  • 15-Year Follow-up of Neuromuscular Function in Patients With Unilateral Nonreconstructed Anterior Cruciate Ligament Injury Initially Treated With Rehabilitation and Activity Modification: A Longitudinal Prospective Study.

    • Background: It has been suggested that neuromuscular function is of importance in the overall outcome after anterior cruciate ligament (ACL) injury. Hypothesis: Good neuromuscular function can be achieved and maintained over time in subjects with ACL injury treated with rehabilitation and activity modification but without reconstructive surgery. Study Design: Case series; Level of evidence, 4. Methods: One hundred consecutive patients (42 women and 58 men) with acute ACL injury at a nonprofessional, recreational or competitive activity level were assessed 1, 3, and 15 years after injury. Their mean age at inclusion was 26 years (range, 15-43 years). All patients initially underwent rehabilitation and were advised to modify their activity level, especially by avoiding contact sports. Patients with recurrent giving-way episodes or secondary meniscal injuries that required fixation were subsequently excluded and underwent reconstruction of the ACL. Sixty-seven patients (71% of those available for follow-up) with unilateral nonreconstructed injury remained at the 15-year follow-up. Fifty-six of these 67 patients were examined with the single-legged hop test for distance and knee muscle strength. The limb symmetry index (LSI), calculated by dividing the result for the injured leg by that of the uninjured leg and multiplying by 100, was used for comparisons over time (paired t test). Results: The LSI for the single-legged hop test was higher at the 3-year follow-up (mean, 98.5%; standard deviation [SD], 7.6%) than at the 15-year follow-up (mean, 94.8%; SD, 10.5%) (mean difference, -3.7%; 95% confidence interval [Cl], -6.1% to -1.2%; P = .004). The LSI for isometric extension was higher at the 15-year follow-up (mean, 97.2%; SD, 13.7%) than at the 1-year follow-up (mean, 88.2%; SD, 15.4%) (mean difference, 9.0%; 95% Cl, 3.7% to 14.4%; P = .001). At the 15-year follow-up, between 69% and 85% of the patients had an LSI >= 90%. Conclusions: Good functional performance and knee muscle strength can be achieved and maintained over time in the majority of patients with ACL injury treated with rehabilitation and early activity modification but without reconstructive surgery.

    • This paper investigates the introduction of and consequences of the “Vision Zero” (Nollvisionen) that was part of a law passed in 1997 and called for a radical reduction of deaths caused by road traffic. We try to answer the question why the Vision Zero has failed to reach its goals for the last ten years. The introduction of the Vision Zero can be explained with a combination of changes in traffic policy and welfare policy, together with a reorganization of the transport sector that created new opportunities for old interest groups. Our main conclusion is that the strong idealistic and visionary political goals in the Vision Zero are in line with a Swedish tradition of over-arching visionary national goals for the transport sector in general. Idealistic goals suppressed critical objections, but at the same time there were insufficient resources for investments and lack of approval for the policy from actors within the sector, which can create lock-in effects and actually prevent effective policies from being implemented. This might explain the relative ineffectiveness of the policy, and the slim chances of the Vision Zero to ever being achieved.

    • Dissolved Air Flotation, DAF, is an effective treatment process for drinking water and wastewater treatment. The advantages are a short start-up time, high hydraulic loading and more compact sludge. Air-saturated water is injected, under high pressure, into the flocculated main water stream. The decrease in pressure results in the formation of microscopic air bubbles. The flocs attach to the bubbles and rise to the surface where they form a foam layer. It is removed by scrapers at the surface and the clear water is let out at the bottom of the tank. The paper presents a two-dimensional one-phase model of a pilot DAF tank that has been developed with the CFD software FLUENT 6.3. The flow simulations are compared to experimental flow measurements. The results show the difficulties encountered when transforming a three-dimensional flow into a two-dimensional model. The design of the outlets influence the results and the effect of three-dimensional movements cannot be taken into account. In future studies the one-phase model will be extended to a two-phase model and considerations should be put on extending the two-dimensional model into three dimensions.

  • Enhanced external counter pulsation in treatment of refractory angina pectoris: two year outcome and baseline factors associated with treatment failure

    • Background: Enhanced external counter pulsation (EECP) is a non-invasive treatment option for patients with refractory angina pectoris ineligible to further traditional treatment. The aim of this study was to evaluate the effect of EECP on patients at a Scandinavian medical centre and to investigate if outcome can be predicted by analysing baseline factors. Methods: 86 consecutive patients (70 male, 16 female) were treated with EECP and followed for two years post treatment. Canadian cardiovascular society (CCS) class was analysed, and medication and adverse clinical events were researched prior to EECP, at the end of the treatment, and at six, 12 and 24 months thereafter. Patients responding to therapy by improving at least one CCS class were compared with those who failed to respond. Any differences in background factors were recorded and analysed. Results: 79% of the patients responded to therapy by improving at least one CCS class. In general, the CCS class improved by one class after EECP treatment (3.05 before versus 2.14 after treatment). A total of 61.5% of the initial responders showed sustained improvement at the 12 month follow-up while 29% presented sustained improvement after 24 months. Treatment was most effective among patients suffering from CCS class III-IV angina pectoris, while patients suffering from CCS class II angina pectoris improved transiently but failed to show sustained improvement after the 12 month follow- up. Diabetes mellitus and calcium channel antagonists were more common among the non-responders (p < 0.05). Conclusion: This study confirms the safety and efficiency of EECP as a treatment option for patients suffering from refractory angina pectoris. The therapy is most beneficial in patients suffering from severe angina (CCS III-IV) while sustained response to therapy could not be verified among patients suffering from CCS class II angina pectoris.
