    • The main purpose of this dissertation has been to investigate how well diaconal work in seven chosen parishes corresponds to an official definition of the diaconal task of the Church of Sweden as above all to represent and direct activities towards the most disadvantaged and/or marginalised groups in society, a definition that well corresponds to the general expectations of the Swedish people regarding the role of the Church. The empirical ground for the dissertation comes from diaconal inventories built mainly on a comprehensive interview material with parish workers and appointed representatives in the chosen parishes. Diaconal work being carried out has been analysed and discussed in relation to the understanding of diaconal work put forward on the national level as mainly intended for those in greatest need. Analyse is carried out with a point of departure in secularization theory. The apparent dichotomy between public and private religion offer some explanation and enhance the understanding of how religious actors on different levels within the Church organisation have elected to confront the marginalised role of the Church. The most important conclusion generated by this dissertation is a clear lack of correspondence between the understanding of the deaconry as formulated on the national level and diaconal practice in the parishes studied. This discrepancy can arise because actors on the national and local levels have different strategies for counteracting the marginalisation. The gap between the national strategy emphasizing the task of the Church to represent the needy and a diaconal practice expressing quite another strategy represents a concrete and grave dilemma for the Church; a dilemma that in a not too distant future can have serious consequences for the Church’s credibility in the eyes of a distanced, nonreligious public.

  • Clinical characteristics and primary treatment of prostate cancer in Sweden between 1996 and 2005

    • OBJECTIVE: The incidence of prostate cancer is rising rapidly in Sweden and there is a need to better understand the pattern of diagnosis, tumor characteristics and treatment. MATERIAL AND METHODS: Between 1996 and 2005, all new cases of adenocarcinoma of the prostate gland were intended to be registered in the National Prostate Cancer Register (NPCR). This register contains information on diagnosing unit, date of diagnosis, cause of diagnosis, tumor grade, tumor stage according to the TNM classification in force, serum prostate-specific antigen (PSA) levels at diagnosis and primary treatment given within the first 6 months after diagnosis. RESULTS: In total, 72,028 patients were registered, comprising >97% of all pertinent incident cases of prostate cancer in the Swedish Cancer Register (SCR). During the study period there was a considerable decrease in median age at the time of diagnosis, a stage migration towards smaller tumors, a decrease in median serum PSA values at diagnosis, a decrease in the age-standardized incidence rate of men diagnosed with distant metastases or with a PSA level of > 100 ng/ml at diagnosis and an increase in the proportion of tumors with Gleason score <6. Relatively large geographical differences in the median age at diagnosis and the age-standardized incidence of cases with category T1c tumors were observed. Treatment with curative intent increased dramatically and treatment patterns varied according to geographical region. In men with localized tumors and a PSA level of <20 ng/ml at diagnosis, expectant treatment was more commonly used in those aged > or =75 years than in those aged <75 years. Also, the pattern of endocrine treatment varied in different parts of Sweden. CONCLUSIONS: All changes in the register seen over time are consistent with increased diagnostic activity, especially PSA testing, resulting in an increased number of cases with early disease, predominantly tumors in category T1c. The patterns of diagnosis and treatment of prostate cancer vary considerably in different parts of Sweden. The NPCR continues to be an important source for research, epidemiological surveillance of the incidence, diagnosis and treatment of prostate cancer.

    • Naturvårdsverket, Riksantikvarieämbetet och Fiskeriverket har fastställt denna nationella strategi för genomförande av skydd enligt delmål 1 Levande sjöar och vattendrag. Dokumentet är myndigheternas redovisning av åtgärdsprogram för särskilt värdefulla natur- och kulturmiljöer som behöver ett långsiktigt skydd, vilket i delmålet åläggs berörda myndigheter att ta fram.För att uppnå miljökvalitetsmålet Levande sjöar och vattendrag behöver särskilt värdefulla natur- och kulturmiljöer skyddas och hållbart förvaltas. Strategin är gemensam och tvärsektoriell och skall bidra till genomförande av skydd enligt delmålet. Ytterligare preciseringar samt vägledningar för tillämpning av styrmedel tas fram av respektive myndighet, medan planering, val av åtgärder och genomförande sker på regional och lokal nivå. Erfarenheterna och resultaten från arbetet med strategin utgör ett viktigt underlag för den fördjupade utvärderingen av miljökvalitetsmålet. Underlaget kommer att användas för att bedöma resurs- och åtgärdsbehov samt för att ge förslag till åtgärder som underlättar genomförandet på lång sikt.Strategin riktar sig till länsstyrelser, skogsstyrelser, kommuner, andra berörda myndigheter på central och regional nivå samt till infresseorganisationer, mark-, vatten- och fiskerättsägare.Arbetet med uppdraget har bedrivits gemensamt av Naturvårdsverket, Riksantikvarieämbetet och Fiskeriverket under medverkan av länsstyrelserna. I arbetsgruppen har från de centrala myndigheterna deltagit Björn Bergquist och Jonas Pettersson FiV, Daniel Nilsson RAÄ, Henrik Schreiber och Lena Tranvik(projektledare) NV.

