the choice of first-line aminoglycoside. Unfortunately, aminoglycoside use is rarely followed up with audiometry, and in Sweden we currently have no systematic surveillance of adverse events after aminoglycoside treatment. We recommend routine assessment of adverse effects, including hearing loss and impairment of renal function, if possible at the start and after treatment with aminoglycosides, and that these data should be included in hospital patient safety surveillance and national quality registries.

    • Computer-based design analysis is nowadays a common activity in most development projects. Used for design evaluation, verification, validation, or as a support for design exploration, it fulfils an important support function for the engineering designer, thus making it essential to have an operationally efficient and effective integration between both the engineering design and design analysis activities in the overall development project. In this area, most works are focusing on software (mainly CAD/CAE) integra-tion, but not on the integration between computer-based design analysis and engineering design at the process level or on the collaboration between the engineering designer and the design analyst. This paper presents a review of the literature on that specific topic, namely the integration of the computer-based design analysis activity in the engineering design process. Different research topics are identified and elaborated upon: integration in general process models; rec-ommendations for the different analysis steps; analysis early in the engineering design process; integration of design analysis in the engineering designer's work; alternative usages of design analysis in the engineering design process; and others, such as recommending guidelines instead of process models, quality assurance aspects, education, and implementation issues. Some neglected aspects were also identified. Among others, there is a lack of research into the so-called technology development (development of design analysis procedures and guidelines), and a need for emphasis on uncertainties, both coupled with the design analysis activity.