  • Immunoaffinity Enrichments Followed by Mass Spectrometric Detection for Studying Global Protein Tyrosine Phosphorylation

    • Phosphorylation of protein tyrosine residues regulates important cell functions and is, when dysregulated, often crucially involved in oncogenesis. It is therefore important to develop and evaluate methods for identifying and studying tyrosine phosphorylated (P-Tyr) proteins. P-Tyr proteins are present at very low concentrations within cells, requiring highly selective enrichment methods to be detected. In this study, we applied immunoaffinity as enrichment step for P-Tyr proteins. Five selected anti-phosphotyrosine antibodies (monoclonal antibodies 4G10, PY100, PYKD1, 13F9 and one polyclonal antiserum) were evaluated with respect to their capability to enrich P-Tyr proteins from cell extracts of the K562 leukemia cell line. The enrichment resulted in the detection of a group of proteins that potentially were tyrosine-phosphorylated (putative P-Tyr proteins). High accuracy identification of actual P-Tyr sites were performed using a highly selective and sensitive liquid chromatography Fourier transform mass spectrometer (LC-FTMS) setup with complementary collision activated dissociation (CAD) and electron capture dissociation (ECD) fragmentations. 4G10 and PY100 antibodies recognized the greatest number of putative P-Tyr proteins in initial screening experiments and were therefore further evaluated and compared in immunoaffinity enrichment of both P-Tyr proteins and peptides. Using the 4G10 antibody for enrichment of proteins, we identified 459 putative P-Tyr proteins by MS. Out of these proteins, 12 were directly verified as P-Tyr proteins by MS analysis of the actual site. Using the PY100 antibody for enrichment of peptides, we detected 67 P-Tyr peptides (sites) and 89 putative P-Tyr proteins. Generally, enrichment at the peptide level made it difficult to reliably determine the identity of the proteins. In contrast, protein identification following immunoaffinity enrichment at the protein level gave greater sequence coverage and thus a higher confidence in the protein identification. By combining all available information, 40 proteins were identified as true P-Tyr proteins from the K562 cell line. In conclusion, this study showed that a combination of immunoaffinity enrichment using multiple antibodies of both intact and digested proteins in parallel experiments is required for best possible coverage of all possible P-Tyr proteins in a sample.

    • The split/splitless inlet system has basically the same fundamental drawbacks it had when it was introduced: poor repeatability of the injected amount of sample and discrimination of high-boiling analytes. Hot needle injection improves the repeatability of the sample transfer but suffers from in-needle discrimination. Injection with a fast autosampler, resulting in minimal heating of the needle, solves this problem but usually requires a glass wool packing in the inlet liner to assist in vaporization of the sample. As glass wool has been reported to cause degradation of labile analytes, it cannot be applied as a general remedy for improving incomplete vaporization. In this paper, a novel concept, based on electrostatic nebulization of the injected sample, is presented. The resulting fine droplets promote a more effective heat transfer and a rapid vaporization. Evaluation of the electrospray inlet in the split mode, using a straight, empty glass liner and a cold needle, showed an improvement in peak area repeatability by about 1 order of magnitude, compared with the results obtained when no electrostatic field was applied, Splitless injection of a series of hydrocarbons up to C-28 in the electrospray inlet with an empty, tapered liner, using a cold needle, showed no measurable analyte discrimination. The relative standard deviation in terms of area count for the largest hydrocarbon (C-28) was < 1.5%, compared to similar to 30% for injections where no high voltage was applied.

  • Inductively Coupled Plasma Spectrometry for Speciation Analysis : Development and Applications

    • In analytical chemistry the main goal is normally to determine the identity and/or concentration of one or more species in a sample. The samples analyzed are often natural samples, containing numerous different species in a complex matrix and the choice of technique for multi-elemental detection is in general inductively coupled plasma spectrometry. The chemical forms of an element can affect many of its characteristics e.g. toxicity, which makes speciation analysis important. Therefore, determination of the identity and quantity of an element is still important, but for many applications measurements of total element concentration provides insufficient information. To be able to perform speciation analysis, separation, identification and/or characterization of the various forms of elements in the sample has to be accomplished. Speciation analysis has been employed in a wide range of disciplines, including for example environmental science, biology and clinical chemistry. This thesis describes work to improve and understand the elemental speciation analysis with liquid chromatography coupled to plasma spectrometry and also highlights the importance and potential of the synergy between atomic spectrometry and molecular mass spectrometry. The combination of the matrix tolerant, robust and very sensitive plasma spectrometry used together with molecular mass spectrometry, which provides structural information and the possibility to identify unknown species, is demonstrated to be a very powerful tool for speciation analysis. In this thesis methods are developed for on-line sample clean-up and pre-concentration coupled to liquid chromatography and plasma spectrometry, which makes handling of small sample volumes easier and also decreases the risk of contamination. The problems associated with organic modifiers in plasma spectrometry are also addressed. Applications of speciation analysis are exemplified by analysis of aluminium-chelated siderophores in field-soil solutions and organic phosphorous species in aquatic sediments. The possibility to analyze un-dissolved samples as slurries with minimal sample preparation is also discussed.

    • Information about each X-ray examination, in a modern digitized X-ray department is generated and stored in a PACS. Appropriate conversion factors, e.g. E/DAP, can be applied to separate projections and summed to the total effective dose for each examination. The objectives of the work were (i) to investigate the accuracy and precision in the calculated effective dose (ii) to identify data for registration of (1) patient dose, (2) exposure data, and (3) patient information (iii) to make it possible to derive dose statistics on patient level for documentation of diagnostic standard doses, optimizations, constancy checks, and future epidemiological studies. The effective doses were calculated using Monte Carlo based computer programs or by using tabulations. Conversion factors were calculated for different levels of information and the individual effective dose was compared to the most precise estimation. The results suggest that the accuracy in the estimations of effective dose increases by added information about the patient (gender, size) and how the examination was performed.
